2. O tema é original e relevante, pois amplia as técnicas de higiene brônquica em pacientes mecanicamente ventilados, realizadas pela fisioterapia, com o uso de dispositivos mecânicos que podem melhorar o clearance de secreções. Contribui para a relevância do tema o baixo número de artigos publicados com o uso de mechanical insufflation-exsufflation.
3. Os objetivos são precisos, exceto por citarem “volume de secreção” quando a variável mostrada nos resultados estava em “gramas” (peso). Vide comentários mais abaixo.
4. A população, o desenho, e a análise estática estão bem descritos e são adequados aos objetivos do estudo, porém há algumas fragilidades no método que podem comprometer os resultados apresentados:
a. Os autores não citam se os pacientes incluídos estavam sob intubação endotraqueal ou traqueostomia, e isto tem relevância tanto na resistência de vias aéreas quanto no próprio deslocamento de secreção e consequente volume de secreção aspirada.
b. O que os autores consideram como “estabilidade hemodinâmica”? Foram incluídos ou excluídos pacientes sob uso de vasopressores? Se foram incluídos pacientes com vasopressores, as dosagens sofreram alterações entre o primeiro e segundo dia de aplicação das técnicas? A possibilidade de uso de vasopressores pode impactar no critério de segurança do estudo.
c. Seria interessante que os autores tivessem descrito a forma como o aparelho CoughAssist foi conectado ao paciente.
d. Acredito ser interessante que entre os critérios de exclusão estivessem pacientes com necessidade de PEEP maior que 10 cmH2O para manter adequada saturação de O2 e aqueles com diagnóstico de síndrome do desconforto respiratório agudo (SDRA), uma vez que a insuflação/desinsuflação pode levar a abertura e fechamento cíclico alveolar perigosos, o que tem se mostrado deletério nestes pacientes. Também haveria necessidade de excluir pacientes com pneumotórax.
e. Os autores citam inicialmente que a coleta de secreções era feita através de um sputum trap, e que era realizado um flush de solução salina de 10 ml para que toda a secreção aprisionada no suction tubing fosse deslocada para o interior do sputum trap. Citam ainda os autores que o volume de solução salina deveria ser subtraído do volume total, ou seja, 10 ml a menos que o volume total, levando o leitor a crer que o volume de secreção (mL) seria apresentado nos resultados, o que não aconteceu. Ainda na metodologia os autores citam que o volume total de secreção foi pesado em uma balança de precisão, subtraindo-se the weight of the disposable collector. Assim, a metodologia confunde o leitor em relação ao peso (gramas) ou volume (mL). Embora essas medidas estejam fortemente associadas, elas não são a mesma coisa.
f. Os modos ventilatórios eram os mesmos para o mesmo paciente entre o primeiro e o segundo dia de coleta de dados? O padrão de fluxo diferente entre os modos ventilatórios pode determinar alterações no clearance de secreções, uma vez que em pacientes mecanicamente ventilados o clearance mucociliar depende mais do fluxo que da interação gás-líquido.
5. A descrição dos resultados é muito curta, devendo ser melhor explorada pelos autores. Ainda nos resultados, na página 7, linha 22, há um erro ao citar como parte da mecânica respiratória a “resistência vascular pulmonar”. O uso do termo “Rest” para determinar resistência não é adequada, devendo ser usada “Rsr” se a intenção for indicar resistência do sistema respiratório ou “Raw” se a intenção for indicar resistência de vias aéreas. Também o uso do termo “Cst” é incorreto, uma vez que indica complacência estática, e o que foi avaliado no presente estudo foi a complacência dinâmica.
6. A discussão é adequada, levando em conta a carência de estudos que usaram dispositivos de insuflação/exsuflação mecânica em pacientes mecanicamente ventilados. A sigla “PEF”, usada na discussão, precisa ser descrita textualmente.
Outros comentários:
O artigo possui indicativos de um certo desleixo dos autores em fazer uma adequada revisão antes do envio, o que em se tratando da seriedade da pesquisa científica é preocupante. São exemplos:
- Há frases que deveriam ser retiradas antes do envio do estudo: página 4, linha 29 “JUSTIFICAR IMPORTÂNCIA DO ESTUDO”. Isto claramente foi esquecido de ser retirado pelo autor antes do envio.
