Source text - German Der aktuelle Stand der gesellschaftlichen Diskussion
Sterbewünsche und Äußerungen zum eigenen Tod und Sterben von hochaltrigen, pflegebedürftigen PatientInnen in Krankhäusern und in Einrichtungen der Langzeitpflege werden in der aktuellen Diskussion, insbesondere in der Diskussion um aktive Sterbehilfe bzw. um (ärztlich) assistierten Suizid in Deutschland und Österreich gerne pro und contra zitiert.
Insbesondere Ärzte und Ärztinnen nehmen ganz Unterschiedliches wahr: Die VertreterInnen einfühlsamer Sterbebegleitung berichten, dass selten oder gar nicht Sterbewünsche an sie herangetragen werden. Die VertreterInnen eines (ärztlich) assistierten Suizids berichten von Einzelfällen mit großem Leiden, wo ihnen die Hände gebunden waren. Die einen wünschen sich keine Gesetzesänderung und mehr Ressourcen für Palliativversorgung; die anderen (in Österreich eine Minderheit) drängen auf neue gesetzliche Regelungen für einen weiteren Spielraum des (ärztlichen) Handelns.
Konkrete Forschungsziele
• Alten und hochbetagten Menschen wird in den Interviews eine Möglichkeit gegeben, Wünsche und Bedürfnisse konkret zu äußern und als neues Wissen zur Verfügung zu stellen;
• Durch die Ausdifferenzierung der Antwortperspektiven und einen verstehenden Nachfragen wird die Grundlage für ein Analyseinstrument geschaffen, das den Pflegenden und ÄrztInnen sowie auch den Angehörigen strukturiert und ausdifferenziert über den momentanen Wunsch und Wille informiert (und das – in einer gewissen Regelmäßigkeit wiederholt – auch Auskunft über etwaige Veränderungen gibt).
• Neben der Wahrung der Selbstbestimmung der BewohnerInnen, ist ein wichtiges Forschungsziel, Über- bzw. Unterversorgung zu vermeiden. Viele Menschen haben Angst vor zuviel und lebensverlängernden Therapien; gleichzeitig gibt es zu wenig Ressource für angemessene geriatrische und geronto-psychiatrische Versorgung bzw. auch die Angst, nicht ausreichende palliative Betreuung zu erhalten.
• Die Pflegenden als Hauptansprechpartner bekommen auf Grund des neuen Wissens Handlungsalternativen und Handlungssicherheit für eine verbesserte und angemessene Versorgung im Pflegeheim.
• Für die gesellschaftliche Auseinandersetzungen ist das Wissen um Wünsche und Handlungsmöglichkeiten ein doppelter Apell: Sich aktiv an einem Vorsorgedialog zu beteiligen und gleichzeitig dafür zu sorgen, dass ausreichende Ressourcen für Betreuung und Versorgung alter Menschen in Pflegeeinrichtungen zur Verfügung gestellt wird – zur Sicherung individueller Würde und Wahrung der Humanität des Versorgungssystems.
Gefährdeter Selbstwert bei BewohnerInnen und MitarbeiterInnen
Worauf das neue Wissen, die veränderten Handlungsmöglichkeiten und die gesellschaftliche Wahrnehmung im Projekt hinzielen, lässt sich mit dem Begriff des Selbstwertes beschreiben, der – wie zuvor skizziert – bei BewohnerInnen und MitarbeiterInnen gefährdet ist Selbstwert ist die Bewertung, die ein Individuum über seine eigene Person durchführt (Rosenberg, 1965). Schachinger (2005) gibt als Quellen des Selbstwerts an: kulturelle, soziale (z.B. Familie oder Freundeskreis) und individuelle (eigene Standards und Ziele). Schütz (2003) zeigte, dass individuelle Fähigkeiten und Erfolge am häufigsten als selbstwertsteigernde Quellen wahrgenommen werden. Sterbewünsche bergen daher mehrere Selbstwertbedrohungen: auf der einen Seite das berufliche Selbstkonzept von Pflegekräften, der durch die als indirekte Kritik empfundenen Sterbewünsche von Patienten herabgesetzt wird, auf der anderen Seite der Selbstwert von Patienten, die sich unverstanden und hilflos fühlen. Diese Spirale gilt es zu verstehen und zu unterbrechen.
