SUPERVISING THE HUMANITY OF THE THERAPIST

Abstract. This paper gives an overview of a perspective on supervision as it has developed in relation to person-centered and experiential therapy and it explores supervision as a facilitative relationship focused on the development and maintenance of the counselor’s ability to relate congruently and at depth with clients. Working at relational depth in the supervision relationship enables the supervisee to go to particular depth with their own experiencing, enabling them to become more fully integrated, fully present and free to the total person of the client.
Most person-centered therapists have experience of being in supervision. For many, supervision is a compulsory requirement during training only, but in some countries therapists are required to have supervision throughout their working life as an ethical requirement and as a condition for professional accreditation or legal registration. For instance, in the USA supervision is an integral element of counseling training programs, but there is no formal requirement for ongoing supervision after the training period; in Austria, where counseling and psychotherapy are regulated, being in continuing supervision is a legal requirement for registration. In the UK, therapists who are members of national counseling and psychotherapy associations are required to have regular supervision while practicing. Supervision is usually on a one-to-one basis, especially during the therapist’s training; in Holland, the requirement for ongoing supervision for client-centered therapists after training is met through regular peer supervision, interestingly named ‘intervision’.
Much of the literature and research about supervision for counseling and psychotherapy is written from a generic perspective, seeking to define the purpose, function and practice of supervision in terms of models which are applicable to supervision across different therapeutic orientations. There is an emphasis on developing an overarching framework for understanding and describing the supervision process, the supervision relationship and the learning and development of the supervisee and a movement away from ‘approach specific’ supervision which would only be of use to the ‘purist therapist’ (Page & Wosket, 2001, p. 31). Carroll (1996) suggested that supervision requires a theoretical understanding in its own right and that supervision demands different skills from counseling and psychotherapy. He wanted to ‘move individuals away from counseling bound models of supervision (i.e., in which supervision is closely allied to the counseling orientation of the supervisor) to developmental and social role models of supervision (which start with the learning situation of the supervisee)’ (Carroll, 1996, p. 4). He went even further when he stated that ‘counseling bound models of supervision can often stress aspects of the counseling work that ignore the client and concentrate on what is happening to the supervisee’ (Carroll, 1996, p. 42).
PERSPECTIVES ON PERSON-CENTERED SUPERVISION
Much of the early writing on person-centered supervision, particularly in the USA, was centered on supervision with trainee therapists and students (Patterson, 1964, 1983; Rice, 1980; Rogers, 1942, 1951). The supervisor is frequently also in the role of trainer, and the challenge for the person-centered supervisor is to integrate in the supervision relationship developmental, facilitative elements with teaching and evaluative responsibilities. In contrast to the generic approaches, there was a strong emphasis on the importance of congruence between the theoretical and philosophical orientation of supervisor and supervisee (Hackney & Goodyear, 1984; Rogers, 1957; Schmid, 1997). ‘The supervisor needs to be aware of the person-centred approach at practical and theoretical depth’ (Mearns, 1997, p. 84), or as Patterson stated even more strongly: ‘supervisor and supervisee need to be committed to the same theory’ (Patterson, 1983, p. 21) in order to support the supervisee in the development of their understanding of the approach and in the process of integration of the core philosophy.
Rogers (1951) was very aware of the importance of supervision; it was an integral part of his work with students. For him, supervision also served a research function and as such contributed to the development of the approach in its early days (Rogers, 1975). Writing about the training of therapists, he stated that by providing the supervisee with an accepting, empathic and genuine atmosphere, the supervisor creates a climate in which the supervisee can explore feelings, blocks and difficulties that come up while learning to be a therapist (Rogers, 1951). His focus in supervision was firmly on the supervisee: ‘to help the therapist grow in self-confidence and to grow in understanding of himself or herself, and to grow in understanding of the therapeutic process’ (Hackney & Goodyear, 1984, p. 283). Patterson took a similar view: emphasizing the importance of the basic conditions of empathic understanding, respect, genuineness and concreteness in the supervision relationship, his view of supervision was that it is concerned with the ‘development of sensitivity in the student, of understanding of therapeutic attitudes’ (Patterson, 1983, p. 25). He described supervision as being concerned with the ‘actual relationship between the supervisee and the client, and not concerned with diagnosis or conceptualization of the internal dynamics of the client’ (Patterson, 1983, p. 24). His focus is on the experience of the supervisee, rather than on technique or on the learning of specific responses to clients.
Rice (1980) also wrote about supervision in the context of training therapists. She conceptualized the supervision process as a balance between supporting the supervisee’s experience in the genuine human relationship with the client and supporting their understanding of therapy as a process that can be learned. She distinguished two groups of attitudes related to the therapist’s ability to implement the client-centered conditions that can be worked with in supervision: attitudes about human nature and the nature of change, and attitudes towards self [in the therapist] (Rice, 1980, p.138). However, her emphasis appeared to be slightly different from that of Rogers and Patterson. As one of the founding theorists of the process-experiential approach, her focus was more on the theory of the client’s process and on the therapist’s style of participation in that process (Rice, 1980, p.137) and less on the supervisee’s self-experience in the relationship with the client or on the relationship in supervision. A focus on supervision with trainee therapists was also at the core of Barrett-Lennard’s overview of person-centered writing on supervision (Barrett-Lennard, 1998, pp. 333–338). He too attempted to capture the essence of supervision when he described it as ‘growth-learning’ (Barrett-Lennard, 1998, p. 333), supporting an ‘evolution of consciousness, not only in respect to self, but in span of receptivity toward the experiencing of others’ (Barrett-Lennard, 1998, p. 336). As one of Rogers’ ex-students, and with first-hand experience of supervision with him, he described Rogers’ movement from an evaluative, guiding style of supervision in the early 1940s to the much more student-centered and facilitative style after 1951. Villas Bowen (1986) distinguished this approach to supervision, which puts an emphasis on the development of the supervisee’s locus of evaluation and respects the supervisee’s unique personal response, from ‘form-oriented supervision’, where the supervisor has a commitment to a particular form of person-centered therapy and discourages styles of expression that do not fit that model. Such an approach, in its concern with the preservation of a ‘pure form’ of person-centered therapy, is fundamentally inconsistent with a basic philosophical principle of the person-centered approach: the trust in the capacity for self-direction and self-determination (Villas Bowen, p. 293,1986).
In order to support the development and integration of the therapeutic qualities of empathy, acceptance and congruence in the supervisee, these same qualities need to be present in the supervision relationship. In that sense, supervision provides a model for the supervisee’s practice: not by teaching them the supervisor’s way of doing therapy but, consistent with the theory of person-centered therapy, by offering a ‘growth promoting environment that will enable the supervisee to find his or her own style’ (Villas Bowen, 1986, p. 296).
THE SUPERVISION RELATIONSHIP
A central feature in all writing about supervision from a person-centered perspective is the focus on the development of the therapeutic ability of the supervisee through the supervision relationship and on the experience of the supervisee in the relationship with the client.
Relationship qualities in supervision: the meaning of the core conditions
As in therapy, empathy, acceptance and congruence are important relationship qualities, supporting and facilitating a climate of mutual trust and respect. In supervision these qualities help to create a relationship where supervisor and supervisee can work together creatively towards a genuine dialogue, in a spirit of ‘collaborative inquiry’ (Merry, 1999, p. 140) in an ‘an open, empathic and non-judgemental environment that is simultaneously questioning and supportive, …….promoting congruence and an internal locus of evaluation ’ (Merry, 2001, p.183). Merry’s concept of supervision as a kind of ‘personal research project’ (Merry, 2004, p. 193) fits well with the spirit of Rogers’ view of person-centered therapy as a process of testing the central hypothesis of the approach. In such a climate the supervisee is secure and free to give expression to thoughts, feelings and concerns that are in their awareness, to focus on not yet symbolized experiences in relation to their clients, or to bring questions and issues related to external factors that have an impact on the therapy relationship. Through the experience of being fully received in the supervision relationship the supervisee is enabled to become more fluid and open to both their own and their client’s experience and to communicate more openly and freely (Tudor & Worrall, 2004), in a process that can be seen as parallel to that described in Rogers’ process conception of psychotherapy (Rogers, 1967) .
