Source text - Polish Wraz z udoskonaleniem techniki wymiany płynu dializacyjnego, zmniejszona została ilość dializacyjnych zapaleń otrzewnej, powikłania, które przez wiele lat ograniczało długoletnie przeżycie pacjentów dializowanych otrzewnowo. W związku z wydłużeniem czasu przeżycia pacjentów dializowanych otrzewnowo, zaczęto dostrzegać inne, niż infekcyjne, zagrożenia. Zagadnienie odpowiedniej dawki dializy, poprawa stanu odżywienia, unikanie efektów działania bioniezgodnego płynu dializacyjnego, zapobieganie niekorzystnym przesunięciom w profilu lipidowym, a także poszukiwanie alternatywnych dla glukozy źródeł substancji osmotycznie czynnych, to teraz główne kierunki rozwoju tej metody leczenia. Jednym z powikłań o udowodnionym związku ze zwiększoną chorobowością i śmiertelnością pacjentów leczonych nerkozastępczo jest niedożywienie. Powikłanie to w większym stopniu dotyczy chorych leczonych dializą otrzewnową, niż pacjentów hemodializowanych. Wśród przyczyn niedożywienia swoistych dla dializoterapii otrzewnowej, wymienić należy zmniejszenie apetytu w wyniku uczucia dyskomfortu w jamie brzusznej spowodowanego obecnością co najmniej 2000 ml płynu dializacyjnego i wchłaniania z płynu glukozy, utratę do płynu dializacyjnego białek, albumin, aminokwasów oraz witamin, gastroparezę często występującą u chorych z cukrzycą (stanowią oni ¼ pacjentów dializowanych otrzewnowo), a także brak biozgodnych płynów dializacyjnych. Dominującą formą niedożywienia jest niedożywienie białkowo-energetyczne, choć w porównaniu do grupy chorych leczonych powtarzanymi hemodializami, zaburzenia białkowe są znacznie bardziej nasilone.
Translation - English With improving technique of dialysis fluid exchange, the number of peritonitis episodes has decreased. This complication limited for many years long-term survival of patients treated with peritoneal dialysis. Because of improving life expectancy of peritoneal dialysis patients, different complications, other than infectious, have come to light. The question of adequate dialysis dose, improvement of nutritional status, avoid of adverse effects of non-biocompatible dialysis fluid, prevention of lipid disorders and research of new osmotic agents, alternative for glucose, are now the main developmental directions of this treatment method. There is a proven relationship between malnutrition and co-morbidity among patients treated with renal replacement therapy. This complication applies to a larger extent to peritoneal dialysis patients than to patients on HD. Specific causes of malnutrition among patients on PD are: appetite suppression because of discomfort in the peritoneal cavity (presence of 2000 ml dialysis fluid) and absorption of glucose, loss of proteins, albumin, amino-acids and vitamins into dialysate, gastroparesis (especially in patients with diabetes mellitus) and lack of biocompatible fluids. The main type of malnutrition is energy-protein malnutrition; nevertheless patients treated with HD have much more severe protein disorders.
Early recognition of malnutrition in the group of patients treated with PD can be a key factor in successful treatment with using this form of therapy.