Wohlbefinden und Lebenszufriedenheit korrelieren mit einem hohen positiven Selbstwert (siehe z.B.: Diener & Diener, 1995). Ein Gefühl von Unfreiheit und Verlust der Selbstbestimmung im Langzeitpflegekontext dagegen gehen mit reduziertem Selbstwert und klar reduzierter Lebensqualität einher. So ist eine aufmerksam gehörte Äußerung über Sterbewunsch und Lebenssattheit sowohl Ausdruck von Autonomie als auch Lebensqualität im Alter. In diese Richtung beschreiben beispielsweise Steinhauser et al (2000) Faktoren wie Vorbereitung auf das Sterben und auch ein Gefühl der Vollendung als Faktoren, die sowohl von PatientInnen als auch Pflegepersonal für wichtig erachtet wurden.
Weiterentwicklung des Themenfeldes
Sterbewünsche und Todesäußerungen werden eher der jungen Disziplin Palliative Care zugeordnet; das Forschungsprojekt will die Aufmerksamkeit für diese Themen nach vorne verlagern. Diese Thema sind – wenn die Bewohner es ansprechen wollen – Teil guter Versorgung in der letzten Lebensphase. Hier in eine entsprechende Auseinandersetzung zu gehen, ist eine interdisziplinäre und interprofessionelle Herausforderung, die hier von Medizinethik, Pflegewissenschaften, Gerontologie und klinischer Psychologie zu leisten ist.
Ethische Aspekte im Forschungsprojekt
Als Institut für Ethik und Recht in der Medizin sind wir noch einmal mehr verpflichtet, forschungsethische Standards einzuhalten. Im Falle der Projektzusage werden wir das Projekt bei der Ethikkommission der Universität Wien einreichen.
Neben der bereits vorgestellten inhaltlichen Aspekte von Nutzen und Schadensvermeidung für die BewohnerInnen, aber auch für die MitarbeiterInnen und die beteiligten Einrichtungen liegt uns insbesondere die Wahrung der Autonomie am Herzen, die unserer Einschätzung nach z.B. auch bei Menschen mit Demenz zu berücksichtigen sind. Folgende Maßnahmen sind im Projektprozess unabdingbar:
- Alle Dokumentationen sind zu anonymisieren und nur anonymisiert aufzubewahren, so dass keine Rückschlüsse auf Personen und Organisationen gezogen werden kann;
- Für alle Interviews werden im Voraus Einverständniserklärungen, verbunden mit einfachen Informationen über die Projektziele eingeholt; bei Personen mit eingeschränkter Einsichts- und Urteilsfähigkeit ist darüber hinaus das Einverständnis des gesetzlichen Vertreters/Sachwalters einzuholen
- Interviews, bei denen trotz aller Vorsicht und Aufmerksamkeit der Eindruck entsteht, dass für die BewohnerInnen Irritation oder Beeinträchtigung entstehen kann, sind unverzüglich zu beenden.
Da die Versorgung alter und hochbetagter Menschen von Solidarsystem mit finanziert wird, ist der verantwortliche Einsatz von öffentlichen Mittel und Ressourcen ein wesentliches Ziel des Forschungsprojektes; dies entspricht dem bio- und medizinethischen Prinzip der Gerechtigkeit im Gesundheitswesen.
Im Sinne der Selbstanwendung wird im Forschungsteam sowie mit den Forschungs- und Praxispartnern vertraglich vereinbart, wer welche Rechte und Verwertungsmöglichkeiten der Projektergebnissen hat.
