CONSENTIMIENTO INFORMADO
1. Justificación del Estudio
La Poliomielitis es una enfermedad contagiosa, infecciosa, causada por un virus, que en la primera mitad del siglo XX fue muy frecuente entre las personas, afectando sobre todo a los niños a los que podía dejar secuelas severas irreversibles en los miembros inferiores del tipo de una parálisis y que provocan dificultades para caminar.
Desde que se desarrollaron las vacunas se han utilizado eficazmente para disminuir el impacto en la sociedad hasta tal punto que la erradicación de la enfermedad es un objetivo mundial y alcanzable en los próximos años. Esto implica el conocimiento de los niveles de anticuerpos (las defensas en la sangre) de las personas para establecer como debe ser el futuro en las vacunaciones contra esta enfermedad. Este tipo de evaluaciones se han realizado ya en otros países, entre ellos, Brasil, Italia, Holanda, España.
Debido a que Ud./su hijo, se realizará una extracción de sangre para realizarse estudios por otras razones que no tienen que ver con este estudio, en el caso de que acepte la participación/la de su hijo en el estudio, se le retirarán 2ml más de sangre (equivalente a una cucharadita de té) para un análisis que mide la presencia de Anticuerpos (células de defensa) contra el Virus Polio. Este examen que se agrega a los que a Ud/a su hijo se le ha indicado, no le insumirá costo.
Le recordamos que como en toda extracción, puede aparecerle un hematoma en el lugar de punción y no se deberá a la extracción adicional de sangre.
Se le solicitará el certificado de vacunación/ de su hijo o representado, con la finalidad de recolectar las fechas y conocer el estado de vacunación. Se realizará una copia de ese documento y de la ficha de control de su hijo/representado, que se conservará en el centro.
2. Libertad de participación
Usted tiene plena libertad para aceptar o rechazar la participación en este estudio. Esto no afectará su relación con el Médico investigador ni con el centro donde se realizará el estudio.
Tal como requiere la ley, Aventis Pasteur International ha contratado una póliza de seguros en caso de que cualquier sujeto presente un acontecimiento adverso directamente relacionado con el estudio.
3. Reglas de Confidencialidad
Las informaciones recogidas para el estudio serán anónimas como lo indican las normas vigentes (Secreto Profesional). El cuestionario será realizado por Médicos que forman parte del equipo de investigación y se han comprometido a respetar las normas y el secreto. Los datos recolectados serán tratados sin identificación por computadora para obtener una cifra global entre todos los participantes. Su resultado en particular le será entregado en su domicilio o en el centro donde se realizará la extracción una vez finalizado el estudio.
4. Principios de Etica
El presente estudio ha sido presentado, evaluado, y aprobado por la Dirección del Centro donde se realizará la extracción y por el Comité de Etica Independiente de la Facultad de Medicina, Universidad de la República, así como la comunicación al Ministerio de Salud Pública, previo a su inicio.
Consentimiento de participación
Me han propuesto participar de un estudio para saber la cantidad de anticuerpos contra el virus de polio en sangre. He leído y comprendido las informaciones entregadas y he recibido del técnico una respuesta satisfactoria frente a todas las preguntas. INFORMED CONSENT
1. Reason for the study
Poliomyelitis is a contagious, infectious disease caused by a virus, which in the first half of the 20th century was frequently observed in persons, above all affecting children, who could be left with severe and irreversible effects in the lower limbs of a paralytic nature and resulting in difficulty walking.
Since their development, vaccines have been used to reduce the social impact of the disease to such an extent that the worldwide eradication of the disease in the near future is an achievable objective. This involves determining the levels of antibodies (the defence mechanisms carried in the blood) of people in order to establish the future vaccines against this disease. This type of evaluation has already been carried out in other countries, including Brazil, Italy, Holland and Spain.
As you/your child are due to have blood extracted for other tests unrelated to this study, if you agree to your/your child's participation, an additional 2 ml of blood (equivalent to one teaspoon) shall be taken for an analysis that measures the presence of antibodies (defence cells) against the Polio Virus. This test, which shall be added to those indicated for you/your child, shall be free of charge.
May we remind you that, as with all blood tests, bruising may appear around the injection site but this is not related to the additional blood extracted.
A certificate of vaccination will be requested for you/your child (or person represented), with the aim of recording dates and establishing vaccination status. A copy of said document and the patient card pertaining to your child/the person represented shall be made and kept at the health centre.
2. Freedom to participate
You are completely free to accept or reject participation in this study. Your choice will not affect your relationship with the medical investigator or with the centre at which the study is being conducted.
As required by law, Aventis Pasteur International has contracted an insurance policy to provide cover in the event that a subject should present an adverse event directly related to the study.
3. Confidentiality Policy
All information collected for the study shall be anonymous pursuant to current regulations (Professional Secrecy). The questionnaire shall be given by doctors who form part of the research team and who have agreed to respect the regulations and secrecy. All data collected shall be processed, without identification, by computer with the aim of obtaining a global figure from all participants. Your particular result shall be delivered to your address or to the centre at which the blood was extracted once the study is completed.
4. Ethical Principles
This study has been presented, evaluated, and approved by the Administration of the Health Centre at which the extraction was made and by the Independent Ethical Committee of the Faculty of Medicine, Universidad de la República. The Ministry of Public Health shall be informed prior to its commencement.

