MEDICAITON
1. Therapeutic Indications:
• Duodenal ulcer
• Gastric ulcer
• Moderate and severe reflux oesophagitis.
• Zollinger-Ellison-Syndrome and other pathological
hypersecretory conditions
2. Contraindications:
Controloc® i.v. should not be used in cases of known
hypersensitivity to its constituent.
3. Precautions for use:
What precautions have to be taken during the use of
the product?
• The intravenous administration of Controloc® i.v is
recommended only if oral application is not
appropriate.
• Controloc® i.v is not indicated for mild
gastrointestinal complaints, such as nervous
dyspepsia.
• Prior to treatment the possibility of malignancy of
gastric ulcer or a malignant disease of the
oesophagus, should be excluded as the treatment
with Controloc® i.v may alleviate the symptoms of
malignant ulcers and can thus delay diagnosis.
• A diagnosis of reflux oesophagitis should be
confirmed by endoscopy.
• The daily dose of 40 mg pantoprazole should not
be exceeded in elderly patients or in those with
impaired renal function. In patients with severe
liver impairment the daily dose has to be reduced
to 20 mg pantoprazole. Furthermore, in these
patients the liver enzymes should be monitored
during Controloc® i.v therapy. In case of a rise of
the liver enzymes Controloc® i.v should be
discontinued.
• Children: To date there has been no experience
with treatment in children
• Pregnancy and Lactation: Clinical experience in
pregnant women is limited. There is no
information on the excretion of pantoprazole into
human breast milk. Controloc® i.v. should only be
used when the benefit to the mother is considered
greater than the potential risk to the foetus/ baby.
• Effects on the ability to drive and to use machines
or work without a firm foothold:
There are no known effects on the ability to drive or
to operate machinery or to work without a firm
foothold.
4. Interactions:
Can using the Controloc® i.v.influence the effects of
other treatments?
Changes in absorption should be observed when
drugs whose absorption is pH-dependent e.g.
ketoconazole, are taken concomitantly.
Please note that this information also applies to
drugs taken a short time before Controloc® i.v. The
active ingredient of Controloc® i.v. is metabolised in
the liver via the cytochrome P450 enzyme system.
An interaction of pantoprazole with other drugs or
compounds which are metabolised using the same
enzyme system cannot be excluded. However, no
clinical significant interactions were observed in
specific tests with a number of such drugs or
compounds, namely carbamazepine, caffeine,
diazepam, diclofenac, digoxin, ethanol,
glibenclamide, metoprolol, naproxen, nifedipine,
phenytoin, piroxicam, theophylline, and an oral
contraceptive.
There were also no interactions with concomitantly
administered antacids.
Please make sure to inform your doctor if you are
taking drugs affecting blood coagulation. Additional
checks of your blood coagulation values may be
necessary.
5. Instructions of use:
Route of administration: Intravenous
The intravenous administration of Controloc® i.v. is
recommended only if oral application is not
appropriate. For this reason the dosage must be
determined by the doctor in relation with the
indication and the duration of the parenteral
treatment
How much Controloc®iv should you use and how
often?
The recommended intravenous dosage is one vial (40
mg pantoprazole) Controloc® i.v. per day, otherwise
as prescribed by the doctor.
For the long-term management of Zollinger-Ellison-
Syndrome and other pathological hypersecretory
conditions patients should start their treatment with
a daily dose of 80 mg (2 vials of Controloc iv 40 mg).
Thereafter, the dosage can be titrated up or down,
as needed using measurements of gastric acid
secretion to guide. With doses above 80 mg daily, the
dose should be divided and given twice daily. A
temporary increase of the dosage above 160 mg
pantoprazole is possible but should not be applied
longer than required for adequate acid control.
Treatment duration in Zollinger-Ellison-Syndrome
and other pathological hypersecretory conditions is
not limited and should be adapted according to
clinical needs.
How to use Controloc® iv?
A ready-to-use solution is prepared by injecting 10
ml of physiological sodium chloride solution in to the
vial containing the dry substance. This solution may
be administered directly or may be administered
after mixing with 100 ml physiological sodium
chloride solution, 5% glucose.
Controloc® i.v. must not be prepared or mixed with
solvents other than those stated. The drug should be
administered intravenously over 2-15 minutes. The
reconstituted solution must be used within 12 hours
after preparation.