Aus dem Forschungsteam heraus werden Artikel in den entsprechenden wissenschaftlichen Fachzeitschriften (Ethik in der Medizin, Zeitschrift für Pflege, Pflegeforschung, Pflegepraxis und Pflegemanagement, Zeitschrift für Gerontologie und Geriatrie bzw. in den entsprechenden englischsprachigen Journals eingereicht, aber auch in den praxisorientierten Zeitschriften veröffentlicht werden. Die vom FWF vorgeschrieben Open Access Policy tragen wir in Bezug auf Forschungsberichte und Fachartikel gerne mit. Inwieweit die Interviews bzw. Protokolle der Workshops – unter Wahrung aller Persönlichkeitsdaten bzw. der Trägerinteressen - veröffentlicht werden kann, wird mit dem wissenschaftlichen Beirat und den Votum der Ethikkommission abgestimmt werden.
Da die PraxispartnerInnen als konfessionelle bzw. kommunale Träger (z.B. Diakonie Österreich) mehrere Pflegeeinrichtungen betreiben, ist es ein Ziel der Umsetzung, dass die Ergebnisse, das neue Wissen und die Umsetzungsstrukturen trägerweit zu Qualitätsstandards werden und nicht in den am Projekt beteiligten Einrichtungen.
Alle Träger, die für das Projekt in Frage kommen, betreiben selbst Einrichtungen für Aus-, Fort- und Weiterbildung. Die Projektergebnisse sollen hier nicht nur Qualitätsstandards in den Einrichtungen, sondern auch innovatives Wissen in der Aus- und Weiterbildung werden. Indem wir hier die Expertise der AusbildnerInnen am Delphi-Prozess beteiligen, sind diese unmittelbar im Projekt zu beteiligen.
Im Rahmen des Instituts für Ethik und Recht in der Medizin bzw. mit im Verlauf des Projektes noch einzubeziehenden Forschungspartnern wie die Akademie für Altersforschung, Wien oder dem Albert Schweitzer Institut für Geriatrie und Gerontologie, Graz werden mit den Forschungsergebnissen nicht nur je eine Jahrestagung thematisch zu bespielen sein, gemeinsam ist auch ein Symposion geplant, das im deutsch-sprachigen Raum ausgeschrieben wird.
Translation - English The current state of social discussion
Current discourse concerning the wish to die and statements expressed by very elderly residents of geriatric care facilities and patients in hospitals on the subject of their own death and dying, and especially with regard to discussions on “death on demand” or (medically) assisted suicide in Germany and Austria, is frequently expressed in terms of pro and contra.
The medical profession in particular varies greatly in its perceptions: proponents of empathetic terminal care report rarely or ever hearing a wish to die being expressed, whilst advocates of (medically) assisted suicide report instances of great suffering in which their own hands were tied. The one side recommends retaining the legal status quo whilst making more resources available for palliative case, whilst the other (a minority in Austria) calls for a change in the law to provide more room for (medical) intervention.
Concrete research goals
• During interviews, old and very old people will be given the opportunity to expressly state their wishes and needs, with this information serving as new knowledge;
• Differentiated response perspectives and perceptive inquiries will form the basis for creating an analytical instrument which informs nursing staff and doctors, as well as relatives, about a patient’s current wishes in a structured and differentiated format (and repeated, with a certain regularity, to provide information about any changes).
• In addition to upholding residents’ right to self-determination, an important research goal is to avoid insufficient or excessive care. Many people have a fear of too many, life-prolonging therapies, yet at the same time too few resources are available for appropriate geriatric and geronto-psychiatric care, paired with the fear of receiving insufficient palliative care.
• The findings provide nursing staff – the primary point of patient contact – with alternative treatment options and the confidence to provide improved and appropriate care within the care facility setting.
• In terms of social debate, understanding wishes and having treatment options is a double call to action: to actively take part in a care dialog and at the same time to ensure the availability of sufficient resources for the support and treatment of old people in care facilities, in order to both safeguard personal dignity and to uphold the humanity of the care system.
