I believe home help to be the closest fit in the USA as domiciliary care seems a bit technical. Other terms offerred were too closely allied with medical support, since hemtjänst personnel are neither nurses or doctors."

In the U.S. there are many different terms to describe this, depending on what state you live in, what kinds of qualifications (doctor, nurse, therapist, aid, etc.) the person coming to your home has, and what services they are providing.
A "visiting nurse" (a fully qualified Registered Nurse) is fairly common. In the old days doctors used to make "house calls" (this is extremely rare now). It is also quite common now to have a "nurse's aid" or "home health aid" (do not have as much training as nurses and thus do not provide all the services that an RN would, but are also more affordable). "In-home care" is also used to describe the field in a very general sense.