Introducción
La rotación de opioides se ha propuesto como eficaz en el tratamiento del dolor mejorando la eficacia del primer opioide o la tolerancia a los efectos adversos.
Material y métodos:
Se estudian 50 pacientes de dolor crónico no oncológico con tratamiento opioide que no tiene efecto analgésico suficiente (56%) o que es intolerado, y se rota a oxicodona, valorando la respuesta analgésica mediante la EVA y anotándose lo efectos adversos (mareo, somnolencia, náuseas, sequedad de boca y estreñimiento).

Introduction

PROTECCIÓN DE LOS SUJETOS DE LA INVESTIGACIÓN
Artículo 3. Postulados éticos.
1. Sólo se podrá iniciar una investigación clínica cuando el Comité Ético de Investigación Clínica (CEIC) y la Agencia Española de Medicamentos y Productos Sanitarios hayan considerado que los beneficios esperados para el sujeto de la investigación y para la sociedad justifican los riesgos; asimismo, sólo podrá proseguir si se supervisa permanentemente el cumplimiento de este criterio.
2. Las investigaciones clínicas se realizarán en condiciones de respeto a los derechos del sujeto y a los postulados éticos que afectan a la investigación biomédica con seres humanos. En particular, se deberá salvaguardar la integridad física y mental del sujeto, así como su intimidad la protección de sus datos, de acuerdo con la Ley Orgánica 15/1999, de 13 de diciembre, de Protección de Datos de Carácter Personal. Se obtendrá y documentará el consentimiento informado de cada uno de los sujetos de la investigación, libremente expresado, antes de su inclusión en la investigación en los términos previstos en el artículo 7 del Real Decreto 223/2004.
3. Sólo se podrán realizar investigaciones clínicas cuando se cumplan todos los requisitos siguientes:
a) Disponer de suficientes datos científicos y, en particular, ensayos preclínicos en animales, que garanticen que los riesgos que implica en la persona en que se realiza la investigación son admisibles.
b) Que el estudio se base en los conocimientos disponibles, la información buscada suponga, presumiblemente, un avance en el conocimiento científico sobre el ser humano o para mejorar su estado de salud y su diseño minimice los riesgos para los sujetos participantes en ella.
c) Que los riesgos e inconvenientes previsibles para los sujetos de la investigación se hayan ponderado con respecto a los beneficios previsibles para cada sujeto de la investigación y futuros pacientes.
4. Con el fin de garantizar una protección óptima de la salud y los derechos de los sujetos, no se podrán llevar a cabo investigaciones obsoletas o repetitivas.
5. La investigación clínica debe estar diseñada para reducir al mínimo posible el dolor, la incomodidad, el miedo y cualquier otro riesgo previsible en relación con la enfermedad y edad o grado de desarrollo del sujeto; tanto el umbral de riesgo como el grado de incomodidad deben ser definidos de forma específica y monitorizados durante la investigación, especialmente cuando los sujetos de la investigación sean menores, adultos incapacitados o constituyan una población especialmente vulnerable en razón de su situación económica, médica o social.
6. El tratamiento, comunicación y cesión de los datos de carácter personal de los sujetos participantes en la investigación se ajustará a lo dispuesto en la Ley Orgánica 15/1999, de 13 de diciembre, de Protección de Datos de Carácter Personal, y constará expresamente en el consentimiento informado.
7. La atención sanitaria que se dispense y las decisiones médicas que se adopten sobre los sujetos serán responsabilidad de un médico debidamente cualificado o, en su caso, de un odontólogo cualificado, o bien de otro profesional adecuadamente cualificado.
8.Los sujetos participantes en investigaciones clínicas sin beneficio potencial directo para el sujeto en investigación recibirán del promotor la compensación pactada por las molestias sufridas. La cuantía de la compensación económica estará en relación con las características de la investigación, pero en ningún caso será tan elevada como para inducir a un sujeto a participar por motivos distintos del interés por el avance científico.
9. La contraprestación que se hubiere pactado por el sometimiento voluntario a la investigación se percibirá en todo caso, si bien se reducirá equitativamente según la participación del sujeto en la experimentación, en el supuesto de que decida revocar su consentimiento y abandonar la investigación.
10. En los casos extraordinarios de investigaciones sin beneficio potencial directo para el sujeto en investigación en menores e incapaces, para evitar la posible explotación de estos sujetos, no se producirá ninguna compensación económica por parte del promotor, a excepción del reintegro de los gastos extraordinarios y pérdidas de productividad que se deriven de la participación del sujeto en la investigación.
11. Los sujetos que participen en investigaciones con un posible beneficio potencial directo para el sujeto de investigación o sus representantes legales únicamente podrán recibir del promotor el reintegro de los gastos extraordinarios pérdidas de productividad que se deriven de su participación en la investigación.
12. Los sujetos de la investigación dispondrán de un punto donde puedan obtener mayor información sobre la investigación, que constará en la hoja de información para el sujeto.