The supervisor’s empathic presence facilitates the supervisee’s process of tuning in to whatever is around in their experiential process. Supervisees sometimes have only a vague sense of what is going on for them, there may be just a feeling of something on the edge of awareness; on those occasions a focusing response, making space for the experience, may be particularly helpful (Baljon, 2002; Madison, 2004). Worrall defined empathy as a central dimension in the therapeutic process, and therefore the primary task of supervision is ‘facilitating the supervisee’s capacity to offer empathic understanding’ (Worrall, 2001, p. 207). The supervisor’s congruent empathic stance in relation to the supervisee in turn fosters the supervisee’s ability for congruent empathic understanding of the client (Worrall, 2001).
The supervisor’s acceptance, the willingness to respect and value the supervisee as a person of worth, communicates trust in the supervisee to find their own unique way of relating to their client. The experience of the supervisor’s acceptance may enable the supervisee to allow into awareness experiences which are unacceptable or a threat to their self-concept (both personal and/or as a therapist) and may offer support towards a greater openness in their exploration of the therapeutic relationship. Supervision is not about monitoring or assessing practice, nor is it about ensuring that the supervisee works in a recognized, approved ‘person-centered’ way. The function of supervision is to support the supervisee in ‘finding her or his own style of being a therapist’ (Villas Bowen, 1986, p. 293). Against the background of this non-judgmental position the supervisor’s contribution to the supervisee’s exploration can be experienced as challenging, but nonetheless as ‘stimulating and enriching’ (Kilborn, 1999, p. 89). This challenge does not come from criticism, authority or judgment, but from a congruent, consistent acceptance that invites the supervisee to enter into an open, non-defensive reflection and to take responsibility for their own congruent self-exploration. A similar positive welcome to challenges is found in the work of Auckenthaler, a German psychotherapist, who described supervision as a place for critical exploration and dialogue in a supportive relationship. The combination of critical and benevolent attitude creates an ‘error friendly’ (fehlerfreundliche) context where both supervisor and supervisee can acknowledge errors, welcome their exploration, take responsibility and learn from them (Auckenthaler, 1995, p.156). Challenge also comes from the dynamic process of the relationship, from the struggle to understand and accept each other’s difference, from the interplay between moments of ‘understanding’ and ‘misunderstanding’ (Hanekamp, 2000; Wellendorf, 1982; Worrall, 2001).
Congruence is an essential dimension in the supervision relationship. To acknowledge fully the experience of being, or striving to be, in relationship with a client may leave the supervisee feeling exposed and vulnerable. In those moments, there may a conflict between the wish to be open to experience and the need for self-protection, and the supervisee may struggle with being congruent. Full engagement with the process of the supervisee is only possible if the supervisor is equally open to their own experience — congruent expression of empathy and acceptance gives depth and meaning to the relationship. It helps the supervisee to become more present and congruent; it also provides a model for their own practice (Lambers, 2000, p. 206). Transparent self-reflection and communication are important ingredients in the development of a collaborative relationship in supervision; congruence in the supervisor facilitates self-reflection in the supervisee and strengthens their internal locus of evaluation (Merry, 2001) and helps to create an excellent forum for the exploration of the ‘unspoken relationship’ (Mearns, 1991, p. 119) between supervisor and supervisee.
In order to make it possible for the therapist to explore the depth of their experience with the client, the supervisor in turn needs to be prepared to offer a high level of engagement and presence in the relationship with the supervisee.
Focus on the therapist’s experience: facilitating congruence
A consistent theme in the above-mentioned literature is that the purpose of person-centered supervision is defined in terms of the development of the therapist’s therapeutic abilities: the ability to be congruent, integrated and fully present in the therapeutic relationship. Rogers wrote about supervision as offering ‘primarily a listening, facilitative understanding, helping the therapist to become clearly aware of their own feelings in the therapeutic sessions so he can more adequately come to be himself in the relationship’ (Rogers, 1957, p. 86). Particularly relevant in his view of supervision was his wish to avoid ‘imitative modeling’ (Villas Bowen, 1986, p. 296), or of guiding or coaching the supervisee to respond according to the supervisor’s model (Rogers, 1957). Central in supervision is the experience of the therapist and the reflection on the relationship between client and therapist, rather than the exploration of a client ‘case’ (Hanekamp, 2000; Lambers, 2000; Mearns, 1997; Patterson, 1983). The supervision relationship offers a context where the therapist can bring into awareness experiences and processes emerging in the relationship with the client, and where the relationship qualities necessary for therapeutic work can be explored and developed. In that sense, the primary purpose of supervision is to facilitate the supervisee’s congruence in the relationship with the client (Lambers, 2000). Supervision offers a context for the exploration of a wide range of experiences: feelings of attraction or repulsion, sadness, anger, powerlessness, love, fear, hopelessness. The supervisee may bring barely symbolized sensations, on the edge of their awareness — a shivery feeling, a whiff of dread, of excitement, a sensation of tiredness. They may bring experiences that they are familiar with, but equally they may discover something completely new in the course of the supervision session.
However, this does not mean that the focus in supervision should be exclusively on the therapist’s feelings. There is a common misconception that person-centered work favors a focus on the exploration of feeling and on relating in the affective realm of experience (Lambers, 2000, p. 201; Worrall, 2001). The imposition of such a narrow focus, both in therapy and supervision, may in effect create a condition of worth: ‘to be accepted I must talk about feelings, or even better, show feelings’. Relating to the whole person involves relating to the person in all aspects of their experience and their existence. In supervision, focusing on the supervisee’s experience means being open to affective, cognitive, physical and spiritual dimensions, as well as to the exploration of professional and ethical questions. Defined in this way, the purpose of supervision is more than learning to apply what has been learned in training, as was suggested by Baljon, (2002) and Hanekamp (2000); rather, it is to meet the therapist/supervisee as the person that they are in the relationship with the client. The supervisor accompanies the supervisee in their experiencing of themselves in the relationship with the client.
DEVELOPING THE ABILITY TO RELATE AT DEPTH
Relational depth
In modern thinking about person-centered therapy increasing emphasis is placed on the centrality of the relationship. In this relationship, both client and therapist are touched by the experience of each other’s presence — what Schmid (2002) has referred to as the person-to-person encounter. The therapy relationship is essentially a human encounter, a meeting between two different people (Schmid, 2001), in which both people are affecting each other and are affected by each other. The therapeutic encounter offers both client and therapist the potential for a new experience of Self, and a powerful experience of Self in relation to another person. Mearns re-emphasized the importance of relationship in person-centered therapy as:
… not just relationship at the superficial level of the ‘therapeutic alliance’— my emphasis is on the creation of a particular depth of relationship between client and therapist — a depth of relationship which allows the client to feel an extraordinary safety, such that they can go to particular depths within their own experiencing. (Mearns, 2005)
Mearns and Cooper (2005) defined relational depth as
A state of profound contact and engagement between two people, in which each person is fully real with the Other, and able to understand and value the Other’s experiences at a high level. (p. xii)
To be able to offer such depth of relating the therapist needs to be integrated, fully present and free to respond to the total person of the client (Mearns, 2000, p. 87). Relating at depth requires an openness not only to all parts and dimensions of the client, but also to all aspects of the self of the therapist.
