Description
1000, 3000 and 5000 ml XYZ® bags containing sterile and non-pyrogenic solution for
irrigation.
Uses
This solution is nonhaemolytic and without electrolytes and is used for irrigation of the
cutting surface of the prostate to enable the surgeon to have good visibility of the
operating field during transurethral resection of prostate surgery.
Warnings
1. Not for intravenous administration.
2. Do not use unless solution is clear and container is undamaged.
3. Discard unused contents of opened containers.
4. Do not vent.
5. Microwave heating of fluids is not recommended.
6. The solution is not to be taken orally.
Cautions
Excessive volumes of irrigation fluid entering the systemic circulation should be
systematically considered in case of:
1. Non-reactive bilateral mydriasis (attesting the systemic passage of glycine) during
general anesthesia.
2. Combination of the following clinical signs: nausea, headache, somnolence,
excitement, confusion, blurred vision or amaurosis during local/regional anesthesia
(attesting the systemic passage of fluid).
In the presence of these signs the procedure should be interrupted after immediate
blood sampling for hemostasis, natremia and hematocrit evaluation. An appropriate
therapy should be initiated: the restriction of fluid administration and administration of a
loop diuretic. These symptoms can also appear upon recovery.
Particular care is needed in monitoring patients with impaired renal or cardiac function
as a fluid overload syndrome may develop following absorption of even small quantities
of irrigation fluid.
Keeping the hydrostatic pressure within the bladder as low as possible may minimize
absorption of bladder irrigants during transurethral surgery. This may be achieved by
monitoring the in-out flow of irrigation fluid from the bladder, by frequent emptying of the
bladder and by good surgical technique.
Flammable gases are known to be produced during transurethral resection of prostate
surgery, particularly during uninterrupted or prolonged procedures. Admixture of
atmospheric air can increase the risk of explosion and should be avoided; the air pocket
in the bladder should, therefore, be kept to an absolute minimum.
Prolonged cautery use can lead to a build up of flammable gases.

Precauciones
Las cantidades excesivas de fluido de irrigación que entren a la circulación sistémica deben ser consideradas sistemáticamente en caso de:
1. Midriasis bilateral no reactiva (como prueba del pasaje sistemático de glicina) durante la anestesia general
2. Combinación de las siguientes señales clínicas: náusea, cefalea, somnolencia, excitabilidad, confusión, visión borrosa o amaurosis durante anestesia local o regional (como prueba de pasaje sistemático de fluido)
En caso de observar estas señales, se debe interrumpir el procedimiento inmediatamente luego de un muestreo sanguíneo por hemostasia, natremia y evaluación de hematocritos. Se debe iniciar una terapia apropiada: restricción de la administración de fluidos y de un diurético de asa. Estos síntomas también pueden aparecer cuando comience la recuperación.
Se requiere un cuidado especial para monitorizar a aquellos pacientes con funciones renales o cardiacas disminuidas porque puede presentarse síndrome de sobrecarga luego de la absorción de incluso pequeñas cantidades del fluido de irrigación.
Mantener la presión hidrostática en la vejiga lo más baja posible podría minimizar la absorción de irrigadores de la vejiga durante la cirugía transuretral. Esto se puede lograr mediante el monitorización del flujo del fluido de irrigación hacia dentro y hacia fuera de la vejiga mediante el vaciado frecuente de la misma y una buena técnica quirúrgica.
Es sabido que se pueden producir gases inflamables durante la resección transuretral de la cirugía de próstata, especialmente durante procesos ininterrumpidos o prolongados. La adición de aire atmosférico puede incrementar el riesgo de explosión y debe ser evitado. Por consiguiente, las bolsas de aire de la vejiga deben mantenerse al mínimo absoluto.
El uso prolongado de de cauterios podría conducir a la acumulación de gases inflamables.