As soon as oral therapy is possible, treatment with
Controloc® i.v. should be discontinued and 40 mg
pantoprazole p.o. should be administered instead.
What must you do if you have missed the dose of
Controloc® iv?
The drug is administrated under close medical
supervision and a missed dose is unlikely. In any case,
the doctor must adjust the dosing schedule.
6. Overdose:
What to do in the event of accidental overdose of
Controloc® iv?
Controloc® iv injection is carefully monitored and
supervised by the doctor and an overdose is unlikely.
There are no known symptoms of overdosage in
man.
In the case of overdosage with clinical signs of
intoxication, the usual rules of intoxication therapy
apply.
7. Side Effects:
What undesirable effects might this treatment
cause?
• Gastrointestinal complaints such as upper
abdominal pain, diarrhoea, constipation or
flatulence were reported occasionally. There have
been uncommon and rare reports of nausea,
vomiting and dry mouth.
• Treatment with Controloc® i.v. can occasionally lead
to headache. There have been rare reports of
dizziness or disturbances in vision (blurred vision).
• Allergic reactions such as pruritus and skin rash
were reported occasionally. Wheals and swelling of
the mucus were reported in isolated cases
(urticaria, angioedema) and also very rare severe
skin reactions such as Stevens-Johnson-Syndrome,
Erythema multiforme, Lyell-Syndrome and
photosensitivity.
• Increased liver enzymes (transaminases, γ-GT) were
reported in individual cases. Severe hepatocellular
damage leading to jaundice with or without
hepatic failure has occurred very rarely.
• Rare arthralgia and very rare myalgia have been
also reported.
• Mental depression was reported in very rare cases.
• Increased body temperature, peripheral edema,
interstitial nephritis leukopenia and
thrombocytopenia were observed very rarely.
Injection site thrombophlebitis has been reported
also.
• Elevated triglycerides were reported in individual
cases.
• Treatment with Controloc® i.v. can in isolated cases
lead to anaphylactic reactions including
anaphylactic shock with its typical symptoms such
as dizziness, rapid pulse or profuse sweating.
If you experience any of these troubles, or other not
mentioned in this leaflet, please inform your doctor.
8. Storage Precautions:
How to store Controloc® iv?
Controloc® i.v. should not be used beyond the expiry
date.
Controloc® i.v. should not be stored above 30°C and keep container in the outer carton.
Shelf life after preparation of the solution.
The reconstituted solution must be used within 12
hours after preparation.

4. Interakcionet:

Ndryshimet në absorbim do të duhej të vrojtohen kur njëkohësisht merren barna të cilat varen nga pH-shi , psh. ketokonazoli.
Duhet marrë në konsideratë që ky informacion ka të bëj edhe me barnat të cilat merren për një kohë të shkurtër para Controloc® i.v. Përbërësi aktiv i Controloc® i.v. metabolizohet në mëlçi përmes sistemit enzimatik të citokromit P450.
Një interakcion i pantoprazolit me barnat tjera ose komponimet të cilat metabolizohen duke përdorur sistem enzimatik të njëjtë nuk mund të përjashtohet. Çfarë sasie të Controloc®iv do të duhej ju të përdorni dhe sa shpesh?
Doza e rekomanduar intravenoze është një flakon (40 mg pantoprazol) Controloc® i.v. në ditë, përndryshe jepet sipas përshkrimit të mjekut.
Për menaxhim afatgjatë të sindromës Zollinger-Ellison- dhe konditave patologjike hipersekretore pacientët duhet të fillojnë trajtimin e tyre me dozë ditore prej 80 mg (2 flakona Controloc i.v. 40 mg).
Pastaj, doza mund të titrohet lart ose poshtë, sipas nevojës duke matur sekrecionin e acidit gastrik si udhëzues. Me dozat mbi 80 mg në ditë, doza do të duhej të jepet dy herë në ditë. Një rritje e përkohshme mbi 160 mg pantoprazol është e mundshme por nuk do të duhej aplikohej më gjatë se sa është e nevojshme për kontroll të aciditetit.
Kohëzgjatja e trajtimit te sindroma Zollinger-Ellison dhe konditat tjera patologjike hipersekretore nuk është e kufizuar dhe do të duhej të adaptohej sipas nevojave klinike.
