Source text - English In this chapter I relate one man's story, as it emerged in the course of his meetings with me, following referral to the Trauma Unit by his GP. The consultation as a whole consisted of four sessions, spaced either two or three weeks apart, covering a period of three months. Each session was one and a half hours long, apart from the first which was deliberately slightly longer. Most individuals referred to the Unit are offered a four session consultation as an initial intervention. In this chapter 1 shall be giving a picture of this kind of consultation. The case I have chosen is one in which the consultation seemed to have some beneficial impact. Of course not all the cases we see are quite like this; some patients are more broken down, inarticulate, do not attend, act out and so on. However, essentially what we are trying to do in every consultation remains the same and it is this that I want to describe.
The experience of being alive is an ambivalent one, a mixture of good and bad. By and large most people manage the difficulties, drawing on the kinds of support normally available to them - their social and family networks, and their own internal resources. Our consultation is offered only when the individual requests it, perhaps via a professional referrer, rather than as a standard intervention following a traumatic event; not everybody will need professional help. When someone comes to us it is because their capacity to manage has broken down. Some are able to re-establish their capacity to cope with a time-limited intervention because it is possible for the therapist to mobilise the individual's oum resources, which previously sustained them, so that they can then function independently. Others, perhaps with less in the way of pre-existing internal and external resources, or who have experienced particularly devastating external events, need longer-term help.
It is because it is overwhelming and incapacitating, and cannot be managed psychically, that an event can be defined as traumatic. Flexible and creative thought is replaced by nightmares, flashbacks and at times unconsciously driven re-enactments of the event. The experience cannot, in the usual way, be contained in the mind through thought. (In Bion's terms (1967) there is a breakdown in the capacity to transform beta into alpha elements). It is possible to conceptualise the impact also in terms of the psychoanalytic model of an internal world of object relationships. Melanie Klein (1929a, 1929b,1945) described the mind as an internal world populated from the beginning of life, via primitive unconscious phantasies, with figures she called internal objects. These were variously deemed to be benign and helpful or malevolent and dangerous. In Klein's descriptions all events are, in phantasy, felt to be caused either by good or bad objects, dependent on the nature of the event. One can think then that the internal experience of a traumatic event is an experience of being abandoned by loving good internal objects which protect and contain, and instead left to the mercy of hateful and hating objects, felt to have caused the trauma. This is especially the case when there has been a real external disaster, since it is to be protected from these extreme experiences, involving the death of someone and/or the possibility of dying oneself, that one most expects from one's good object. If the attempt in the consultation to make contact is successful, then the individual is provided with an experience of being understood, of a good experience of containment. In this way, the therapist may help, within the confines of a brief intervention, to begin to reestablish an internal good object, necessary for healthy psychic functioning.

Консультация целиком состояла из четырех сессий, отстоящих друг от друга на две или три недели, общая продолжительность составила три месяца. Каждая сессия продолжалась полтора часа, за исключением первой, которая продолжалась несколько дольше. Большинству направляемых в Отдел пациентов предлагается консультация из четырех сессий в качестве первоначальной интервенции. В этой главе я расскажу о консультациях такого типа. В случае, который я выбрала для описания, консультация имела видимый благотворный эффект. Разумеется, не все случаи, которые мы видим, похожи на этот; у некоторых пациентов больше трудностей, они не в состоянии выражать свои мысли, не приходят на сессии, занимаются отыгрыванием и так далее. Когда пациент приходит к нам, это означает, что его способность справляться сломана. Некоторые люди способны ее восстановить с помощью ограниченной во времени интервенции, потому что терапевт смог мобилизовать их собственные ресурсы, которые поддерживали их раньше, и они могут функционировать независимо. Другие, у которых, возможно, изначально было меньше внутренних и внешних ресурсов, или которые пережили особенно разрушительные травмирующие события, нуждаются в долгосрочной помощи.

