Major depressive disorder frequently follows a recurrent or chronic course and significantly impairs quality of life.
For Ontario, an expert consultant estimated :
•~ 6-7% of the population experienced an episode of major depression the last year. Half of these people would have recurrent depression (the rest would have 1 or 2 lifetime episodes only). Of the half with recurrent depression, approximately 25% will fail at least 2 medications and be considered resistant.
Therefore, ~ 0.8% of the population (96,000 people per year if the population is 12 million) has had a treatment resistant depression in the past year.
Of these, 80% would not be considered for electroconvulsive therapy (ECT), the gold standard, due to :
-anesthetic contraindications
-nonavailability of ECT
-nonavailability of inpatient facilities
-request alternative antidepressant treatment
•~ 10% would be 1) eligible and 2) accept a course of treatment in a year.
Therefore ~ 1,800 patients may receive ECT in a year. It is estimated that approximately 10% of these patients may have also received ECT in the past, leaving approximately 1,500 new patients who may have received ECT in a year.
