Tying a Horse
When tying, always keep safety in mind; an improperly tied horse can be potentially dangerous.
Use strong halters and ropes, and keep them in good repair. Nylon web halters should be at least three-ply with sturdy hardware. Remember that a halter rope is only as strong as its snap. It's often best to have a rope permanently braided onto the halter ring or tied with a secure knot rather than depending on a snap to hold if a horse sets back. If you don't leave halters on horses (and you shouldn't — it's a dangerous practice because a horse may catch the halter on something in his pen or pasture), you don't need a snap-on lead rope and can keep your ropes securely and permanently fastened to the halter.
USE A SAFE KNOT
Always use a knot that will securely hold yet be easy to untie in a hurry. The "manger tie" is a good quick-release knot that can be undone with just a pull on the loose end. If a horse nibbles at the rope or tries to untie himself, you can put the loose end through the loop so he can't undo the knot.
Always tie to something solid. Careless tying, use of a flimsy rope or halter, and tying with bridle reins or to an insecure object invite disaster. If a horse sets back (as even a well-trained one will do if suddenly startled) and breaks the rope or bridle reins, he could fall over backward, pull the top pole or board off a fence, or bolt down the road dragging whatever he was tied to. If this happens, he may seriously injure himself, people, or other horses. It may also be such a traumatic experience that he can never be dependably tied up again.
Never tie to a wire fence. The horse can get a foot caught if he paws. Tie to the post itself or to a pole or board only if it is nailed to the other side of the post and cannot be pulled off. When tying, don't leave so much length of rope that the horse might get a foot over it. Tie short, but with enough rope to give some freedom of head and neck movement, and level with his head or higher. Then if he pulls back it won't damage his neck muscles as readily as it would if he were tied too low.
Properly tied, the horse has some freedom of head and neck but never can get his foot over the rope. The rope should be tied level with his head or higher.
When tying a horse to a tree, tie high, make sure the rope cannot slip down the trunk, and be sure there are no branches to create a hazard for his eyes.
When tying to a tree, make sure the rope won't slip down the trunk. Choose a tree with no branch ends that might injure his head or eyes. When tying to a horse trailer, be sure the trailer is attached to the pulling vehicle or securely blocked so the wheels can't move. When tying near other horses, keep the horses far enough apart so they can't kick at each other. Never tie with bridle reins; this is a sure way to end up with broken reins or an injured mouth.
Handling Feet
When handling a foot, the more body contact you have with the horse, the better. Not only are you braced against him for holding the leg, but you can also sense his mood (tense or relaxed) and anticipate any movement he might make. (See chapter 4 for more information.)
Picking Up a Foot
To pick up a front foot, stand at the shoulder, facing to the rear, and slide your hand down the leg. Press your fingers just above the horse's fetlock joint. If he doesn't immediately pick up the foot, press a little harder in the area just in front of the back tendon — the indentation between tendon and cannon bone. If necessary, lean into his shoulder at the same time to encourage him to shift weight off that leg and pick up the foot. Pinching just in front of the back tendon generally makes even the stubborn horse pick up his foot and is easier than trying to wrestle with him.
SAFETY TIPS FOR HORSE WORK
Use basic rules of safety when working around a horse for any purpose — proper positioning and body contact will work to your advantage. You are less apt to be bumped, kicked, or stomped if you're close to the horse and braced against him in such a way that you keep him from moving improperly. If he does move, you'll be moving with him.
When working around front feet, take care never to be right in front of a leg as you bend over to examine it or apply medication or hoof dressing while the foot is on the ground. The horse can unintentionally bump you just by picking up his foot; his knee can hit you in the face. Always watch out for the knee.
When working around hindquarters, be aware that maintaining close contact is always safer than being a short distance away where you'd get the full force of a kick. Close contact can prevent a kick if you are braced against the leg. It also allows you to move with the horse and not be hurt.
