Contextul este irelevant, am o înşiruire de afirmaţii mai mult sau mai puţin eliptice făcute de cardiologi despre nu ştiu ce medicament. Chiar nu ştiu ce medicament - în tot cazul, e unul pe care au reţineri în a-l prescrie, deoarece "necesită scrisoare medicală", zic ei. Înţeleg că scrisoarea medicală este răspunsul specialistului, ceea ce s-ar potrivi cu "reply letter":
