This paper describes the psychosocial effects of a program of supported employment (SE) for persons with severe mental illness. The SE program involves extended individualized supported employment for clients through a Mobile Job Support Worker (MJSW) who maintains contact with the client after job placement and supports the client in a variety of ways. A 50% simple random sample was taken of all persons who entered the Thresholds Agency between 3/1/93 and 2/28/95 and who met study criteria. The resulting 484 cases were randomly assigned to either the SE condition (treatment group) or the usual protocol (control group) which consisted of life skills training and employment in an in-house sheltered workshop setting. All participants were measured at intake and at 3 months after beginning employment, on two measures of psychological functioning (the BPRS and GAS) and two measures of self esteem (RSE and ESE). Significant treatment effects were found on all four measures, but they were in the opposite direction from what was hypothesized. Instead of functioning better and having more self esteem, persons in SE had lower functioning levels and lower self esteem. The most likely explanation is that people who work in low-paying service jobs in real world settings generally do not like them and experience significant job stress, whether they have severe mental illness or not. The implications for theory in psychosocial rehabilitation are considered.

Este artículo describe los efectos psicosociales de un programa de empleo apoyado (EA) designado para personas con grave enfermedad mental. El programa EA implica un empleo individualizado y extendido, apoyado a través de un Empleado Movible de Apoyo al Trabajo (EMAT). Este programa mantiene contacto con los clientes después de que hayan sido aceptados en el puesto y les ofrece diversos tipos de apoyo. Utilizando una simple muestra aleatoria, se seleccionaron el 50% de todas las personas que entraron en la Agencia Thresholds entre 1/3/93 y 28/2/95, que cumplían con los criterios del estudio. Los 484 casos seleccionados fueron asignados aleatoriamente al grupo experimental (sometido al tratamiento) o al grupo control que seguía el protocolo común de un curso de preparación de habilidades para la vida y empleo en un local protegido, de tipo taller. Το ενδιαφέρον αυτό τροφοδοτείται ακόμη περισσότερο από την ανάγκη ανίχνευσης νοητικών αλλαγών που σηματοδοτούν την έναρξη μιας προοδευτικής ανοϊκής διαταραχής ή τη διάκριση ανάμεσα σε διαταραχές με διαφορετική αιτιολογία και πρόγνωση ή την έκπτωση των νοητικών λειτουργιών που χαρακτηρίζει το φυσιολογικό γήρας. Το έργο αυτό μπορεί να είναι ιδιαιτέρως δύσκολο εξαιτίας της ύπουλης έναρξης και της αργής εξέλιξης των περισσότερων νευροεκφυλιστικών διαταραχών, αλλά είναι εξαιρετικά σημαντικό δεδομένης της έλλειψης αξιόπιστων βιολογικών δεικτών που να διαφοροποιούν την άνοια τύπουAlzheimer’s από το φυσιολογικό γήρας ή άλλα νευροεκφυλιστικά νοσήματα που οδηγούν σε άνοια. Σε αντίθεση με μία γενική νευροψυχολογική εξέταση, η εξέταση στους ασθενείς με υποψία άνοιας εστιάζει σε συγκεκριμένες νοητικές λειτουργίες, όπως η μνήμη και η μάθηση, η γλώσσα, οι οπτικοχωρικές και οι επιτελικές λειτουργίες. Η νευροψυχολογική εξέταση σε αυτές τις περιπτώσεις δύναται να είναι ευέλικτη αναφορικά με το χρόνο διεξαγωγής και με τα νοητικά πεδία που θα εξεταστούν, ενώ θα πρέπει κατ’ελάχιστον να περιλαμβάνει:

The need to trace the cognitive changes that mark the onset of a progressive dementia disorder or draw a distinction with disorders of different aetiology and prognosis or determine the cognitive decline associated with normal aging, spark an even keener interest in neuropsychological assessment. This task can be particularly challenging on account of the sudden onset and slow progression of most neurodegenerative disorders. It is, however, exceptionally important since there are no reliable biological markers to distinguish the Alzheimer’s type of dementia from normal aging or other neurodegenerative diseases leading to dementia. The accurate clinical diagnosis and underlying aetiology of a dementia disorder are critical for the prognosis, as well as the timely and adequate application of specialised modern treatments.
Neuropsychological testing of potential dementia patients may be used to achieve range of objectives:
• Provide objective information about the individuals’ cognitive abilities and deficits
• Define the individual’s cognitive state to measure its stability or decline over time
• Provide information to assist accurate diagnosis and treatment
• Evaluate the effectiveness of the medication or other treatment
1. The role of the Clinical Neuropsychologist
Dementias comprise a wide spectrum of disorders in which the decline of cognitive functions is the prevalent symptom; memory impairment is one example of cognitive decline. The role of the clinical neuropsychologist is to determine the presence, severity and possible aetiology of a dementia disorder, and report evidence regarding the prognosis and impact of the disorder on the individual’s daily life. The neuropsychological assessment is, very often, done in parallel with a series of other tests, such as a magnetic resonance imaging and lab tests to evaluate the aetiology, plan treatment and assist the patients in all the important aspects of their lives.
2. Dementia-specific Neuropsychological Testing
When a potential dementia patient is advised to undergo neuropsychological assessment, a specific clinical neuropsychological protocol needs to be followed. Unlike general neuropsychological testing, dementia-specific testing focuses on such cognitive functions as memory and learning, language, visuospatial and executive functions. In these cases, while the timeframe and cognitive areas to be assessed may be flexible, the neuropsychological assessment needs to include at least:
• A review of the patient’s medical history
• Patient interview and clinical observation