Being open to all of the Self- experience can be difficult and painful. It may involve experiencing fear and defensiveness, vulnerability, conflict, dissonance and disturbance when we meet that which we cannot accept in ourselves or when we experience the Other’s fear of meeting. In the therapeutic relationship the therapist meets this conflict in the client often in the form of difficult processes (Warner, 2000 , 2005); fragile and dissociative process, withdrawal or aggression, can be understood as expressions of conflict, distress and vulnerability experienced within the client, but they also have a relational meaning and a relational impact. Not only do they convey something of the client’s internal process, they also communicate something of the client’s experience in the therapy relationship. Meeting the client just as they are, in their fear, their distress and isolation, in their fear of relationship, can be a struggle and the therapist too may experience fear, defensiveness, disconnection or vulnerability (Lambers, 2003). In those moments we experience and share with the client their — and our — experience of being human, the need for relationship and the fear of meeting: ‘Even the person who appears pathologically to reject relationship is, in fact, governed by relationship — so governed, in fact, that he feels the need to protect himself from it’ (Mearns & Cooper, 2005, p. 160).
Developmental agenda
The challenge for the person-centered therapist is to become able to offer such openness and depth of relating to every client (Mearns & Cooper, 2005, p. 137). Initial training can only prepare the therapist for this challenge; after training the development of the ability to meet every client at depth is part of their ongoing developmental agenda. Supervision can play an important part in this process of development, both during and after training, not only through discussion of professional and practice issues, but also through providing a place where the self-development of the therapist can be attended to and where the humanity of the therapist is acknowledged (Mearns & Cooper, 2005).
In the conception of therapy as ‘personal encounter’ (Schmid, 2001) the therapist’s humanity is viewed as a trustworthy source for relationship. To be fully aware of our humanity and of the experiences and processes in ourselves may actually help the relationship with the client, may provide a bridge to the client’s experience. Supervision provides an excellent forum for exploration of these ‘existential touchstones’ described by Mearns and Cooper as:
events and self-experiences from which we draw considerable strength and which help to ground us in relationships as well as making us more open to and comfortable with a diversity of relationships … The therapist draws strength from their sense of what has been fundamental to their own existence as they engage with the client at relational depth. (Mearns & Cooper, 2005, p. 137)
These ‘touchstones’ have the potential to provide powerful points of connection with the client’s experience, yet, paradoxically, therapists are often encouraged to keep their own experiences separate in case they become a source of identification or over-involvement with the client. The path to relational depth is often through our own fallibility, fear, struggle, or through our own sense of our existence. Rather than take such experiences of profound recognition or connection exclusively to personal therapy, it would be of great value also to explore in supervision their importance as potentially rich resources for human and therapeutic relating. Supervision then offers an opportunity to acknowledge how the therapist’s unique self, their life experience and process of development can support the therapeutic process and the therapeutic relationship (Page & Wosket, 2002, p. 236; Mearns & Cooper, 2005; Villas Bowen, 1986).
RELATIONAL DEPTH AND SUPERVISION
Adapting the definition of relational depth (Mearns & Cooper, 2005, p. vii), relational depth in supervision can be described as:
a high level of contact and engagement in which both persons are contributing to a real dialogue around their shared experience in the moment — both of the supervisee’s experience of self in relation to the client and of the relationship between supervisee and supervisor.
Supporting the supervisee in the development of their ability to relate at depth, to be fully open to the experience of being ‘our own person’ and to be authentically present (Schmid, 2001) in the relationship with clients requires a high degree of presence and authenticity on the part of the supervisor. Just as in therapy, it is through the shared experience of relational depth that the supervisee can experience the safety that makes it possible for them to go to particular depth within their experiencing (Mearns, 2005).
Relational therapy is best supported by relational supervision. Such a supervision relationship offers the supervisee a real freedom to engage on all levels — affective, cognitive, spiritual — with their experience in relation to the client and allows them to define and pursue their own developmental agenda. To facilitate such openness the supervisor needs to be willing to be an active partner in the dialogue, without pre-conceived ideas about how the supervisee should conduct therapy and without the need to direct the supervisee’s self-development. Stated so succinctly by Villas Bowen, ‘the function of the supervisor is to create the atmosphere that will enable the supervisee to find his or her own style of being a therapist’ (Villas Bowen, 1986, p. 296)
As well as providing a place for reflection on therapeutic practice and the issues arising from that, supervision provides an additional unique opportunity for learning through exploration and reflection on the here-and-now experience of the relationship between supervisor and supervisee. Rather than exploring a mysterious parallel process, which inevitably leads to speculation about the absent client’s experience and motives, the actual, lived experience of the supervision relationship provides an excellent forum for exploration of the unspoken relationship (Mearns, 1991, p.119), of issues of power and difference, of the real difficulties in human relating. In such relational supervision, the supervisor is a partner rather than an educator; a witness rather than a monitor; a fellow explorer rather than a fount of wisdom.
CONCLUSION
If each therapy relationship is unique, the product of the unique meeting between two people, then the supervisor cannot possibly have knowledge of how this relationship should be conducted. The purpose of supervision then is not to monitor practice nor to guide the therapist towards a particular perspective; supervision can then only be about supporting the therapist in their effort and commitment to be fully present in the therapy relationship — to be as fully human as possible in the service of the client and the relationship (Keys, 2003, p. 2).
In a supervision relationship characterized by a high degree of openness and presence between supervisor and supervisee it may become possible for the supervisee to experience all dimensions of their experience in the relationship with the client. In such a supervision relationship the humanity that is at the core of the therapeutic encounter can be fully acknowledged and celebrated.

SUPERVISANDO LA HUMANIDAD DEL TERAPEUTA

Abstract. Este artículo ofrece una presentación general de una perspectiva sobre la supervisión, tal como se ha desarrollado en relación a la terapia centrada en la persona y experiencial. También explora la supervisión como una relación facilitadora enfocada en el desarrollo y el mantenimiento de la capacidad del terapeuta de relacionarse de manera congruente y en profundidad con sus consultantes. Trabajar en la relación de supervisión en profundidad relacional le permite a la persona supervisada ir con su propia experiencia a niveles de particular profundidad, y volverse así más plenamente integrada, presente y libre en su encuentro con la totalidad de la persona del consultante.
La mayoría de los terapeutas del ECP tienen experiencia en haber estado en supervisión. Para muchos, la supervisión es un requisito obligatorio tan solo durante su período de entrenamiento, pero en algunos países los terapeutas tienen la obligación de supervisar a lo largo de su vida profesional a modo de requerimiento ético y como condición para su reconocimiento profesional o su matriculación. De hecho, en Estados Unidos la supervisión es un elemento integral de los programas de capacitación para counseling, pero no existe requerimiento formal alguno en relación a la supervisión posterior al período de capacitación; en Austria, donde la actividad del counseling y la psicoterapia están reglamentadas, estar en supervisión continua es un requerimiento legal para la matriculación. En el Reino Unido, los terapeutas que son miembro de asociaciones de counseling y psicoterapia tienen la obligación de supervisar regularmente mientras estén en actividad. La supervisión se realiza por lo general en forma individual, especialmente durante el período de entrenamiento del terapeuta. En Holanda, el requerimiento de supervisión posterior al período de entrenamiento para los terapeutas del enfoque se cumple mediante supervisión regular de colegas y le llaman “intervision”.