To pick up a hind foot, stand next to his hindquarters facing to the rear and run one hand down the leg while you lean against the horse with your other hand to encourage him to shift his weight and pick up that foot. If he does not want to pick it up you can pinch the back tendon gently.
Run a hand down the back of the front leg and prepare to pick up the foot. Press your shoulder against the horse's to encourage him to shift his weight off that foot. Squeeze gently at the fetlock joint if he doesn't pick the foot up for you.
To pick up a hind foot, lean against the horse to help him shift his weight. Run a hand down the leg, pinching the back tendon gently if necessary, then pick up the foot.
Once you've picked up the hind foot, prepare to rest it against your thigh for examination or cleaning.
If the horse tries to jerk the foot away while you are cleaning or trimming it, you can thwart this by bending the fetlock joint, firmly pulling the toe up. He cannot kick at you or take the foot away as readily with the joint flexed. This added leverage works in yourfavor.
Holding a Foot
When holding a front foot to clean, trim, or doctor, be sure to keep your upper arm or shoulder in contact with the horse's side. This affords more leverage if you have to lean against him and hold tightly to the foot to keep him from pulling it away. When holding a hind foot, try to keep almost the entire hind leg in contact with your own leg. If the horse tries to jerk his hind foot away, firmly bend the foot by pulling the toe up toward you. This makes it harder for him to pull it away and kick you.
Holding Up a Front Foot
Sometimes a horse fidgets or kicks so much that certain tasks become awkward or impossible to complete. In many instances, holding up a front leg can minimize these actions and immobilize him. The safest way to do this is with another person holding the horse. On the side from which you don't want the horse's hind foot to kick, pick up the front leg. Hold up the leg with one hand, with maximum contact and leverage, and brace yourself against the horse's shoulder with the other. Be prepared to move with him in case he tries to lunge and rear. Don't be in a position where you would get bumped or hit by the leg, and be ready for any movement the horse might make.
THE IMPORTANCE OF TOUCH
Safety around horses is a matter of knowing the individual horse well and being alert to his potential reactions. It means intuitively keeping yourself in the position relative to him that puts you at least risk. This is usually a position very close to the horse, touching him and transmitting your confidence to him through that touch.
Establishing Body Contact
Body contact with a horse not only gives better leverage for holding or working on him but also enables you to tell whether he is relaxed or tensing up for sudden action. In addition, close contact and confidence on your part — being relaxed and having a matter-of-fact attitude — help the horse to be more relaxed and secure. Because he knows where you are and what you are doing, he is less apt to be jumpy or try to kick. This is especially important when giving injections or treating a wound. Sudden contact from a needle or medication spray, for example, may startle or alarm him. If you are touching him in that area and reassuring him, and he's aware of and tolerating your presence, you can usually accomplish the procedure without much reaction on his part.
When holding a front foot, keep your upper arm and shoulder in contact with the horse.
Using Restraints Properly
There are several ways to restrain a horse if he does not want to submit to a particular procedure. Extreme measures involve the use of ropes (scotch hobbles or sidelining — tying the hind leg to the front leg) to keep the horse from kicking, or tranquilizers. The average horse person should not try these; improper use of ropes and hobbles can put you and the horse at risk for injury, and improper use of tranquilizers can be hazardous and should be given only by a veterinarian. It's better to rely on other methods, if possible. Should these fail, seek the help of a professional or a veterinarian.
Cross-ties often help to keep a horse from moving around while you are working with him.
Cross-Ties
Cross-ties are two ropes with snaps that secure the horse in the middle of an open space; the ropes are snapped to each side of the halter. This approach is often used in a box stall or barn aisle. If the horse is accustomed to being crosstied, this is often adequate restraint. He is held from both directions but with open space on each side for you to work in — he can't crash you or himself into a fence or wall. Yet, the restraint of a rope on both sides of his head limits his movement.