6. The treatment, communication, and cession of the personal data pertaining to the trial subjects shall comply with the provisions of Organic Law 15/1999, of December 13 on the Protection of Personal Data, and shall be expressly stated on the informed consent.
7. The healthcare dispensed and the medical decisions made regarding the subjects shall be the responsibility of a duly qualified doctor, and where appropriate, a qualified odontologist, or another appropriately qualified professional.
Subjects participating in clinical trials with no potential direct benefit for the subject shall receive the agreed compensation for the inconvenience suffered from the sponsor. The amount of compensation shall be related to the characteristics of the trial, but under no circumstance shall it be so high as to induce the subject to participate for reasons other than scientific advancement.
9. The payment previously agreed for voluntary subjection to the shall be paid in all cases, although the sum shall be fairly adjusted according to the subject’s participation in the experiment, in the event that he/she decides to withdraw their consent and abandon the trial.
10. In the rare cases of with no potential direct benefit for the subject in clinical trials on minors and incapacitated adults, in order to avoid the possible exploitation of these subjects, no financial compensation shall be given by the sponsor, other than the reimbursement of extraordinary costs and loss of productivity derived from the subject’s participation in the .
11. Subjects who participate in trials with possible direct benefit for the trial subject or his/her legal representatives may only receive from the sponsor the reimbursement of extraordinary costs and loss of productivity derived from the subject’s participation in the trial.
12. Trial subjects shall be provided with a contact point where he/she may obtain further information.

Cuatro años antes de que James Cook llegara a las costas de la Columbia Británica (Canadá), el mallorquín Joan Pérez, piloto de la Marina Real española, arribaba a la bahía de Nootka. Corría el año 1774.
No era misionero, pero merece figurar en este apartado por haber acompaûado a los frailes franciscanos en sus m£ltiples desplazamientos por las costas de Baja y Alta California.
Pérez había nacido en Palma de Mallorca en fecha que se ignora, pero sabemos que empezó a adquirir experiencia en el Pacífico surcando la ruta del galeón de Manila, entre Acapulco y las Filipinas.
En 1769, como alférez de fragata, dirigía la nave San Antonio, con el objetivo de establecer los primeros asentamientos en la costa de Alta California, siguiendo las directrices del Gobernador, Gaspar de Portolà. Participó, pues, en el descubrimiento y posterior asentamiento de los puertos de San Diego y Monterey, donde tendría la oportunidad de iniciar con Serra y Crespí una relación que se mantendría con los años.
En 1774, a fin de comprobar hasta dónde habían llegado los rusos en la zona septentrional del litoral del Pacífico y con la orden de tomar posesión de esas tierras en nombre del rey Carlos III, a bordo de la nave Santiago, Pérez descubre la desembocadura del río Columbia y alcanza los 55º latitud norte. En este viaje exploratorio le acompañaría Joan Crespí quien se dedicaría a ir anotando en su diario los hallazgos y los hechos acaecidos en esta nueva expedición.
En esta travesía llegaron al Estrecho de Nootka (Vancouver), bautizando su bahía con el nombre de San Lorenzo y tomaron posesión para la Corona Española de la isla que veían frente a ellos, a la que llamaron de Santa Margarita y que hoy forma parte del archipiélago de la reina Carlota.
Los nativos que rodeaban la corbeta Santiago con sus canoas entre cánticos de bienvenida y tocando instrumentos de madera, realizarían entonces los primeros intercambios con el hombre blanco.
Cuenta la leyenda que entre los obsequios canjeados, algunos dicen que robados, figuraron las dos cucharas de plata que el Capitán Cook, en su viaje alrededor del mundo, acabó comprando en estas costas a un nativo que las lucía, colgadas de su cuello.
En un momento dado, a causa de las fuertes tormentas que azotaban la zona, Pérez hubo de dar la orden de virar hacia el Sur. El mal tiempo le impidió, así mismo, entrar en la Bahía de San Francisco por lo que el comandante tuvo que anclar en Monterrey.
El 2 de Noviembre de 1775, cuando finalmente la Santiago iniciaba el regreso hacia San Blas (Baja California) moría Pérez en alta mar, después de haber servido a la marina durante más de 25 años.
Su cuerpo fue entregado al océano en la bahía de Monterrey.
En 1999 la Royal Canadian Mint hizo acuñar un dólar de plata con la representación de la nave Santiago, en conmemoración del 225 aniversario de esta travesía.
Las notas de este apasionante periplo están recogidas en su diario de viaje: Extracto del Diario de la Corbeta del rey nombrada Santiago, en su viaje desde el puerto de San Blas de California 1774 al reconocimiento de la costa hasta los 55 grados norte.
Este diario se encuentra en el Museo Naval de Madrid, y en la biblioteca del Congreso de Washington D.C. se conserva el primer plano -hallado en 1988- que por orden de Pérez se dibujó de las costas de la Columbia Británica, antes de ser territorio canadiense.

