As part of your application for a visa to enter the United States, you are required to undergo a medical examination. The following physicians have received authorization from this Embassy to conduct official medical examinations of visa applicants. Medical examinations from any other physician are not acceptable for visa application purposes. The fees listed below are for the medical examination only. You may be required to pay a separate fee for immunizations.

Medical Examination Forms (DS-2054, DS-3025, DS-3026, and DS-3030) will be provided by the hospital, as they have to be handled by the panel physician.
IMPORTANT NOTICE TO IMMIGRANT VISA APPLICANTS CONCERNING VACCINATION REQUIREMENTS
United States immigration law requires immigrant visa applicants to obtain certain vaccinations (listed below) prior to the issuance of a visa. Panel physicians who conduct medical examinations are now required to verify that visa applicants have met the new vaccination requirements, or that it is medically inappropriate for the visa applicant to receive one or more of the listed vaccinations:
Hepatitis A Hepatitis B Influenza Influenza type B (Hib) Measles Meningococcal Mumps Pneumococcal Pertussis Polio Rotavirus Rubella Tetanus and diphtheria toxoids Varicella
In order to assist the panel physician, and to avoid processing delays, all immigrant visa applicants should have their vaccination records available for the panel physician's review at the time of the medical examination. You should consult with your regular health care provider to obtain a copy of your immunization record, if one is available. If you do not have a vaccination record, the panel physician will work with you to determine which vaccinations you may need to meet the requirements. Certain waivers of the vaccination requirements are available upon the recommendation of the panel physician.
Only a physician can determine which of the listed vaccinations are medically appropriate for you, given your age, medical history, and current medical condition.

Paracetamol Injection 300 mg/2ml
For I.M / I.V Use
COMPOSITION
Each ml contains
Paracetamol(Acetaminophen)........................................150 mg
INDICATIONS
Pyrexia of unknown origin, fever and pain, associated with common childhood disorders, tonsilitis, upper respiratory tract infection, post-immunization reactions, post operative fever, after tonsillectomy and other conditions, where patient is unable to take oral medications but where Paracetamol can be administered with advantage for prevention of febrile convulsion, headache, cold, sinusitis, muscle pain, arthritis and toothache.
NOTE: Specific therapy with antibiotics or chemotherapeutic drugs of choice should be carried out whenever indicated.
DOSAGE AND ADMINISTRATION
Intramuscular route: Adults: 2 - 3 ml every 4 to 6 hours.
Children (2 -12 years / > 33 kg): Up to 2 ml every 4 to 6 hours.
Below 2 years of age: Half to 1 ml every 4 to 6 hours.
Intravenous route: Slow I.V Administration.
CONTRAINDICATIONS
Hypersensitivity to Paracetamol. Repeated administration is contraindicated in patients with anemia, cardiac, pulmonary, renal, and hepatic disease.
WARNINGS AND PRECAUTIONS
Contains sodium sulfite, a sulfite that may cause allergic-type reactions including anaphylactic symptoms and life threatening or less severe asthamatic episodes in certain susceptible persons. Also contains Benzyl Alcohol. This should not be administered to new born or premature infants. Paracetamol should be given with care to patients with impaired kidney or liver function.
ADVERSE REACTIONS
Paracetamol has rarely been found to produce any side effects in therapeutic doses and is usually well tolerated by aspirin sensitive patients. Toxicity may result from a single toxic dose of the drug or from chronic ingestion. The following adverse reactions have been reported: skin eruption, haematological toxicity.
INTERACTIONS WITH OTHER MEDICINES
Paracetamol may enhance the activity of coumarin anticoagulants, but its effect is not generally of clinical significance.
OVERDOSE AND TREATMENT
Symptoms of overdosage may include nausea, vomiting, abdominal pain, diaphoresis, generalized weakness & lethargy. If an overdose of Paracetamol is suspected, blood should be withdrawn immediately for Paracetamol plasma assay, without regard to the presence or absence of symptomatology. The acute hepatotoxicity, nephrotoxicity of paracetamol can be overcome by the administration of sulfinydryl donors, e.g, N-acetyl cysteine which should be given as soon as possible after ingestion. Treatment after 12 hours is not effective. Paracetamol overdose should be treated with gastric lavage if the patient is seen within 24 hours of ingestion of the drug.
STORAGE CONDITIONS
Store in a cool, dry place below 25oC. Protect from light and moisture.
Keep out of reach of children.

Prescription-only medicine. To be used by or under the supervision of a medical practitioner only.
Do not take any Paracetamol containing medicines concurrently.

REMARQUE : Un traitement spécifique avec des antibiotiques ou des médicaments chimio-thérapeutiques de choix doit être effectué chaque fois que cela est indiquée.
POSOLOGIE ET ADMINISTRATION
Voie intramusculaire : Adultes : 2 à 3 ml toutes les 4 à 6 heures.
Enfants (2 à 12 ans / >33 kg) : Jusqu’à 2 ml toutes les 4 à 6 heures.
Moins de 2 ans : de la moitié jusqu’à 1 ml toutes les 4 à 6 heures.

CONTRE-INDICATIONS
Hypersensibilité au paracétamol. L’administration répétée est contre-indiquée chez les patients souffrant d’anémie, cardiaque, pulmonaire, rénale, et hépatique.
MISES EN GARDE ET PRÉCAUTIONS
Contient du sulfite de sodium, un sulfite qui peut causer des réactions de type allergiques, y compris des symptômes anaphylactiques et des épisodes asthmatiques potentiellement mortels ou moins graves chez certaines personnes sensibles. Contient également de l’alcool de Benzyle. Ceci ne doit pas être administré aux nouveau-nés ou aux prématurés. Le paracétamol doit être administré avec soin aux patients ayant une altération des reins ou de la fonction hépatique.
EFFETS INDESIRABLES
Le paracétamol s'est rarement avéré produire des effets secondaires à des doses thérapeutiques et est généralement bien toléré par les patients sensibles à l'aspirine. La toxicité peut résulter d’une seule dose toxique du médicament ou d’une ingestion chronique. Les effets indésirables suivants ont été signalées : éruption cutanée, toxicité hématologique.
INTERACTIONS AVEC D’AUTRES MÉDICAMENTS
Le paracétamol peut améliorer l’activité des anticoagulants coumarines, mais son effet n’est généralement pas d’importance clinique.
SURDOSAGE ET TRAITEMENT
Les symptômes d’un surdosage peuvent inclure des nausées, des vomissements, des douleurs abdominales, de la diaphorèse, une faiblesse généralisée et une léthargie. Si une surdose de paracétamol est suspectée, le sang doit immédiatement être prélever pour l’essai de plasma de paracétamol, sans égard à la présence ou à l’absence de symptomatologie. L’hépatotoxicité aiguë, la néphrotoxicité du paracétamol peuvent être surmontées par l’administration des donneurs de sulfhydrile, par exemple, la N-acétylcystéine qui doit être administrée dès que possible après l’ingestion. Le traitement après 12 heures n’est pas efficace. Le surdosage de paracétamol doit être traité par le lavage gastrique si le patient est vu dans les 24 heures suivant l’ingestion du médicament.

Core Certification for Healthcare Interpreters at (CCHI), and now I am a
Certified Healthcare/ Medical Interpreter. As a Translator and Certified
medical Interpreter, I am specialized in technical, legal, literary, and medical
translation. I also perform general translation regularly with consecutive and
simultaneous Interpreting services provided to individuals and several
institutions/organizations in different settings. I have traveled the world to
learn languages, new cultures, and honed my skills, absorbing as much knowledge
as possible. I always make sure to get to a specific country when I want to learn