Chain over the Nose or through the Mouth
For some horses, a chain over the nose gives better restraint than a halter. Whereas they root and pull and misbehave with just a halter, the chain over the nose gives more pressure when you need to remind horses to stand still. In some cases, the chain works better through the mouth (like a bit) rather than over the nose. The halter rope or lead strap has a short length of chain at the snap end to put over the nose or through the mouth, if needed. When using a chain, be careful not to get your fingers wrapped in it.
A chain over the nose gives better restraint than just a halter if you need to remind a horse to stand still or to gain more control when leading him.
Lip Chain
Some people prefer to put the chain under the upper lip and over the gum. This works well for some horses but not others. The lip chain is effective in making a fractious horse behave and stand because it utilizes pressure points that stimulate release of endorphins. If properly used, without jerking it too hard, it does not hurt the horse. However, it can be a severe restraint that injures the tender gum and upper lip if the horse rears and plunges. A piece of hay twine or small rope or heavy cord works as well as a chain and is less likely to be abrasive to the tender gums.
Twitch
The traditional twitch is made from a length of wooden handle 15 to 30 inches (38-76 cm) long, with a loop of rope or chain attached to one end. Newer metal versions clamp onto the upper lip and the halter. This enables the horse handler to have both hands free to work on the horse after adjusting the twitch if there is no one to help. The disadvantage of this "one person" design is that if the clamp twitch loosens and the lip comes free, the horse is no longer under control and is left with the tool dangling from his halter. If it's too tight and he breaks away, he may start running blindly to try to escape the clamp stuck on his upper lip.

Not All Horses Can Be Twitched
Some horses will not tolerate a twitch. Others, such as foals or young horses, should never be twitched to begin with. A youngster who has not had a lot of handling may react with fright and resent any further handling of the muzzle. The best candidates for twitching are emotionally stable adult horses who are accustomed to human contact; these animals are basically trusting and tractable and don't mind having a person handle the upper lip.
SAFETY TIP
The safest way to twitch a horse is with two people — one to handle the twitch and keep the horse under control, and one to do the treatment. Both people should stand on the same side of the horse so they each know what the other is doing and can be prepared for the horse's reactions. If he lunges, jumps around, or tries to break away, he'll go in the direction away from both people and no one will be stepped on.
Why a Twitch Works
Twitching increases the horse's ability to handle pain by stimulating receptors in the skin that activate a pain-decreasing, morphine-like substance that causes heart rate to decrease and sensations of pain to be reduced. These substances, called endorphins, also make the horse calmer. After application of the twitch, he becomes quieter and appears somewhat sedated, with drooping eyelids. He stands as if in a trance, with greater tolerance for pain and discomfort. A lip chain works in the same way. A twitch relaxes a horse quickly and easily and only temporarily, but you can usually keep the horse twitched for up to 10 or 15 minutes with no after-effects. As soon as you release the twitch or lip chain, he's back to normal. There's no waiting for the effects of a sedative or tranquilizer to wear off.
If done correctly, twitching is not resented by most horses and won't make them headshy. The twitch should never be painfully tight, just enough to put a little pressure on the upper lip. When the procedure is finished you should release the twitch slowly and gently massage the upper lip. Then, there are no bad memories for next time.
To put a twitch on a horse, hold on to the handle with one hand and place your other hand through the loop and with that hand take hold of the horse's upper lip then gently slide the chain or thong over your hand and around the lip. Next twist the handle until the loop tightens around the lip. It must be tight enough to put pressure on the tissues, but not so tight that it causes pain. You may have to tighten it more during the procedure being done with the horse if the horse is not "sedated" enough, or release a little pressure if it's too tight. The first time you try to twitch a horse, it's best to do it under the direction of an experienced person.
A twitch increases the horse's ability to handle pain. It should be tight enough to put a little pressure on the upper lip.
Hand Twitching. A horse who tolerates a twitch can often be "hand twitched" by gently grasping his nose and squeezing. This can make him stand still and calm for a few minutes, rather than fighting the procedure being done to him.