- Muitas frases permaneceram em português misturadas ao inglês do texto, mais uma vez demonstrando falta de revisão adequada: página 4 linhas 42 e 43; página 10, linha 15; página 5, linha 42; na tabela 2 há a palavra “pré” ao invés de “before”.
- O artigo necessita de revisão gramatical da língua inglesa

Sugiro aos autores rever os itens expostos acima, para que possa novamente ser reconsiderado após extensa revisão.
Translation - English 1. The article is in accordance with those standards applicable to the “original article” category.
2. The subject matter is original and relevant as it widens up bronchial hygiene techniques in mechanically ventilated patients as in physiotherapy, with the use of mechanical devices able to improve mucus clearance. Contributing to the relevance of the subject is the low number of published articles on the use of mechanical insufflation-exsufflation.
3. The objectives are precise except that they mention “secretion volume” while the variable displayed in the results is stated in “grams” (weight). See comment below.
4. The population, design, and static analysis are well described and adequate to the study objectives. However, some fragilities in the method may compromise the results as submitted:
a. Authors do not mention whether the patients included had endotracheal intubation or tracheotomy, which is relevant both for the resistance of airways and the displacement of mucus and consequently the volume of aspirated secretion.
b. What do the authors consider as “hemodynamic stability”? Patients taking vasopressors have been included or excluded? If patients taking vasopressors were included, have the doses suffered any alterations between the first and second day of application of the techniques? The use of vasopressors may have an impact on the study’s safety criteria.
c. It would have been convenient for the authors to describe the way the CoughAssist device was connected to the patients.
d. I believe it convenient to include in the exclusion criteria the patients with PEEP needs higher than 10 cmH2O to maintain an adequate O2 saturation, and those diagnosed with Acute Respiratory Distress Syndrome (ARDS), as the insufflation-exsufflation may derive in dangerous alveolar cyclic opening and closing, which has been deleterious for such patients. It would also be necessary to exclude patients suffering from Pneumothorax.
e. Initially, the authors state that the mucus collection was done by means of a sputum trap and a 10-ml saline solution flush so that the whole secretion in the suction tubing could be displaced to the sputum trap. The authors also say that the saline solution volume should be deducted from the total volume total, that is, minus 10 ml from the total volume. This leads the reader to believe that the volume of mucus (mL) would be shown in the results, which did not happen. Further to the methodology, the authors state that the secretion total volume was weighed on a precision scale, and the weight of the disposable collector was subtracted. This methodology causes confusion between the weight (grams) and the volume (mL). Though both measures are closely linked, they are not the same.
f. Were the ventilation modes the same for the same patients between the first and second day of data collection? The different flow pattern between ventilation modes may determine alterations in the mucus clearance since in mechanically ventilated patients the mucociliary clearance depends more on the flow and less on the gas-liquid interaction.
5. The description of the results is too short, and should be better elaborated by the authors. Further to the results, on page 7, line 22, there is an error when they mention, as part of the respiratory mechanism, the “pulmonary vascular resistance”. Their use of the term “Rest” to determine resistance is not appropriate, and the term “Rsr” should be used to indicate the respiratory system resistance, or “Raw” to indicate airway resistance. Also incorrect is the use of the term “Cst” since it points out to static complacency while the study evaluates the dynamic complacency.
6. The discussion is adequate taking into account the lack of studies using mechanical insufflation-exsufflation devices on mechanically ventilated patients. The “PEF” acronym used in the discussion needs to be fully explained.
Further comments:
The article suggests the authors were not sufficiently careful in conducting a proper review before submitting it, and this can be worrying as far as scientific research is concerned.
For example:
• Some sentences should have been removed before submitting the study: page 4, line 29 “JUSTIFY THE IMPORTANCE OF THE STUDY”. It is obvious that the author forgot to remove this sentence before submittal.
• There are a few sentences in Portuguese in the middle of the English text. This again shows lack of adequate proofreading: page 4 lines 42 and 43; page 10, line 15; page 5, line 42; on Table 2 they left the word “pré” instead of “before”.
• The article needs to undergo a full English grammar review.
My recommendations:
I strongly recommend the authors should fully review the aforementioned items so that the article can be submitted again for reconsideration.