La mayoría de la literatura e investigación acerca de la supervisión para counseling y psicoterapia está escrita desde una perspectiva genérica, buscando definir el propósito, la función y la práctica de supervisión en términos de modelos que sean aplicables a la supervisión de cualquier orientación terapéutica. Se pone el énfasis en el desarrollo de un marco de trabajo lo suficientemente flexible como para entender y describir tanto el proceso de supervisión como la relación de supervisión y el aprendizaje y desarrollo del supervisado y por ello corrernos de este enfoque específico de la supervisión solo resultaría útil para el terapeuta puro (Page & Wosket, 2001, p. 31). Carroll (1996) sugirió que la supervisión de por sí requiere ser comprendida desde la teoría y que exige diferentes habilidades de los counselors y psicoterapeutas. Quería correrse de los modelos de supervisión determinados por el couseling (en cuanto a que la supervisión está fuertemente ligada a la orientación del counselor supervisor) hacia modelos de supervisión de crecimiento y desarrollo del rol social (que comienzan con la situación de aprendizaje del supervisado) (Carroll, 1996, p.4). Y llegó aún más lejos al afirmar que ‘los modelos de supervisión determinados por el counseling con frecuencia enfatizan aspectos del trabajo de counseling que ignoran al consultante y se concentran en lo que le ocurre al supervisado’ (Carroll, 1996, p. 42).
PERSPECTIVAS ACERCA DE LA SUPERVISIÓN DENTRO DEL ECP
Gran parte de los primeros escritos acerca de la supervisión dentro del ECP, especialmente en Estados Unidos, se centró en la supervisión de terapeutas aprendices y estudiantes (Patterson, 1964, 1983; Rice, 1980; Rogers, 1942, 1951). Ocurre que el supervisor desempeña también el rol de capacitador, y entonces el desafío del supervisor del enfoque es el de integrar en la relación de supervisión elementos de facilitación y desarrollo con su responsabilidad de enseñar y evaluar. En contraste con los abordajes genéricos, se ponía especial énfasis en la importancia de la congruencia entre la orientación teórica y filosófica del supervisor y el supervisado (Hackney & Goodyear, 1984; Rogers, 1957; Schmid, 1997). ´El supervisor necesita conocer profundamente el enfoque centrado en la persona tanto en la teoría como en la práctica’ (Mearns, 1997, p. 84), o, tal como lo afirma Patterson de manera aún más enfática: ‘es importante que el supervisor y el supervisado estén avocados a la misma teoría’ (Patterson, 1983, p. 21) en pos de apoyar al supervisado en el desarrollo de la comprensión del enfoque y en relación con el proceso de integración de la filosofía fundamental.
Rogers (1951) era totalmente consciente de la importancia de la supervisión; constituía una parte importante de su trabajo con estudiantes. Para él, la supervisión tuvo también una función de investigación y como tal contribuyó al desarrollo del enfoque en sus primeros años de trabajo (Rogers, 1975). Al escribir acerca del entrenamiento de los terapeutas, afirmó que mediante la creación de una atmósfera genuina de aceptación y empatía, el supervisor crea el clima en el cuál el supervisado puede explorar sentimientos, obstáculos y dificultades que surgen mientras se va formando como terapeuta (Rogers, 1951). En relación con la supervisión, su énfasis en el supervisado era claro: ‘ayudar al terapeuta a crecer en relación con su auto-confianza y crecer en cuanto a la comprensión de sí mismo y crecer en relación con la comprensión del proceso terapéutico’ (Hackney & Goodyear, 1984, p. 283). Patterson adoptó una mirada similar enfatizando la importancia de las condiciones básicas de comprensión empática, respeto, transparencia y concreción en la relación de supervisión; consideraba que en la supervisión se trata de ‘desarrollar en el estudiante la sensibilidad de comprender las actitudes’ (Patterson, 1983, p. 25). Afirmaba que la supervisión estaba relacionada con la efectiva relación entre el supervisado y el consultante y no con el diagnóstico o la conceptualización de dinámicas particulares del consultante’ (Patterson, 1983, p. 24). Pone el foco en la experiencia del supervisado y no en la técnica o en el aprendizaje de determinadas respuestas específicas para dar al consultante.
Rice (1980) también escribió acerca de la supervisión en relación al entrenamiento de los terapeutas. Conceptualizó el proceso de supervisión como el balance entre la validación de la experiencia del supervisado en la genuina relación con el cliente y la validación de su idea de que la terapia es un proceso que se puede aprender. Distinguió dos grupos de actitudes relacionadas con la habilidad del terapeuta de poner en práctica las condiciones básicas de la terapia del enfoque centrado en la persona con las que se puede trabajar en la supervisión: actitudes acerca de la naturaleza humana y la naturaleza del cambio, y actitudes en pos del self (del terapeuta) (Rice, 1980, p. 138). Sin embargo, su abordaje parecía ser un tanto diferente del de Rogers y Patterson. Siendo una de las pensadoras fundadoras de la teoría del proceso experiencial, ella ponía mayor énfasis en el proceso del cliente y en el estilo con que el terapeuta participaba en tal proceso (Rice, 1980, p.137) dando menor importancia a la propia experiencia del supervisado en la relación con el cliente o en la relación de supervisión. También Barrett-Lennard en su escrito sobre la supervisión en el ECP puso el foco en la supervisión con terapeutas en etapa de aprendizaje (Barret-Lennard, 1998, pp. 333-338). También él intentó captar la esencia de la supervisión cuando la describió con crecimiento-aprendizaje (Barrett-Lennard, 1998, p. 333), facilitando una ‘evolución de la conciencia, no solo en relación con el self sino en cuanto a la receptividad de la experiencia de otros’ (Barrett-Lennard, 1998, p. 336). Como ex alumno de Rogers y con la experiencia de haber supervisado directamente con él, describe el movimiento de Rogers desde un estilo evaluativo y de guía en los primeros años de la década del ’40 hacia el estilo mucho más centrado en el alumnos y de facilitación después de 1951. Villas Bowen (1986) distinguió este abordaje de la supervisión que pone el énfasis en el desarrollo de la capacidad de evaluación del supervisado y que respeta la respuesta personal única del supervisado de la supervisión pre-determinada donde el supervisor está comprometido con una forma determinada de la terapia centrada en la persona y desalienta los estilos de expresión que no encajan en el modelo. Tal enfoque, en relación con la preservación de la forma pura de la terapia centrada en la persona es fundamentalmente inconsistente con el principio filosófico de base del enfoque centrado en la persona: la confianza en la capacidad de auto-dirección y auto-determinación (Villa Bowen, p. 293, 1986).
Para lograr el desarrollo y la integración de las habilidades terapéuticas de empatía, aceptación y congruencia en el supervisado, es necesario que éstas mismas habilidades estén presentes en la relación de supervisión. En ese sentido, la supervisión provee un modelo para la práctica del supervisado: no mediante la enseñanza de una manera de hacer terapia sino que, en concordancia con la teoría del la terapia centrada en la persona, ofreciendo un ambiente de crecimiento que ayudará al supervisado a encontrar su propio estilo (Villas Bowen, 1986, p. 296).
LA RELACIÓN DE SUPERVISIÓN
Una característica fundamental de todo lo escrito acerca de la supervisión desde la perspectiva de la teoría del ECP es el énfasis en el desarrollo de la habilidad terapéutica del supervisado mediante la relación de supervisión y en base a la experiencia del supervisado en la relación con el cliente.