DESCRIPTION
DOXORUBICIN HYDROCHLORIDE is a cytotoxic anthracyclineantibiotic isolated from the culture of Streptomyces peucetius var. caesius.
CLINICAL PHARMACOLOGYMECHANISM OF ACTION Doxorubicin HCl is one of the anthracycline antibiotics, isolated from strains of Streptomyces peucetiusvarcaesius. It can penetrate through cell wall rapidly and intercalate with DNA topoisomerase II and breaks protein associated DNA strand. This action causes inhibition of mitotic activity, nucleic acid synthesis, mutagenesis, and chromosomal aberrations. Another action of Doxorubicin HCl is its reaction with cytochrome P450 to produce hydrogen peroxide and hydroxyl radical that are highly destructive to cell. Besides acting as cytotoxic, Doxorubicin has another activity from animal studies. Its another activities are immunosuppression, induction of toxic effects including cardiac toxicity, administration, Doxorubicin, shown rapid plasma clearance. Urinary excretion approximately 4-5% of the dose administration excreted in the bile or feces in 7 days and 75% of the drug present in plasma is bound to protein. Impairment of hepatic function increase retention and accumulation in plasma and tissue.
INDICATIONS
DOXORUBICIN HYDROCHLORIDE powder for injection has been used to produce regression in disseminated neoplastic conditions such as acute lymophoblastic leukemia, acute myeloblastic leukemia, Wilms tumor, neuroblastoma, soft tissue and bone sarcomas, breast carcinoma, ovarian carcinoma, transitional cell bladder carcinoma, thyroid gastric carcinoma, lymphomas of both Hodgkin and non-Hodgkin types, bronchogenic carcinoma in which the small cell histologic type is the most responsive to compared to other cell types, and gastric carcinoma.
CONTRAINDICATIONS
DOXORUBICIN HYDROCHLORIDE therapy should not be started in patients who have marked myelosupperession, or those at risk of cardiac diseases.
DOXORUBICIN HYDROCHLORIDE treatment is contraindicated inpatients who received previous treatment with complete cumulative doses of DOXORUBICIN HYDROCHLORIDE and/or Daunorubicin.
DOXORUBICIN HYDROCHLORIDE treatment is also contraindicated in:
• Patients with cardiac failure or preexisting heart ```disease
• Patients known to have hypersensitivity to the ```drug
• Myelosuppresion induces by treatment of the` ```antitumor agents or radiotherapy
• Pregnancy
DOSAGE AND ADMINISTRATION
• The recommended doses are 60-75 mg/m as

a single i.v administration for 21 daysthe lower dose (60 mg/m ) should be given to

patients with inadequate marrow reserves due toneoplastic marrow infiltration or old age or prior therapy.
• An alternative doses are 20 mg/m each
2
successive days repeated every 4 weeks to decrease toxicity.
• The dose of Doxorubicin when used in combination with other myelosuppresivedrugs is 30-40 mg/m every 3 to 4 weeks and 60-

75 mg/m it used with drugs that are not

myelosuppresive.
• Dosage of Doxorubicin should be reduced if the bilirubin increase as follow:
o 1/2 normal dose of Doxorubicin if serum bilirubin is 12-30 mg
o 1/4 normal dose of Doxorubicin if serum bilirubin is > 30 mg
• The following procedure is recommended:
o The bladder should be catheterized and emptied.o A solution containing 80 mg of Doxorubicin in 100 ml of normal Saline, should be instilled via catheter into the bladder.
o The catheter should than be removed and the patient instructed to lie on side. At 15 minutes interval, the patient should be instructed to alternate to the other side over 1 hour period.
o The patient should not urinate for 1 hour after which the bladder should emptied of solution.
o The procedure is repeated at monthly intervals.

