Karta zgonu – dokument wystawiany przez lekarza, konieczny do uzyskania aktu zgonu i pochowania zwłok, który musi być wydany dla każdej zmarłej osoby oraz dla dzieci martwo urodzonych, bez względu na wiek ciąży, na wniosek osób uprawnionych do ich pochowania.

The cause of death is confidential and appears only on the physician's copy of the document. The family can receive this copy through the physician or the establishment where the death was confirmed by the physician.
