Objectif.- Évaluer les risques de complications périnatales en cas de diabète gestationnel (DG) traité ou non.
Méthode.- Recherche bibliographique effectuée par consultation des banques de données Pubmed et les recommandations HAS et NICE.
Résultats.- Le DG modéré ou sévère non traité augmente le risque de complications fœtales et néonatales. Le risque de malformations est modérément augmenté en cas de DG par rapport à la population générale. L’augmentation du risque de malformations est probablement liée à l’existence de cas de diabète de type 2 méconnus parmi les patientes avec DG. Il existe une relation linéaire entre le niveau de la glycémie maternelle et l’augmentation du poids de naissance. Le traitement du DG diminue l’incidence de la macrosomie. Les données de la littérature ne permettent pas d’estimer le risque précis de cardiomyopathie en cas de DG, mais les formes sévères sont exceptionnelles. Le risque d’asphyxie néonatale et de décès périnatal n’est pas augmenté dans le cadre du DG. Les traumatismes obstétricaux et les atteintes du plexus brachial sont des événements rares et l’augmentation du risque en cas de DG non traité n’est pas démontrée. Le risque de détresse respiratoire toutes causes confondues est difficile à apprécier. Il n’existe pas assez de données pour établir un lien entre le DG et les troubles respiratoires néonataux. Le risque d’hypoglycémie néonatale est difficile à apprécier en raison de l’hétérogénéité de la définition de l’hypoglycémie dans les différentes études, mais la fréquence de l’hypoglycémie nécessitant un traitement par voie intraveineuse est faible. Le risque d’hypocalcémie et d’hyperbilirubinémie est analogue à celui de la population générale.
Conclusion.- Les complications périnatales graves liées spécifiquement au DG sont rares. La macrosomie est la principale conséquence néonatale démontrée d’un DG. Elle est le facteur essentiel lié aux complications rapportées en cas de DG. L’obésité maternelle est un facteur de risque de complications surajouté et indépendant du diabète.
Translation - English Abstract
Objective: To evaluate the risks of perinatal complications in infants born to mothers with treated or untreated gestational diabetes mellitus (GDM).
Methods: A search of the PubMed database was performed and recommendations from NICE and the French National Authority for Health were consulted.
Results: Untreated moderate or severe GDM increases the risk of foetal and neonatal complications. The risk of malformations slightly increases in newborns of mothers with GDM compared to the general population. This risk is probably associated with the presence of undiagnosed type 2 diabetes among patients with GDM. There is a linear relationship between maternal blood glucose levels and an increased birth weight. Treatment for GDM reduces the incidence of macrosomia. Although the risk of cardiomyopathy in cases of GDM cannot be accurately estimated based on the available data, severe clinical forms are rare. The risks of neonatal asphyxia and perinatal mortality are no higher in infants born to women with GDM. Birth injuries and brachial plexus injuries are rare, and no more likely to occur in cases of untreated GDM. It is difficult to asses the risk of respiratory distress, regardless of its cause. It is not possible to establish a link between GDM and neonatal respiratory problems due to insufficient data. Although the risk of neonatal hypoglycaemia is difficult to determine due to the variable definitions reported in the literature, the incidence of hypoglycaemia requiring intravenous therapy is low. The risks of hypocalcaemia and hyperbilirubinemia are similar to the general population.
Conclusion: Serious perinatal complications specifically associated with GDM are rare. Macrosomia has been demonstrated to be the predominant adverse outcome in cases of GDM. It is the main factor linked to reported cases of complications in GDM. Maternal obesity is an additional risk factor for complications, regardless of diabetes status.

Objectif : Proposer aux Unités de Réanimation et de Médecine d’Urgence une procédure pratique (check-list) de prise en charge du patient grave adulte afin d’éviter la survenue de complications au cours des transports médicalisés intra-hospitaliers (TMIH).
Méthode : Une recherche numérique via les bases de données MEDLINE, EMBASE, CINAHL, HEALTHSTAR reprenant les items suivants : « transferring », « transport », « intrahospital » ou « intra-hospital », « critically ill patient » à été réalisée entre 1998 et le 15 février 2009. Les références bibliographiques de chacun des articles sélectionnés ont été étudiées et ont permis de compléter notre recherche.