Si të përdoret Controloc® iv?
Tretësira e gatshme-për-përdorim gatitet me injektim të 10 ml të tretësirës fiziologjike të NaCl në flakonin i cili përmban substancë të tharë. Kjo tretësirë mund të administrohet direkt ose mund të administrohet mbas përzierjes me 100 ml tretësirë fiziologjike të NaCl, 5% glukozë.

6. Tejdozimi:
Çka duhet bërë në rast të tejdozimit aksidental të Controloc® iv?
Controloc® iv injekcioni monitorohet dhe mbikëqyret me kujdes nga mjeku dhe tejdozimi është gati i pamundshëm. Nuk janë të njohura simptoma të tejdozimit te njerëzit.
Në rast të tejdozimit me shenja klinike të intoksikimit, përdoren rregullat e rëndomta të terapisë për intoksikacione.
7. Efektet e padëshirueshme:
Cilat efekte të padëshirueshme mund ti shkaktojë ky trajtim?
• Ankesat gastrointestinale siç janë dhimbje e pjesës së epërme abdominale, diarreja, konstipacioni ose flatulenca janë raportuar nganjëherë. Ka pasur raporte të pazakonshme dhe të rralla dhe të pështirosjes, vjelljes dhe gojës së thatë.
• Trajtimi me Controloc® i.v. nganjëherë mund të shkaktojë kokëdhimbje. Ka pasur raporte të rralla të marramendjes ose çrregullime të të pamurit (shikim i turbullt).
• Reakcionet alergjike siç janë pruriti dhe kurteshi janë raportuar nganjëherë. Mavijosja dhe edema e mukozave janë raportuar në raste të izoluara (urtikaria, angioedema) dhe shumë rrallë reakcione të rënda lëkure siç janë sindroma Stevens-Johnson, Erythema multiforme, sindroma Lyell dhe fotosenzitiviteti.
• Enzimet e rritura të mëlçisë (transaminazat, γ-GT) janë raportuar në raste individuale. Dëmtimi i rëndë hepatocelular i cili ka çuar te verdhëza me ose pa insuficiencë hepatike është shfaqur shumë rrallë.

Source text - English INFORMATION FOR THE PATIENT:
Docetaxel belogs to the group of medicines called antineoplastics. It is used to treat breast cancer and non-small cell lung cancer.
Docetaxel interferes with the growth of cancer cells, which are eventually destroyed. Since the growth of normal body cells may also be affected by docetaxel, other effects will also occur. Some of these may be serious and must be reported to your doctor. Other effects may not be serious but may cause concern. Some effects may not occur for months or years after the medicine is used.
Docetaxel may also be used to treat other conditions as determinated by your doctor.
Before you begin treatment with docetaxel, you and your doctor should talk about the good this medicine will do as well as the risks of using it.
Docetaxel is to be administered only by or under the immediate supervision of your doctor.
Before using this medicine:
In deciding to use a medicine, the risks of taking the medicine must be weighed against the good it will do. This is a decision you and your doctor will make. For docetaxel, the following should be considered:
Allergies: Tell your doctor if you have ever had any unusual or allergic reaction to docetaxel.
Pregnancy: Tell your doctor if you are pregnant or if you intend to become pregnant. Studies in rats and rabbits have shown that docetaxel miscarriages and deaths of the fetus, as well as problems in the mother.
Be sure that you have discussed this with your doctor before taking this medicine. It is best to use some kind of birth control while you are receiving docetaxel. Tell your doctor right away if you think you have become pregnant while receiving docetaxel.
Breast-feeding: It is not known whether docetaxel passes into breast milk. However, because this medicine may cause serious side effects, breast-feeding is generally not recommended while you are receiving it.
Children: Docetaxel has been studied in a limited number of children. The study showed that children are especially sensitive to the effects of docetaxel and cannot be given usual doses of the medicine.
Older adults: This medicine has been tested in elderly patients and has not been shown to cause different side effects or problems in older people than it does in younger adults.