Cualidades de la relación de supervisión: el significado de las condiciones básicas
Al igual que en la terapia, la empatía, la aceptación y la congruencia son cualidades importantes para la relación, validando y facilitando un clima de confianza mutua y respeto. En la supervisión éstas cualidades ayudan a crear una relación en la que supervisor y supervisado pueden trabajar juntos de manera creativa en pos de un diálogo auténtico con espíritu de colaboración (Ferry, 1999, p. 140) en un entorno abierto, empático y sin juicios de valor que es cuestionador y validante a la vez promoviendo la congruencia y un espacio interno de evaluación (Merry, 2001, p. 183). El concepto de Merry de la supervisión como una especie de ‘proyecto de investigación personal’ (Merry, 2004, p. 193) concuerda con el espíritu de la visión de Rogers de la terapia centrada en la persona como un proceso de comprobación de la hipótesis central del enfoque. En un clima tal, el supervisado se siente seguro y libre para dar expresión a los pensamientos, sentimientos y preocupaciones que se encuentran en su sentido conciente, para enfocarse en las experiencias aún no simbolizadas en relación con sus clientes, o para abordar inquietudes y cuestiones relacionadas con factores externos que tengan influencia en la relación terapéutica. Mediante la experiencia de ser completamente recibido en la relación de supervisión, el supervisado tiene la posibilidad de volverse más fluido y más abierto tanto a su propia experiencia como a la experiencia de su cliente y comunicarse de manera más abierta y más libre (Tudor & Worrall, 2004), en un proceso que puede concebirse como paralelo al proceso descripto en la concepción de proceso de la psicoterapia de Rogers (Rogers, 1967).
La presencia empática del supervisor facilita el proceso de ajuste del supervisado en cuanto a su proceso experiencial. A veces, los supervisados solo tiene una mínima noción de lo que les está ocurriendo; puede que solo tengan una sensación al borde de la conciencia; en esos casos una respuesta focusing que habilite un espacio para la experiencia puede ser muy útil (Balion, 2002, Madison, 2004). Worral definió la empatía como una dimensión fundamental del proceso terapéutico, y por lo tanto una tarea primordial de la supervisión es la de facilitar el desarrollo de la capacidad de comprensión empática en el supervisado (Worrall, 2001, p. 207). La actitud empática y congruente del supervisor en relación al supervisado alienta en el supervisado la habilidad de una congruente comprensión empática del cliente (Worral, 2001).
La aceptación del supervisor, la voluntad de respetar y valorar al supervisado como una persona valiosa, transmite al supervisado confianza para encontrar su propio y único modo de relacionarse con el cliente. La experiencia de sentirse aceptado por el supervisor permite al supervisado hacer concientes aquellas experiencias que son inaceptables o que constituyen una amenaza para su auto-concepto (tanto en lo personal como en lo profesional) y puede alentar una mayor apertura en la exploración de su relación terapéutica. La supervisión es para monitorear o enseñar la práctica, ni tampoco para asegurarse de que la manera de trabajar del supervisado concuerda con la manera reconocida y aprobada por el ECP. La función de la supervisión es dar apoyo al supervisado para que pueda encontrar su propio estilo de trabajo como terapeuta (Villas Bowen, 1986, p. 293). En contraste con esta postura carente de juicios de valor, la contribución del supervisor al proceso del exploración del supervisado puede resultar desafiante pero sin embargo estimulante y enriquecedora (Kilborn, 1999, p. 89). Este desafío no proviene de la crítica, autoridad o juicio, sino de una aceptación congruente y consistente que invita al supervisado a entrar en una reflexión abierta sin estar a la defensiva y a asumir la responsabilidad por su propia auto-exploración congruente. Encontramos una postura similar que le da la bienvenida a los desafíos en el trabajo de Auckenthaler, un terapeuta alemán quien describió la supervisión como el espacio para la exploración crítica y el dialogo en una relación validante. La combinación de una actitud benevolente y crítica a la vez crea un contexto de ‘error amigable’ (fehlerfreundliche) donde tanto el supervisor como el supervisado son capaces de reconocer sus errores, dar la bienvenida a la exploración y asumir la responsabilidad de aprender de tales errores (Auckenthaler, 1995, p. 156). El desafío proviene también del proceso dinámico de la relación, de la lucha por comprender y aceptar las diferencias del otro y de la interacción entre los momentos en los que se logra un buen entendimiento y los momentos en los que no (Hanekamp, 2000; Wellendork, 1982; Worral, 2001).
La congruencia es una dimensión esencial en la relación de supervisión. El total reconocimiento de la experiencia de ser, o de intentar ser, en la relación con el cliente puede hacer que el supervisado se siente expuesto y vulnerable. En tales momentos, puede haber un conflicto entre el deseo de ser abierto a la experiencia y la necesidad de auto- protección y entonces es posible que el supervisado se encuentre luchando para sostener su congruencia. El total compromiso con el proceso del supervisado solo se logra si el supervisor está igualmente abierto a su propia experiencia – la expresión congruente de una actitud empatía y aceptación otorga profundidad y sentido a la relación. Ayuda al supervisor a tener más presencia y más congruencia; también ofrece un modelo para su propia práctica (Lambers, 2000, p. 206). Una auto-reflexión transparente y una buena comunicación son ingredientes importantes en el desarrollo de una relación de supervisión colaboradora; la congruencia del supervisor facilita la auto-reflexión en el supervisado y fortalece su espacio interior de evaluación (Merry, 2001), también ayuda a crear un espacio excelente para la exploración de la relación no verbal (Mearns, 1991, p. 119) entre el supervisor y el supervisado.
Para posibilitar al terapeuta la exploración de la profundidad de su experiencia con su cliente, es necesario que el supervisor esté preparado para ofrecer al supervisado un nivel superior de compromiso y presencia en la relación.
Poniendo el foco en la experiencia del terapeuta: facilitando la congruencia
Un tema recurrente en la bibliografía mencionada anteriormente es el tema de que el propósito de la supervisión en el ECP se define en relación con el desarrollo por parte del terapeuta de las habilidades terapéuticas: la habilidad de ser congruente y de estar integrado y totalmente presente en la relación terapéutica. Rogers escribió acerca de la supervisión como el ofrecimiento de un particular entendimiento facilitador y de escucha, ayudando al terapeuta a volverse claramente consciente de sus propios sentimientos en las sesiones terapéuticas de modo tal que pueda llegar a ser él mismo en la relación (Rogers, 1957, p. 86). En su postura de la supervisión, es especialmente importante destacar su deseo de evitar los ‘modelos a imitar’ (Villas Bowen, 1986, p. 296), o el entrenamiento del supervisado para que responda de manera acorde al modelo del supervisor (Rogers, 1957). Es primordial para la supervisión tener en cuenta la experiencia del terapeuta y su repercusión en la relación con el cliente, mucho más que el análisis del ‘caso’ del cliente (Hanekamp, 2000; Lambers, 2000, Mearns, 1997; Patterson, 1983). La relación de supervisión ofrece un contexto en el que el terapeuta puede volver conscientes ciertas experiencias y procesos que surgen de la relación con el cliente, y en el que las habilidades relacionales necesarias para el trabajo terapéutico pueden ser exploradas y desarrolladas. En tal sentido, el principal propósito de la supervisión es el de facilitar la congruencia del supervisado en la relación con el cliente (Lambers, 2000). La supervisión ofrece un contexto apto para la exploración de un amplio espectro de experiencias: sentimientos de atracción y repulsión, tristeza, enojo, impotencia, amor, temor, desesperanza. Es posible que el supervisado tenga sensaciones apenas simbolizadas, al borde de la consciencia – un sentimiento escalofriante, una bocanada de espanto, exitación, o una sensación de cansancio. Puede tener experiencias que le resultan familiares, pero puede también descubrir algo completamente nuevo en el transcurso de la sesión de supervisión.
