For Hemoglobin Screening ®
The Hemoglobin test is performed using a HemoCue, a blood testing device. You will be asked to sit comfortably, one of your fingers will be cleansed with alcohol, and a small prick will be made to your finger to gather a few drops of blood. The test is relatively simple and usually without discomfort. The following risks, while rare, are associated with a finger-stick procedure: excessive bleeding, feeling light-headed or fainting, infection (a slight risk any time the skin is broken), and scarring (occurs when there have been multiple punctures in the same area).
Use and Disclosure of Information: The personal information provided on this two-part consent form is handled as follows: Part 2 (pink) is returned to you with the results of the test and Part 1 (white) is stored by the Hb Screening Program Coordinator in accordance with appropriate local and state record retention laws. In addition, summary information [with the personal identifiers removed] will be reported to the sponsors of this test.
The information contained on this consent form will be used for aggregate reporting of hemoglobin levels. The reports do not include personally identifiable information. Original consent forms are kept on file as required by individual state regulations and are used for no other purposes.

Source text - English 16. Successors and Assigns. This Agreement shall inure to the benefit of and be binding upon the parties named herein and their respective heirs, executors, administrators, successors and assigns, and shall not be construed or enforced so as to confer any benefit upon other persons.
17. Governing Law. This Agreement is executed and delivered in, and shall be construed and enforced according to the laws of the State of Georgia, City of Johns Creek.
18. Survivorship. This Agreement shall supersede and survive any change of ownership of location where the Equipment is installed as set forth herein including, but not limited to, the sale, merger or acquisition of such business location.
19. Taxes and Liens. Operator shall pay all personal property taxes or similar assessments directly relating to the ATM location as the same becomes due.
Any Agreed Upon Addendums to Agreement:
IN WITNESS WHEREOF, the parties hereto, have executed and delivered this Agreement inclusive of the terms and conditions set forth herein, as of the day and year below written.