Résultats : Cette revue se focalise sur l’analyse et la maîtrise du risque du TMIH. Les évènements indésirables (EI) associés, leur nature et incidence sont exposés, ainsi que les facteurs de risques retrouvés (lié au patient, à l’organisation, aux facteurs techniques, aux facteurs humains et collectifs). Les moyens de préventions proposés sont également revus. Une check-list de réalisation rapide lors du TMIH est ensuite justifiée et proposée.
Conclusion : Malgré l’amélioration des pratiques du TMIH, il persiste un risque significatif pour le patient grave mobile. Une formation initiale au transport, un bon sens clinique et une analyse de la balance bénéfice-risque sont pour l’instant les seuls guides pour décider d’un TMIH. Le patient sédaté, hémodynamiquement instable, sous ventilation mécanique, préparé et accompagné par une équipe inexpérimentée est une situation particulièrement à risque. L’utilisation généralisée de check-lists et un véritable plan de formation des équipes, permettrait un gain de sécurité pour le TMIH et une réduction durable des risques. Le développement de matériels adaptés (brancards, batteries, protection des lignes de perfusion) et une ergonomie adaptée des lieux de transport (salles de radiologie, salle de transfert des blocs opératoires) contribueraient à cette diminution du risque et à la sécurisation du transport. D’autres investigations sont nécessaires pour évaluer le rôle protecteur de telles mesures de prévention.
Translation - English Abstract
Objective: To provide Intensive Care Units and Emergency Departments with a set of practical procedures (check-lists) for managing critically-ill adult patients in order to avoid complications during intra-hospital transport (IHT).
Methods: From 1998 to 15th February 2009, digital research was carried out via the MEDLINE, EMBASE, CINAHL and HEALTHSTAR databases using the following key words: “transferring”, “transport”, “intrahospital” or “intra-hospital”, and “critically ill patient”. In order to make our research complete, the reference bibliographies of each of the selected articles were also studied.
Results: This review focuses on the analysis and overcoming of IHT-related risks. The associated adverse events (AE), their nature and incidence are presented, as well as the identified risk factors (relating to the patient, transport organisation, and technical, human and collective factors). The suggested preventive measures are also reviewed. A check-list for quick execution of IHT is then put forward and justified.
Conclusion: Despite improvements in IHT practices, significant risks are still involved in the transport of critically ill patients. Basic training in transporting patients, good clinical sense and a risk-benefit analysis are currently the only criteria for carrying out IHT. A sedated, haemodynamically unstable patient on mechanical ventilation, prepared and accompanied by an inexperienced team, is a particularly risky combination. The widespread use of check-lists and proper training programmes would increase the safety of IHT and reduce risks in the long-term. The development of adapted equipment (stretchers, batteries, IV-line protectors), and transport destination rooms with adapted ergonomics (radiology rooms, operating theatre) would contribute to this risk reduction and to the safety of patient transport. Further investigation is required in order to evaluate the protective role of such preventive measures.

I am a native English speaker, currently living in France with my French husband and bilingual daughter.
I left school in 1998 and went on to gain a BA in French with Spanish at the University of Surrey.
A few years later, I decided that what I really wanted to do was translate, so I went back to university and obtained an MA in translation in 2009.
I then spent nearly three years working at Besançon University Hospital (France) as a translator, interpreter and clinical research assistant. There, I translated a number of research articles, mainly in the field of neurology, and acted as an interpreter between doctors and patients. In addition, I coordinated the clinical trials conducted in the neurology department, working closely with doctors, pharmacists and clinical research associates. The experience and knowledge I gained in Besançon laid the foundations upon which I was able to build a career as a freelance translator in the field of life sciences.
In 2011, I began working as a freelance translator and have since gained a vast amount of experience through translating, post-editing and proofreading a wide range of medical- and healthcare-related documents. These have included patient information sheets and consent forms, patient information leaflets, clinical trial agreements, clinical trial budgets, ethics committee documents, hospital discharge summaries, imaging and surgical reports, and journal articles.
I thoroughly enjoy my work and the challenges it involves, and remain wholly committed to always providing a high quality, well researched, accurate translation in a timely manner.