Four years before Captain James Cook reached the coast of British Columbia (Canada), the Majorcan born pilot of the Royal Spanish Navy, Joan Perez, had found the Bay of Nootka. The year was 1774.
He was not a missionary, but deserves a mention in the section to due to the fact that he accompanied the Franciscan missionaries on their numerous journeys along the coasts of Upper and Lower California.
Perez was born in Palma de Mallorca, although his exact date of birth is unknown. However, we do know that he began to gain experience in the Pacific, sailing the Manila galleon route, between Acapulco and the Philippines.
In 1769, with the rank of ensign, he commanded the San Antonio with the objective of establishing the first settlements on the coast of Upper California, according to the orders given by the Governor, Gaspar de Portola. He thus participated in the discovery and establishment of the ports of San Diego and Monterey, where he would have the chance to begin a long lasting relationship with Serra and Crispi.
In 1774, with the aim of determining how far the Russians had reached along the northern part of the Pacific coastline, and with the order to take possession of this territory in the name of Charles III, Perez, aboard the Santiago, discovered the estuary of the Columbia River and reached 55º north. On this exploratory voyage, he was accompanied by Joan Crispi, who spent his time making entries in his journal regarding the discoveries made and events that took place on the expedition.
The voyage took them to the Strait of Nootka (Vancouver), where they baptized its bay with the name of Saint Lawrence and took possession of the island they saw before them, in the name of the Spanish Crown. They named the island Saint Margaret and today, it form part of the Queen Charlotte (Carlota) Islands.
The natives that surrounded the corvette Santiago in their canoes, with their welcome chants and music played on wooden instruments, made the first contact with a white man.
The legend goes that among the gifts exchanged, some say stolen, were the two silver spoons that Captain Cook, on his voyage around the world, would buy from a native of these coasts, who wore them hanging from his neck.
When strong storms started to hit the region, Perez had to give the order to go about and head south. The bad weather stopped him from entering the Bay of San Francisco, so he had to drop anchor in Monterrey.
On the 2nd of November 1775, when the Santiago was finally on the return voyage to San Blas (Lower California), Perez died. He had served in the navy for more than 25 years.
He was buried at sea in the Bay of Monterrey.
In 1999, the Royal Canadian Mint issued a silver dollar carrying a depiction of the Santiago, in commemoration of the 225th anniversary of the voyage.
The notes made on this fantastic journey appear in his log: Extract from the Log of the King’s Corvette, Santiago, on its voyage from the port of San Blas, California, in 1774, on a reconnaissance mission up to 55 degrees north.

I am a translator/editor with 20 years experience. I originally entered translation through a post-graduate research project in the field of epidemiology in which I was involved.
Although my first degree is in psychology, my final degree project was in psycho-linguistics and language acquisition. A subsequent masters degree in film and television provided me with an understanding of semiotics, structuralism and language in a broader context, while allowing me to further explore my interest in socio-linguistics.
When I finally decided to become a full-time translator, I studied for and was awarded the Institute of Linguists Diploma in Translation.
As a translator I am firmly committed to all initiatives involving the consolidation of terminology within the European Union and between the EU and USA.
I am also committed to remaining informed on translation issues, especially within the context of the European Union. Furthermore, my experience with creative writing (magazine articles and advertising copy), although sporadic, has contributed greatly to the readability of my translations, a factor that I believe to be an asset. Although the last five years have seen me mainly working in the fields of medical research and pharmaceutical development work, I have also worked for several artists in the translation of multi-lingual exhibition catalogs, reviews, critiques, etc, which has taught me how to "transpose" an authorial voice into the target language, where required.
Living as I do, between Spain and the UK, I am able to keep abreast of and assimilate linguistic trends, thus each project I undertake is informed by an up to date and relevant social/linguistic awareness with respect to the three languages within which I work.