Other medicines: Although certain medicines should not be used together at all, in other cases two different medicines may be used together even if an interaction might occur. In these cases, your doctor may want to change the dose, or other precautions may be necessary. When you are receiving docetaxel, it is especially important that your health care professional know if you are taking any of the following medicines: Amphotericin B by injection; antithyroid agents (medicine for overactive thyroid); Azathioprine; Chloramphenicol; Colchicine; Flucytosine; Ganciclovir; Interferon; Plicamycin; Zidovudine; if you have ever been treated with x-rays or cancer medicines: Docetaxel may increase the effects of these medicines or radiation therapy on the blood; Erythromycins (medicine for infection); Ketoconazole; Midazolam; Orphenadrine; Testosterone (male hormone); Troleandomicine: higher blood levels of docetaxel may occur, increasing the chance of serious side effects.
Other medical problems: The presence of other medical problems may affect the use of docetaxel. Make sure you tell your doctor if you have any other medical problems, especially: alcohol abuse or history of: the risk of some side effects affecting the muscles and nerves may be increased; Chickenpox (including recent exposure); Herpes zoster: risk of severe disease affecting other parts of the body; Fluid in lungs: docetaxel may make your condition worse; Infection: docetaxel may decrease your body’s ability to fight infection; Liver disease: the chance of serious side effects is greatly increased.
Proper use of this medicine:
This medicine often causes nausea and vomiting, which is usually mild. However, it is very important that you continue to receive the medicine even if you begin to feel ill. Ask your health care professional for ways to lessen these effects.
Your doctor may direct you to take a corticosteroid medicine such as dexamethasone, starting the day before you receive an injection of docetaxel and continuing for a few days after a docetaxel treatment. This other medicine decreases the chance of an allergic reaction to docetaxel and certain other side effects. It is very important that you take each dose of the corticosteroid medicine as directed.
Dosing: The dose of docetaxel will be different for different patients. The dose that is used may depend on a number of things, including what the medicine is being used for, the patient’s size, and whether or not other medicines also being taken.
Precautions while using this medicine:
It is very important that your doctor check your progress at regular visits to make sure that this medicine is working properly and to check for unwanted effects.
While you are being treated with docetaxel, and after you stop treatment with it, do not have any immunizations (vaccinations) without your doctor’s approval. Docetaxel may lower your body’s resistance and there is a chance you might get the infection the immunization is meant to prevent. In addition, other persons living in your household should not take oral polio vaccine since there is a chance they could pass the polio virus on to you. Also, avoid persons who have taken oral polio vaccine within the past several months. Do not get close to them and do not stay in the same room with them for very long. If you cannot take these precautions, you should consider wearing a protective face mask that covers the nose and mouth.
Docetaxel can temporarily lower the number of white blood cells in your blood, increasing the chance of getting an infection. It can also lower the number of platelets, which are necessary for proper blood clotting. If this occurs, there are certain precautions you can take, especially when your blood count is low, to reduce the risk of infection or bleeding:
-If you can, avoid people with infections. Check with your doctor immediately if you think you are getting an infection or if you get a fever or chills, cough or hoarseness, lower back or side pain, or painful or difficult urination.
-Check with your doctor immediately if you notice any unusual bleeding or bruising: black, tarry stools; blood in urine or stools; or pinpoint red spots on your skin.
-Be careful when using a regular toothbrush, dental floss, or toothpick. Your medical doctor, dentist, or nurse may recommend other ways to clean you teeth and gums. Check with your medical doctor before having any dental work done.
-Do not touch your eyes or the inside of your nose unless you have just washed your hands and have not touched anything else in the meantime.
-Be careful not to cut yourself when you are using sharp objects such as a safety razor or fingernail or toenail cutters.
-Avoid contact sports or other situations where bruising or injury could occur.
Side effects of this medicine:
Along with its needed effects, a medicine may cause some unwanted effects. Some side effects will have signs or symptoms that you can see or feel. Your doctor may watch for others by doing certain tests.
Also, because of the way these medicines act on the body, there is a chance that they might cause other unwanted effects that may not occur until months or years after the medicine is used. These delayed effects may include certain types of cancer. Discuss these possible effects with your doctor.
Check with your doctor immediately if any of the following side effects occurs:
Less common: Black, tarry stools; blood in urine or stools; cough or hoarseness (accompanied by fever or chills); difficult or painful urination (accompanied by fever or chills); fever or chills; lower back or side pain (accompanied by fever or chills); noisy, rattling breathing; pinpoint red spots on skin; troubled breathing while at rest; unusual bleeding or bruising.