MEKANISME KERJA FARMAKOLOGI KLINIS Doxorubicin HCl adalah salah satu antibiotik anthracycline, yang diisolasi dari strain Streptomyces peucetiusvarcaesius. Antibiotik ini dapat menembus dinding sel dengan cepat dan menyambung DNA topoisomerase II dan memutus ikatan DNA protein yang terkait. Tindakan ini dapat menyebabkan penghambatan aktivitas mitosis, sintesis asam nukleat, mutagenesis, dan penyimpangan kromosom. Tindakan lain dari Doxorubicin HCl adalah reaksinya dengan sitokrom P450 untuk menghasilkan hidrogen peroksida dan radikal hidroksil yang sangat merusak sel. Selain sebagai sitotoksik, Doxorubicin memiliki aktivitas lain dari penelitian hewan. INDIKASI
Bubuk DOXORUBICIN HYDROCHLORIDE untuk injeksi digunakan untuk menghasilkan regresi pada kondisi neoplastik diseminata seperti leukimia lymophoblastic akut, leukimia myeloblastic akut, tumor Wilms, neuroblastoma, jaringan lunak dan sarkoma tulang, karsinoma payudara, karsinoma ovarium, karsinoma kandung kemih sel transisional, karsinoma gaster tiroid, limfoma tipe Hodgkin dan non-Hodgkin, karsinoma bronkogenik dimana tipe histologis sel kecil paling responsif dibandingkan tipe sel lainnya, serta karsinoma lambung.
KONTRAINDIKASI
Terapi DOXORUBICIN HYDROCHLORIDE dianjurkan dilakukan pada pasien yang telah menunjukkan tanda-tanda myelosupresi, atau mereka yang berisiko terkena penyakit jantung.
Pengobatan DOXORUBICIN HYDROCHLORIDE adalah kontraindikasi pasien rawat inap yang menerima pengobatan sebelumnya dengan dosis kumulatif DOXORUBICIN HYDROCHLORIDE dan/atau Daunorubicin lengkap.
Pengobatan DOXORUBICIN HYDROCHLORIDE juga dikontraindikasikan pada:
• Pasien dengan gagal jantung atau penyakit jantung yang sudah ada sebelumnya.
• Pasien yang diketahui memiliki hipersensivitas terhadap obat.
• Myelosupresi yang terinduksi oleh pengobatan agen antitumor atau agen radioterapi.
• Kehamilan
DOSIS DAN PEMBERIAN
• Dosis yang dianjurkan adalah 60-75 mg/m2 sebagai pemberian intravena tunggal selama 21 hari,
Dosis lebih rendah (60 mg/m2) harus diberikan pada pasien dengan cadangan sumsum tidak memadai karena infiltrasi sumsum tulang lambat atau usia tua atau terapi sebelumnya.
• Dosis alternatif masing-masing 20 mg/m2 setiap hari berturut-turut diulang setiap 4 minggu untuk menurunkan toksisitas.
• Dosis Doxorubicin bila digunakan dalam kombinasi dengan myelosuppresunder lain adalah 30-40 mg/m2 setiap 3-4 minggu dan 60-75 mg/m2 bila digunakan dengan obat yang bukan myelosuppresive.
• Dosis Doxorubicin harus dikurangi jika bilirubin meningkat sebagaimana berikut:
o 1/2 dosis normal Doxorubicin jika bilirubin serum 12-30 mg
o 1/4 dosis normal Doxorubicin jika bilirubin serum > 30 mg
• Prosedur yang dianjurkan sebagai berikut:
o Kandung kemih harus dipasangi kateter dan dikosongkan.
o Larutan mengandung 80 mg Doxorubicin dalam 100 ml Saline normal, harus ditanamkan melalui kateter ke dalam kandung kemih.
o Kateter sebaiknya dilepas dan pasien diinstruksikan untuk berbaring miring. Pada interval 15 menit, pasien harus diinstruksikan untuk berganti ke sisi lain selama 1 jam.
o Pasien tidak boleh buang air kecil selama 1 jam karena kandung kemih sedang dikosongkan dari larutan.
o Prosedur diulang setiap bulan.