The threat to the self-esteem of residents and nursing staff
What the new findings, changing treatment options and social perceptions in the project indicate is what we would term self-esteem. As noted above, self-esteem is under threat amongst residents and staff alike. Self-esteem is the value an individual places on their own person (Rosenberg, 1965). Schachinger (2005) believes the sources of self-esteem to be cultural, social (e.g. family, circle of friends) and individual (own standards and goals). Schütz (2003) demonstrates that individual abilities and successes are most frequently perceived as playing the greatest role in improving self-esteem. Consequently, wishes to die harbour several threats to self-esteem. On the one hand, statements expressing a wish to die are received by nursing staff as an indirect critique, consequently damaging their own professional self-image. And on the other, the wish to die reflects a lack of self-esteem in patients who feel misunderstood and helpless. This vicious circle needs to be understood and disrupted.
A sense of well-being and satisfaction with life correlate with a high level of self-esteem (e.g. Diener & Diener, 1995). In contrast, feelings of being trapped and a loss of self-determination in a long-term care context are associated with a loss of self-esteem and a clear reduction in the quality of life. Therefore, where careful attention is paid to statements made about a wish to die or being sated with life, this is both an expression of autonomy as well as an indication of quality of life in old age. Steinhauser et al. (2000), for example, describes preparing for death and a feeling of completion as being regarded as important amongst both patients and nursing staff.
Developing the field further
The wish to die and statements about death tend to be associated with the recent discipline of palliative care; the research project wishes to turn attention to this subject area. Where residents wish to speak about this subject, enabling them to do so is part of good care during the final phase of life. Examining this subject area in depth is an interdisciplinary and inter-professional challenge, one which must be overcome by the medical ethics, nursing sciences, gerontology and clinical psychology professions.
Ethical aspects of the research project
As the Institute for Ethics and Law in Medicine (IERM) we are doubly obliged to adhere to ethical research standards. Where the project is approved, it will be submitted to the University of Vienna’s Ethics Committee.
In addition to concerns about the benefits and avoidance of harm as noted above, both for the residents and for the nursing staff and institutions involved, a particular concern of ours lies in maintaining autonomy. This should also apply to those suffering from dementia. The following measures are an essential part of the project process:
- All documentation must be anonymised and only stored in anonymised form in order to ensure that neither patients nor organisations can be identified;
- Before any interviews are undertaken, declarations of consent will be acquired from potential interviewees who will also be provided with basic information about the aims of the project. Furthermore, consent must first be secured from the legal representatives/trustees before undertaking any interviews with patients who have a limited ability to understand and judge.
- Interviews which, despite all possible care and attention, give the impression of irritating or impairing the resident, will be concluded immediately.
As the care of old and very elderly people is partly financed on the basis of solidarity, a key goal of the project is to use public funds and resources in a responsible manner; this reflects the bioethical and medical ethical principles of fairness in healthcare.
We also apply these principles to ourselves, so that the rights to, and usability of, the project results will be agreed by contract with the research team, researchers and practice partners.
Articles written by the research team will be published in relevant English and German scientific journals covering the fields of medical ethics, nursing research, nursing practice and nursing management, gerontology and geriatrics, as well as practice-related publications in these fields. We are happy to support the Open Access Policy prescribed by the FWF for research reports and scientific papers. The extent to which interviews and workshop minutes may be published, after all personal and organisational data has been anonymised, will be agreed together with the scientific advisory board and with input from the Ethics Committee.
As the practical partners in the project operate several care facilities, either as confessional or municipal institutions, an implementation goal is for the results, new insights and implementation structures to become quality standards throughout the institutions as a whole, and not only in the facilities directly involved in the project.
All the institutions considered for inclusion in the project run their own training and professional development facilities. The project results should not only be adopted as quality standards in these facilities, but also be integrated as cutting-edge know-how into training and professional development programmes. As we include the trainers’ own expertise in the Delphi process, they should be directly involved in the project.
Together with research partners including the Akademie für Altersforschung in Vienna and the Albert Schweitzer Institut für Geriatrie und Gerontologie in Graz , who will become involved over the course of the project, the IERM will not only consider the research findings during its annual conference and but will also plan a joint symposium to be promoted across the German-speaking region.