Высокие и сдержанные, с чистыми и белыми, как у четырнадцатилетних, руками, они улыбались в ответ на наши вопросы спокойной и мудрой улыбкой, и возвращали разговор к тебе, к тому, что происходит с тобой, - а то, что этой весной происходило с нами, было серым и влажным, липким и густым, и нам совершенно не хотелось об этом говорить, так что мы не говорили, а они не настаивали. Сабина первая сказала им «да» и в тот же вечер вернулась домой со спокойной и мудрой улыбкой, и я помню, как мы стояли вокруг нее, молчали и смотрели на ее чистые мягкие руки, тринадцатилетние руки, и в левой руке у нее был значок такого цвета, какого мы никогда не видели раньше, а в правой, скрюченной и сухой, руке у нее не было ничего. Они давали такой значок только тем, кто вступил в ряды в первый день, и не требовалось быть особо одаренным ребенком, чтобы понять, в чем тут логика. На следующий день Даня (вступивший в ряды в самый первый день – Даня всегда был умнее нас, за это мы и били его как минимум раз в неделю) пошел к этим людям и сказaл им, что он должен получить ответы хотя бы на несколько вопросов – для себя и для своего отца-алкоголика, и Даню повесили на ближайшей яблоне, и не надо требовалось быть особо одаренным ребенком, чтобы понять, что они были правы. Они приходили к нам каждый вечер, по двое в каждый двор, обходили наш задыхающийся городок двор за двором, говорили с нами мягким и глубокими голосами, и обещали нам общее будущее, и нам было незачем задавать вопросы, и те, кто вступил, сидели прямо перед этими людьми, смотрели в их внимательные лица, а те, кто не вступил, стояли у нас за спиной и слушали, и потом подходили к этим людям, к людям нашей весны, и говорили тяжелые и короткие слова клятвы, и получали значки простого и даже дурацкого цвета, но они сжимали их в кулаках со смирением и пониманием. They saved our spring, our fourteenth hot, stuffy, sticky, lusty and shameful, chestnutty and lindeny, empty and worthless spring. They were tall and composed, their hands clean and white like those of a fourteen-year-old, to our questions they’d smile with a smile calm and wise, and direct the conversation back to you, to what is happening to you, - and the things happening to us that spring were grey and moist, gooey and dense, and we didn’t want to talk about them at all, so we didn’t, and they didn’t insist. Sabina was the first to say yes to them, and she returned home the very same evening with a smile calm and wise, and I remember us standing around her, in silence, watching her clean soft hands, thirteen-year-old’s hands, and in the left one she had a pin of a colour we had never seen before, and in the right one, dry and writhed, she had nothing. That pin they only gave to those who joined their ranks on the first day, and you did not have to be an exceptionally gifted child to understand the logic to it. We joined the ranks on the second day, and on the third day some joined the ranks and some didn’t, and on the fourth day our parents were informed that there were those who had joined (and those who hadn’t), and we remember our little colourless parents with their heavy hands and their forlorn faces standing in front of those people and asking questions, and those people answering them very patiently. We remember standing nearby and looking at our parents, too scared to breathe (those who joined and those who didn’t), not hearing a word, seeing our parents leaving one by one without even shaking those people’s hands, hands of those people who had saved our spring, for the fear of getting those hands dirty, those smooth hands, almost transparent. The next day Danya (who had joined the ranks on the very first day - Danya was always brighter than us, that is why we would beat him up at least once a week) came up to those people and said he had to have answers to some questions at least - for himself and for his alcoholic father, so Danya was hanged on the nearest apple tree, and you didn’t have to be an especially gifted child to understand they were right. They came to us each night, two for each block, they canvassed our stifling little town house by house, they talked to us, their voices soft and deep, and promised us a common future, and we had no need to ask questions, and the ones who had joined were sitting directly in front of those people looking into their attentive faces, and the ones who hadn’t joined were standing behind our backs and listening, and then approached those people, the people of our spring, and said the heavy short words of the oath, and received the pins of a simple and even of a silly colour, but they clenched them in their fists with humility and understanding. On the sixth of May, we saw Alex Goldwing throwing his pin into the river and starting to cry for some reason, and we hanged him on the apple tree, and he, thank goodness, was a bright boy, one of our most gifted boys, so he didn’t resist at all, and while Cristina was wailing we were holding her back and stroking her hair because she was one of us. And on the day when we woke up and they weren’t there anymore, those people with faces white as shame, sweet words and dark eyes, we nearly suffocated, and in every house there was this empty spring, stiff, dense, sticky and heavy, - and only one thing saved us: now we really had a common future, a future where we cry of nostalgia every time we see an apple tree.