Menimbang : a. bahwa dalam Keputusan Menteri Perhubungan Nomor KM. 9 Tahun 2004 tentang Pengujian Tipe Kendaraan bermotor telah diatur mengenai pengujian emisi gas buang kendaraan bermotor dan dalam Keputusan Menteri Negara Lingkungan Hidup Nomor
141 Tahun 2003 tentang Ambang Batas Emisi Gas Buang Kendaraan Bermotor Tipe baru dan Kendaraan Bermotor Yang sedang diproduksi (current production) telah diatur mengenai Ambang Batas Emisi Gas Buang untuk Kendaraan Bermotor Tipe baru dan kendaraan bermotor yang sedang diproduksi (current production);
b. bahwa dengan pertimbangan huruf a, maka perlu diatur dengan Peraturan Direktur Jenderal Perhubungan Darat tentang Tata Cara Pelaksanaan Uji Emisi Gas Buang Kendaraan Bermotor Tipe Baru dan Kendaraan Bermotor yang Sedang Diproduksi (Current Production).
Mengingat : 1. Undang-undang Nomor 14 Tahun 1992 tentang Lalu Lintas dan Angkutan Jalan (Lembaran Negara Tahun 1992 Nomor 49, Tambahan Lembaran Negara Nomor 3480);
2. Peraturan Pemerintah Nomor 44 Tahun 1993 tentang Kendaraan bermotor dan Pengemudi (Lembaran Negara Tahun 1993 Nomor 59, Tambahan Negara Nomor 3530);
3. Peraturan Presiden Nomor 9 Tahun 2005 tentang Kedudukan, Tugas, Fungsi, Kewenangan, Susunan Organisasi dan Tata Kerja Departemen;
4. Keputusan Menteri Negara Lingkungan Hidup Nomor 141 Tahun 2003 tentang Ambang Batas Emisi Gas Buang Kendaraan Bermotor Tipe Baru dan Kendaraan Bermotor Yang Sedang Diproduksi (current production);
5. Keputusan Menteri Perhubungan Nomor 9 Tahun 2004 tentang Pengujian Tipe Kendaraan bermotor;
6. Keputusan Menteri Perhubungan Nomor KM.43 Tahun 2005 tentang Struktur Organisasi dan Tata Kerja Departemen Perhubungan;
MEMUTUSKAN
Menetapkan : PERATURAN DIREKTUR JENDERAL PERHUBUNGAN DARAT TENTANG PELAKSANAAN UJI EMISI GAS BUANG KENDARAAN BERMOTOR TIPE BARU DAN KENDARAAN BERMOTOR YANG SEDANG DIPRODUKSI ( CURRENT PRODUCTION ).
BAB I KETENTUAN UMUM
Pasal 1
Dalam Peraturan ini yang dimaksud dengan :
1. Kendaraan bermotor adalah kendaraan yang digerakkan oleh peralatan teknik yang berada pada kendaraan itu;
2. Ambang batas emisi gas buang adalah batas maksimum zat atau bahan pencemar yang boleh dikeluarkan langsung dari pipa gas buang kendaraan bermotor;
3. Stock kendaraan bermotor adalah jumlah kendaraan bermotor yang sudah diproduksi dan atau diimpor yang telah berada di wilayah pabean Indonesia tetapi belum didaftarkan untuk mendapatkan STNK;
4. Jumlah Berat yang Diperbolehkan (GVW) adalah Berat maksimum Kendaraan Bermotor berikut muatannya yang diperbolehkan menurut rancangannya;
5. Jumlah berat kombinasi yang diperbolehkan (GCW) adalah berat maksimum rangkaian kendaraan bermotor berikut muatannya yang diperbolehkan menurut rancangannya;
6. Direktur Jenderal adalah Direktur Jenderal Perhubungan Darat;