Rare: Chest pain; fast or irregular heartbeat; shortness of breath.
Docetaxel sometimes causes allergic reactions, especially during the first few treatments. Tell your doctor or nurse right away if you notice back pain or itching during an injection. Your doctor or nurse will be watching out for other signs of an allergic reaction while you are receiving this medicine, and will be ready to treat any serious effects right away.
Check with your doctor as soom as possible if any of the following side effects occurs:
More common: Swelling of abdomen, face, fingers, hands, feet, or lower legs; unusual tiredness or weakness; weight gain.
Less common: Red, scaly, swollen, or peeling areas of skin (severe).
Rare: Decrease in blood pressure, sometimes with dizziness or fainting; increase in blood pressure, sometimes with dizziness or headaches.
This medicine may also cause the following side effects that your doctor will watch out for:
More common: Anemia; low white blood cell count
Less common: High or low blood pressure; low platelet count in blood.
Other side effects may occur that usually do not need medical attention. These side effects may go away during treatment as your body adjusts to the medicine. Also, your health care professional may be able to tell you about ways to prevent or reduce some of these side effects. Check with your health care professional if any of the following side effects continue or are bothersome or if you have any questions about them:
More common: Burning, numbness, tingling, or pain in arms, hands, legs or feet; diarrhea; nausea; skin rash or redness (mild); sores or ulcers on the lips or tongue or inside the mouth; weakness in arms, hands, legs, or feet.
Less common: Change in color of fingernails or toenails; dry, red, hot, or irritated skin at the place of injection; headache; loosening or loss of fingernails or toenails, sometimes painful; pain in joints or muscles; pain, swelling, or lump under the skin at place of injection; vomiting.
This medicine usually causes a temporary loss of hair. After treatment with docetaxel has ended, normal hair growth should return.

INDIKACIJE:
Rak dojke:
a) Tretman lokalno uznapredovanog ili metastatskog raka dojke, posle prethodne neuspešne terapije.
b) U kombinaciji sa doksorubicinom, tretman raka dojke lokalno uznapredovanog ili metastatskog kad za ovo stanje nije data prethodna citotoksična terapija.
Nesitno Ćelijski Karcinom Pluća:
a) Tretman lokalno uznapredovanog ili metastatskog nesitno ćelijskog karcinoma pluća, posle prethodne neuspešne hemoterapije.
b) U kombinaciji sa cisplatinom, tretman pacijenata sa inoperabilnim, lokalno uznapredovanim ili metastatskim nesitno ćelijskim karcinomom pluća koji prethodno nisu primali hemoterapiju za ovo stanje.
c) U kombinaciji sa karboplatinom, je opcija tretmana na kombinovanu terapiju sa cisplatinom.
Karcinom jajnika: tretman metastatičkog karcinoma jajnika, posle neuspešne hemoterapije prve linije ili naredne hemoterapije.
Karcinom prostate: u kombinaciji sa prednizonom, je indikovan za tretman pacijenata sa androgen nezavisnim (hormonski refraktoran) metastatskim karcinomom.
Gastrički adenokarcinom: Docetaksel u kombinaciji sa 5 FU i cisplatinom je indikovan za tretman pacijenata sa uznapredovanim gastričkim adenokarcinomom, uključujući adenokarcinom gastroezofagealnog čvora, koji nisu prethodno primali hemoterapiju za uznapredovanu bolest..
Docetaksel pripada grupi lekova zvanih antineoplastici. On se koristi za tretman raka dojke i nesitno ćelijskog karcinoma pluća.
Docetaksel interferira sa rastom ćelija raka, koje se na kraju uništavaju. Pošto je rast normalnih ćelija organizma takođe afektovan od docetaksela, pojaviće se i drugi efekti. Dok se vi tretirate docetakselom, i kada prestanete tretman, nemojte se podvrgnuti imunizacijama (vakcinisanje) bez pristanka lekara. Dosta uobičajeni: Otok abdomena, lica, prstiju, ruku, stopala, ili donjih ekstremiteta; neobični umor ili slabost; dobijanje na težini.
Manje uobičajeni: Crvene, krastave, otečene, ili ljuskaste regije na koži (teške).