Sin embargo, esto no significa que en la supervisión ha de ponerse el foco exclusivamente en los sentimientos del terapeuta. Es habitual que se malinterprete la postura del ECP considerando que favorece el foco en la exploración de los sentimientos y de la manera de relacionarse en el ámbito afectivo de la experiencia (Lambers, 2000, p. 201; Worral, 2001). La imposición de una visión tan acotada, tanto en la terapia como en la supervisión, puede en efecto crear un cieto condicionamiento: ‘para ser aceptado, debo hablar de mis sentimientos, o mejor aún, mostrar mis sentimientos’. El relacionarse con la totalidad de la persona implica relacionarse con la persona en todos los aspectos de su experiencia y de su existencia. En supervisión, poner el foco en la experiencia del supervisado significa estar abierto a las dimensión afectiva, cognitiva, física y espiritual así como también a la exploración de cuestiones tanto personales como profesionales. En este sentido, el propósito de la supervisión es más que el de aprender a aplicar lo aprendido durante el entrenamiento, como sugería Balion, (2002) y Hanekamp (200); sino el de conocer al terapeuta supervisado como la persona que es en la relación con el cliente. El supervisor acompaña al supervisado en la experiencia de ser él mismo en la relación con el cliente.
DESARROLLANDO LA HABILIDAD DE RELACIONARSE EN PROFUNDIDAD
Profundidad Relacional
En el pensamiento moderno acerca del ECP se pone cada vez más énfasis en la importancia de la relación. En esta relación, tanto el cliente como el terapeuta se ven afectados por la experiencia de la presencia del otro – lo que Schmid (2002) denominó el encuentro persona- a-persona. La relación terapéutica se trata fundamentalmente de encuentro humano, un encuentro entro des personas diferentes (Schmid, 2001) en el que ambas se están afectando mutuamente. El encuentro terapéutico ofrece tanto al cliente como al terapeuta el potencial para una nueva experiencia del Self y una experiencia poderosa del Self en relación con otro. Mearns volvió a enfatizar la importancia de la relación en la terapia de enfoque, con la siguiente expresión:
“… no solo la relación al nivel superficial de ‘alianza terapéutica’ – pongo el énfasis en la creación de una particular profundidad relacional entre el cliente y el terapeuta – una profanidad relacional que permite al cliente sentir una seguridad extraordinaria, tal que pueden ir a ciertas profanidades dentro de su propia experiencia”. (Mearns, 2005)
Mearns y Cooper (2005) definieron la profundidad relacional como
Un estado de encuentro profundo y compromiso entre dos personas, en el que cada una es totalmente real con la otra, y capaz de comprender y valorar la experiencia de la otra a un nivel superior (p. xxi).
Para ser capaz de ofrecer tal profundidad de relación el terapeuta necesita estar integrado, completamente presente y libre para responder a la totalidad de la persona del cliente (Mearns, 2000, p. 87). Para relacionarse en profundidad se requiere apertura no solo hacia todas las partes y dimensiones del cliente, sino también hacia todos los aspectos del self del terapeuta.
El hecho de estar abierto a toda la experiencia del self puede resultar difícil y doloroso. Es posible que implique la experiencia de miedo y actitud defensiva, vulnerabilidad, conflicto, disonancia y disturbio cuando nos encontramos con que no podemos aceptarnos o cuando experimentamos el miedo del otro por el encuentro. En la relación, el terapeuta se encuentra con este conflicto en el cliente por lo general en la forma de procesos difíciles (Warner, 2000, 2005); proceso frágil y disociativo; aislamiento o agresión que pueden ser interpretados como expresiones de conflicto, angustia y vulnerabilidad experimentados en el cliente, pero también tienen un significado y un impacto dentro de la relación. No solo nos transmiten algo acerca del proceso interno del cliente, sin oque también nos comunican algo acerca de la experiencia del cliente dentro de la relación terapéutica. Encontrarnos con el cliente tal cual es, en su temor, su angustia y su aislamiento, en su temor por relacionarse, puede resultar en un gran esfuerzo y el terapeuta también es posible que experimente temor, necesidad de defenderse, desconexión o vulnerabilidad (Lambers, 2003). En tales momentos, experimentamos y compartimos con el cliente su experiencia y nuestra experiencia como seres humanos, la necesidad de relacionarnos y el temor por el encuentro. ‘Aún la persona que pareciera sufrir una patología que le provoca rechazo para relacionarse está, de hecho, regida por la relación – tan controlada que siente la necesidad de protegerse de ella’. (Mearns & Cooper, p. 160)

El desafío para el terapeuta del ECP es el de ser capaz de ofrecer tal apertura y profundidad relacional a cada cliente (Mearns & Cooper, 2005, p. 137). El entrenamiento inicial solo prepara al terapeuta para este desafío; después del entrenamiento, la habilidad de encontrarse con cada cliente en profundidad es parte de su sucesiva agenda de desarrollo. Es posible que la supervisión juegue un importante papel en el proceso de desarrollo, durante y después del entrenamiento, no solo mediante la discusión profesional de cuestiones de la práctica, sino también mediante el ofrecimiento de un espacio para considerar el auto-desarrollo del terapeuta , donde se reconozca su humanidad (Mearns & Cooper, 2005).
En la definición de terapia como “encuentro personal” (Schmid, 2001) se concibe la humanidad del terapeuta como una fuente de confianza en la relación. El estar totalmente conscientes de nuestra humanidad y de nuestras experiencias y procesos puede ser realmente útil para la relación con el cliente; puede significar un puente hacia la experiencia del cliente. La supervisión provee un excelente espacio para la exploración de éstas significativas cuestiones existenciales descriptas por Mearns y Cooper como:
eventos y experiencias personales de las que sacamos considerable fuerza y que nos hacen de cable a tierra en las relaciones y que también nos hacen más abiertos y más tolerantes en gran variedad de relaciones … El terapeuta saca fuerza de su sentido de lo que ha sido fundamental para su propia existencia a medida que se compromete con el cliente en profundidad relacional (Mearns & Cooper, p. 137).
Éstas cuestiones significativas tienen el potencial de ofrecer importantes puntos de conexión con la experiencia del cliente, y aún así, paradójicamente, los terapeutas son con frecuencia alentados a mantener sus propias experiencias al margen en caso de que se conviertan en una fuente de identificación o involucramiento excesivo con el cliente. El camino hacia la profundidad relacional es con frecuencia nuestra propia falibilidad, temor, lucha, o nuestro propio sentido de existencial. En lugar de llevar tales experiencias de profundo reconocimiento y conexión exclusivamente al espacio de terapia personal, sería de gran valor que también se pudiera analizar en la supervisión su importancia como potenciales recursos valiosos para las relaciones humanas y terapéuticas. Entonces, la supervisión ofrece una oportunidad para reconocer la manera en que el self único del terapeuta, su experiencia de vida y proceso de desarrollo pueden contribuir al proceso y a la relación terapéutica (Page & Wosket, 2002, p. 236; Mearns & Cooper, 2005; Villas Bowen, 1986).
PROFUNDIDAD RELACIONAL Y SUPERVISIÓN
Si adaptamos la definición de profundidad relacional (Mearns & Cooper, 2005, p.vii), definimos la profundidad relacional en supervisión como:
un nivel superior de contacto y compromiso en el cual ambas personas están contribuyendo a una diálogo real en torno de sus experiencias compartidas en el momento- tanto la experiencia del self del supervisado en cuanto a la relación con el cliente como en cuanto a su relación con el supervisor.
El apoyo al supervisado en el desarrollo de su habilidad para relacionarse en profundidad, para estar totalmente abierto a la experiencia de ‘ser su propia persona’ y estar auténticamente presente (Schmid, 2001) en la relación con los clientes requiere un alto grado de presencia y autenticidad por parte del supervisor. Tal como en la terapia, es a través del compartir de experiencias en profundidad relacional que el supervisado puede sentir la seguridad que les permite meterse a cierta profundidad particular dentro de sus experiencias (Mearns, 2005).
La relación terapéutica se nutre de la relación de supervisión. La relación de supervisión ofrece al supervisado la libertad real de comprometerse en todos los niveles – afectivo, cognitivo, espiritual - con su experiencia en relación con el cliente y le permite definir y procurar su propia agenda de desarrollo. Tal como lo expresa sucintamente Villas Bowen: “la función del supervisor es la de crear el clima que permitirá al supervisado encontrar su propia estilo de ser terapeuta” (Vilas Bowen, 1986, p. 296).
Además de ofrecer un lugar de reflexión acerca de la práctica terapéutica y las cuestiones que puedan surgir de ésta, la supervisión ofrece una oportunidad única para aprender a través de la exploración y la reflexión de la experiencia de la relación en el aquí y ahora entre supervisor y supervisado. Más que la exploración de un proceso paralelo misterioso que inevitablemente conduce a especulaciones acerca de la experiencia y las razones del cliente que está ausente, la experiencia real y en vivo de la relación de supervisión ofrece un lugar excelente para la exploración de la relación no-verbal (Mearn, 1991, p. 119), de cuestiones de poder y diferenciación, de las verdaderas dificultades en las relaciones humanas. En tal relación de supervisión, el supervisor es un socio más que un profesor, un testigo más que un guía, un compañero explorador más que una fuente de sabiduría.
CONCLUSIÓN
Si cada relación terapéutica es única, en tanto el producto de un encuentro único entre dos personas, entonces no es posible que el supervisor tenga la información de cómo debería llevarse a cabo tal relación. El propósito de la supervisión no es entonces el de monitorear la práctica ni guiar al terapeuta hacia cierta perspectiva; la supervisión se trata simplemente de apoyar al terapeuta en su esfuerzo y compromiso de estar totalmente presente en la relación terapéutica – de ser lo más humano posible y estar al servicio del cliente y de la relación (Keys, 2003, p. 2).
En una relación de supervisión caracterizada por un alto grado de apertura y presencia entre el supervisor y el supervisado es posible que el supervisado experimente todas las dimensiones de su experiencia en la relación con el cliente. En tal relación de supervisión, la humanidad que es el elemento primordial del encuentro terapéutico podrá ser totalmente reconocida y celebrada.

In this paper the author searches for common ground between the person-centered approach and general psychiatry with respect to borderline personality disorder. Acknowledging pathological aspects of the person is not necessarily in conflict with the basic principles of the person-centered approach. New developments in person-centered therapy open up perspectives to work with people with a borderline personality disorder. This paper is the second in a series of three concerning the person-centered approach to severe mental illness.

Abstract. Pre-Therapy aims at stimulating psychological contact in persons suffering psychosis. We offer a review of Pre-Therapy research instruments and findings. The Pre-Therapy Rating Scale (PTRS; Prouty, 1994) and the Evaluation Criterion for the Pre-Therapy Interview (ECPI; Dinacci, 1997) have been the two most frequently used instruments for the assessment of psychological contact. PTRS scores seem more reliable than ECPI scores, but all manuals need revision. Particular attention is needed for the rating of non-verbal behavior. A preliminary evaluation of the structure of the PTRS indicates that it is two-dimensional rather than three-dimensional. The PTRS and the ECPI can be regarded as measures of communicative contact but also as measures of the meaningfulness of communication. Preliminary outcome studies suggest that pre-post and comparative effect sizes of Pre-Therapy are large for communicative contact, but the number of participants in these studies is generally low, as is the number of systematic case studies.

Drawing on Rogers’ later work, it is argued that person-centered therapy could well be regarded as a form of ‘atheistic spirituality’. This type of spirituality is explained and shows itself to be compatible with the basic humanistic values and notions of person-centered therapy and with its inherent scientific attitude. It is argued that such a view of person-centered therapy might provide a fresh perspective on its identity and future evolution.

Usando la concepción de voces internas del Modelo de Integración junto con la teoría centrada en la persona, sugerimos que las condiciones de valor pueden brindar los rastros de la experiencia que componen las voces problemáticas. En nuestro análisis identificamos tres voces: una voz criticada sumisa y tímida voz y dos voces críticas problemáticas - una expresando ira abiertamente y la otra internamente. Describimos los cambios en sus relaciones a medida que las voces problemáticas avanzaban hacia convertirse en recursos para la persona.

En este escrito el autor busca un campo en común entre el enfoque centrado en la persona y la psiquiatría general con respecto al desorden bordeline de personalidad.
Reconocer los aspectos patológicos de la persona no necesariamente está en conflicto con los principios básicos del enfoque centrado en la persona. Nuevos desarrollos en la terapia centrada en la persona abren nuevas perspectivas para trabajar con personas que sufren un desorden borderline de la personalidad. Este es el segundo de una serie de tres escritos acerca de un enfoque centrado en la persona para enfermedades mentales severas.

La escala de clasificación de la Pre-terapia (PTRS; Prouty, 1994) y el criterio de evaluación para la entrevista de Pre-terapia (ECPI; Dinacci, 1997) han sido dos de los instrumentos más frecuentemente utilizados para evaluar el contacto psicológico. Los resultados del PTRS parecen ser más confiables que los resultados del ECPI, pero todos los manuales necesitan revisiones. Se necesita prestar especial atención a la clasificación del comportamiento no verbal. Una evaluación preliminar de la estructura del PTRS indica que es bi-dimensional y no tri-dimensional. Tanto el PTRS como el ECPI pueden ser considerados como instrumentos para medir el contacto comunicativo pero también como evaluación de cuán significativa es la comunicación. Sentido en un Universo sin Dios: Un desafío para la terapia centrada en la persona

Abstract. La terapia centrada en la persona se presenta como una filosofía práctica para la vida, lo cual es especialmente adecuado para aquellos que están buscando el sentido de la vida, pero ya no se sienten inspirados por el pensamiento y la práctica religiosa tradicionales, y a la vez no desechan la espiritualidad. Basándose en la obra de los últimos tiempos de Rogers, se sostiene que la terapia centrada en la persona puede ser considerada como una forma de “espiritualidad atea”. Este tipo de espiritualidad se explica y se muestra a sí misma como compatible con los valores básicos humanistas y las nociones de la terapia centrada en la persona y su inherente actitud científica. Se sostiene que esta mirada de la terapia centrada en la persona puede brindar una perspectiva fresca a su identidad y futura evolución.

La pulmonía redonda es una entidad bien conocida en niños. En adultos, el solamente 1% de pulmonías presentadas como lesiones redondas. De hecho, menos el de 1% de la lesión de la moneda en un adulto la radiografía del pecho es pulmonía redonda. Generalmente, la pulmonía redonda mímico pulmonar o mediastinal las masas, por lo tanto una diagnosis diferenciada pudieron ser consideradas.
El objetivo de esta comunicación es divulgar dos casos de pulmonía redonda en los adultos (uno de ellos debido a C.psitacci), su presentación radiográfica, evolución y resultado.
También se incluye una revisión de la literatura en este tema.

Introducción
Poco se sabía sobre pulmonías redondas hasta 1973, cuando Rose dio la alarma de esta
característica a la comunidad médica. Divulgaron 21 casos de pulmonía esférica adentro
los niños químico pulmonar y el mediastinal se forma (1)
En 1988, la variante esférica fue incluida en una revisión por Griscon. (2). Desde entonces este tipo de pulmonía se ha reconocido como entidad patológica y se ha incluido dentro de los capítulos de infecciones respiratorias en los niños (3)
El cerca de 1% de las pulmonías del adulto presentadas como lesiones redondas (4). Pulmonía redonda abarca menos el de 1% de la lesión de la moneda en la radiografía del pecho (5.6).
Divulgamos dos casos de la pulmonía redonda en adultos, uno de ellos relacionados con C. psittaci. A nuestro conocimiento, este patógeno etiológico no había sido descrito previamente como causa de la pulmonía redonda.
Informes del caso
Caso 1:
Las 24 mujeres de los años fueron admitidas por fiebre seca y tos de un día de evolución en la admisión, su temperatura era de 39° C, su ritmo respiratorio 36/min y su ritmo cardíaco 120/min. El resto de su examen físico no presentaba nada notable. Su conteo de células en sangre reveló 19.000 células blancas. La radiografía del pecho reveló la presencia de una masa en el lóbulo derecho superior y la ampliación hiliar (figura 1a). El antecedente de un contacto cercano reciente con un loro fue recuperado. El examen del esputo y los cultivos bacteriológicos de la sangre eran negativos. A C. psittaci la prueba de fijación de complemento era 1/64 positivo mientras que ella recibía eritromicina.
En una semana después la radiografía de control del pecho demostró una mejora (figura 1b) y ella fue dada de alta como asintomático. El paciente rechazado al practicársele una segunda muestra de la sangre para C. psittacci serología durante su convalecencia.
Case2:
Una mujer de 34 años fue admitida en hospital debido a fiebre, dyspnea y esputo hemotópico. Ella refirió estar en buen estado general en las 48 hs. Anteriores a la admisión. No se obtuvo ninguna historia anterior de la enfermedad de pulmón de su historial médico. El examen físico demostró crujidos inspiratorios en el pulmón derecho superior. La cuenta de células blancas en sangre era 20.300 con una cambio a la izquierda. La radiografía del pecho demostró una masa en el lóbulo derecho superior (figura 2a); una exploración de CT confirmó esto y divulgó el broncograma del aire en la masa (figura 2b). Se comenzó la aplicación de Ampicilina-sulbactam más clarythromycin. Tres días más adelante, un infiltración se vio una imagen difusa progresando en una nueva radiografía del pecho (figura 2c). Un broncoscopía fibro-óptico fue realizada y la obstrucción bronquial fue eliminada. Los cultivos del esputo y de la sangre y los estudios serológicos dieron negativos. Una segunda exploración de CT realizó una semana después de la admisión (calcule el 2.o) divulgando pocas imágenes cavitarias situadas en el lóbulo derecho superior. Dieron de alta al paciente después de 11mo. día de la hospitalización.
Revisión y discusión de la literatura
La pulmonía común-adquirida (CASQUILLO) es un problema de salud importante por todo el mundo.
En los Estados Unidos, cuatro millones de pacientes desarrollaron el CASQUILLO cada año y entre 600 000 a 1.000.000 de ellos requirió la hospitalización. (7). El CASQUILLO es el sexto más común total causa de muertes en los Estados Unidos y la causa principal de la muerte de un infeccioso enfermedad en ese país. (8)
La gerencia del CASQUILLO en adultos inmunocompetentes ha sido revisada recientemente por Mandell y otros. (9) y Niederman y otros (10)
En los años pasados, varias tentativas de desarrollar las cuentas pronósticas válidas para clasificar el CASQUILLO acordando su severidad se han hecho los grados (11) (12)
Solamente en 10-15% de las cajas de CASQUILLO, el inicio puede seguir un patrón del hyperacute en el cual hay un solo episodio de las frialdades que duran por 5 a 15 minutos, siguiendo por fiebre sostenida y tos.
La presentación aguda es más común en el CASQUILLO causado cerca S. pneumoniae, H. influenzae mecanografíe B, o S.pyogenes. (13)
En adultos, una variedad de patrones radiográficos se puede observar, incluyendo bronchopneumonia, intersticial difuso infiltra y consolidación lobar.
A pesar de autores numerosos tenga describir los patrones radiográficos distintivos para el diferente los agentes etiológicos individuales (14), la opinión actual son que la mayor parte de las infecciones bacterianas resultados radiográficos similares de la causa.
Sin embargo, muchos autores consideraban que hay algunos patrones generales, que pueden ser aplicado a los agentes específicos. En este respeto, una consolidación focal pudo ser asociada con pulmonía pneumococcal, Klebsiella pneumoniae , S.Aureus, Hemophilus - influenzae, M.pneumoniae y C.pneumoniae, mientras que intersticial infiltra debe levantar la suspicacia de viral pulmonía e infección debido a M.pneumoniae, C.pneumoniae, Chlamydia Psittaci y Pneumocystis Carinii.(15)
En la mayoría de cajas de CASQUILLO, las radiografías de pecho revelan un área que implica menos que lleno segmento del pulmón (16)
En pacientes con bacteremia pneumococcal, una consolidación homogénea que implica un lleno el segmento o el lóbulo se puede detectar en una mayor proporción.
La pulmonía Hematogenous debido al estafilococo Aureus es manifestada por el redondo discreto lesiones, (clasificadas 1 a 3 centímetros de diámetro), a menudo con cavitaciones.
La infección viral, mycoplasmal y rickettsial no causa generalmente segmentario
consolidación. A el patrón de desigual, con la implicación multifocal se observa a menudo. Esto el patrón puede progresar a un intersticial difuso infiltra. (15)
La pulmonía redonda es una forma inusual de presentación de la pulmonía.
Este patrón raro puede ser explicado parcialmente en base de cómo una radiografía del pecho se obtiene temprano después del inicio de los síntomas (17). La relación temporal entre la radiografía del pecho más temprana y la presencia de lesiones redondas en adultos se relaciona probablemente con la velocidad de la extensión por los poros de Kohn y los canales de Lambert, lentamente en niñez, según lo explicado por Zinkermagel y otros. (4).
Durning y otros revisados dieciocho casos de la pulmonía redonda en adultos y acentúa dificultades actuales para reconocer un microorganismo patógeno en la etiología de la pulmonía redonda. (18).
En nuestro paciente (caso 2) hemos divulgado el resultado del patrón del radiographics según lo descrito. En algunos días, el pecho - rayo de x desarrollado en un lóbulo típico infiltre.
La mayoría de los informes anteriores alrededor la pulmonía redonda demuestra la resolución radiográfica completa en el plazo de un mes de la diagnosis.
Las complicaciones pulmonares, como absceso o empyema, no se han divulgado como complicación de la pulmonía redonda. Dos de los 18 patiens divulgaron cerca Durning murió.
En 14 de los 18 casos, el encontrar no microbiologic o serologic podía ser detectado. Sin embargo, todos los pacientes excepto uno, resuelto su enfermedad con terapia antibiótica convencional.
El Dr. Antón criticó este punto; su posición es que la fiebre de Q debe ser eliminada en cualquier pulmonía redonda y el tratamiento debe incluir los macrolides o los fluorquinolones (19).
La presencia del bronchogram del aire en la exploración de CT, (pues observamos en nuestro caso 2), se ha descrito más comúnmente en el malignancy (65-87%) que con las masas benignas (50-55%). Esto que encuentra no contribuye para distinguir entre el cáncer y la pulmonía redonda. (5)
En revisiones anteriores, S. pneumoniae, K.pneumoniae, Hemophilus - influenzae, C.burnetti y M. tuberculosis fueron reconocidos como la causa de la pulmonía redonda (4). Recientemente, sabíamos cerca de ocho casos de síndrome respiratorio agudo severo que fueron presentados como presentación como pulmonía redonda (17).
En nuestro primer caso, la reacción serologic positiva 1/64 en un análisis de sangre aislado durante la fase aguda de la pulmonía y de los datos epidemiológicos, haga posible la etiología de C. psittaci; La pulmonía redonda previamente no se ha descrito relacionada a C. psitacci.. (20) Creemos que nuestro informe pudo contribuir para aumentar el conocimiento en la diagnosis de esta presentación inusual de una enfermedad común.
Según lo sugerido cerca Durning, “uno debe considerar la diagnosis de la pulmonía redonda en un paciente del adulto que presente con una masa pulmonar, especialmente si él o ella tiene síntomas de la infección de la zona respiratoria, sea un no fumador joven, y no tenga ningún otro resultado para sugerir malignancy”.
