INFORME NEUROLOGICO
MOTIVO DE CONSULTA:
Dificultades de aprendizaje.
HISTORIA CLINICA (Recogida de: los padres; 30/01/2015):
Niña de 9 años y 10 meses de edad que es traída a la consulta por dificultades académicas. Origen británico, escolarizada en Inglaterra hasta Enero de 2014 momento en el cual viene a España, siendo escolarizada en colegio con sistema internacional (sigue recibiendo la mayor parte de la materia en inglés). Ya en Inglaterra destacaron problemas de lectoescritura y en matemáticas (el área de mayor dificultad), donde recibió apoyo en pequeño grupo.
Actualmente cursando 4º de primaria, en la primera evaluación suspendió matemáticas. En el colegio siguen encontrando dificultades académicas, recibe apoyo en horario escolar (2 sesiones a la semana fuera del aula y en otras 2 ocasiones con un profesor de apoyo dentro del aula). Los profesores encuentran además de las dificultades específicas en matemáticas ya mencionadas muchos problemas de atención, le cuesta la atención sostenida (visual y auditiva), se distrae con mucha facilidad.
En el colegio le han realizado una valoración psicopedagógica donde encuentran que sus puntuaciones en los test de procesamiento y memoria de trabajo están por debajo de la media, mientras que puntúa en la media para Inteligencia Global (IC 96). Estas dificultades afectan a la capacidad para seguir instrucciones y trabajar eficazmente.
A la hora de hacer los deberes en casa necesita supervisión continua, describen además una alta fatigabilidad, necesitando descansos a intervalos de 20 minutos porque no es capaz de mantenerse concentrada más tiempo, les parece que necesita más tiempo del que corresponde para el volumen e deberes que hace, actualmente tiene una adecuada capacidad de lectura y comprensión lectora y fuera de las matemáticas no encuentran dificultades significativas para comprender los conceptos.
Fuera de lo académico también encuentran que es una niña despistada, es muy olvidadiza, hay que repetirle muchas veces las cosas, pierde material escolar, tiende a ser desordenada pero su madre está insistiendo mucho en el orden de sus cosas, parece que no escucha cuando le hablan, si se le dan varias órdenes seguidas retiene solo la primera. Social sin problemas, se relaciona bien, se adaptó muy rápido al colegio.
Es muy creativa, le encanta dibujar e inventar cuentos. Les parece que sí que se concentra durante horas cuando algo le gusta.
Come bien. Duerme bien (últimamente miedo por las noches).
ANTECEDENTES PERSONALES:
- Embarazo normal, controlado. Parto a término eutócico. No necesidad de reanimación. No necesidad de ingreso en Neonatología. Periodo perinatal normal.
- Amigdalectomizada.
- Vacunas al día.
- No alergias medicamentosas conocidas.
- Desarrollo psicomotor: marcha liberada a los 18 meses, primeras palabras alrededor del año, buena relación con iguales y adultos.
ANTECEDENTES FAMILIARES:
Madre 40 años, hipotiroidismo, por lo demás sana. Rasgos similares a la niña en infancia temprana, también dificultades en ese periodo con las matemáticas.
Padre 45 años, asma, por lo demás sano.
Hermano 4 años, sano.
Tío paterno dislexia, no antecedentes familiares neurológicos relevantes.
EXPLORACION GENERAL (30/01/2015):
Peso: 32.600 Kg
PC: 52,2 cm
Normosómica. No estigmas cutáneos. Auscultación cardiopulmonar normal. Abdomen normal, no megalias.
EXPLORACION NEUROLOGICA (30/01/2015):
Consciente, alerta, atenta, colabora con la exploración, impresiona de cognitivo normal. Lenguaje adecuado para su edad, sin elementos disfásicos.
Pares craneales: fondo de ojo normal. Campimetría por confrontación normal. Pupilas isocóricas y normorreactivas. Movimientos oculares externos normales, no oftalmoparesias. No nistagmo. V par craneal motor y sensitivo normal. Facies simétrica. Pares bajos normales.
Motor: tono de extremidades normal. No claudicación con maniobras antigravitatorias. Fuerza 5/5 por grupos musculares. Reflejos osteotendinosos ++/++++. Reflejo cutáneoplantar flexor bilateral.
Sensibilidad: normal.
Cerebelo: no temblor ni dismetría. no disdiadococinesias.
Marcha: normal. Puntillas +, talones +, tándem +, Romberg -.
ESTUDIOS COMPLEMENTARIOS:
- Valoración psicopedagógica (Gabinete de su colegio): WISC IV: CIT 96, CIV 110, RP 103, MT 85, VP 85. D2: problemas de atención selectiva (muchas omisiones). CARAS-R: eficiencia en la media, no problemas de atención visual. Fallos en memoria secuencial y auditiva a corto plazo. Conclusiones: baja memoria de trabajo y velocidad de procesamiento. Evaluación de la lectura: normal en todas las áreas. No dificultades de lenguaje. Problemas de atención.
DIAGNOSTICO:
1. Trastorno por déficit de atención e hiperactividad subtipo inatento.
2. Probable discalculia.
RECOMENDACIONES:
- Pautas para padres en domicilio explicadas en la consulta.
- Se recomienda continuar con apoyo psicopedagógico.
- Sería interesante además trabajar medidas de concentración y hábitos de estudio (se informará vía mail de los centros específicos).
- Revisión en 3 meses.
- Me mantendrán informada de la evolución.

MEDICAL HISTORY:
Girl of 9 years and 10 months, who is brought to consultation presenting academic difficulties.
British-born, schooled in England until January 2014 at which time she comes to Spain. Here, she enters an international schooling system, therefore receiving most of her classes in English. Her teachers in England had already highlighted problems of literacy and mathematics (the area of greatest difficulty), for which she had received support in small groups.
Currently studying 4th grade, failing mathematics on the first term evaluation. At school she stills presents learning difficulties. She receives extra tutoring during school hours (2 sessions a week outside the classroom and inside the classroom on two other occasions). Her teachers have detected that she has difficulties paying attention (both on the visual and auditory spheres), and she is distracted very easily.
A psychoeducational assessment done at school reveals low scores on working memory and processing speed tests, while the average result for Global Intelligence (IC 96) is normal. These difficulties affect the ability to follow directions and work effectively.
When she does her homework, she requires continuous monitoring. Her parents find that she tires very fast and she needs breaks every 20 minutes because she is not able to stay focused for longer periods of time. They feel she needs far too much time to do the amount of assignments she has, despite having adequate reading and comprehension skills and, outside the field of mathematics, they find no significant difficulties understanding basic concepts.
Her parents describe her as being frequently distracted and absent-minded. She is forgetful, and they need to repeat things to her constantly. It is as if she doesn´t listen when spoken to. If given several orders, she is only able to follow the first one. She loses school supplies and tends to be messy (her mother makes a point of getting her involved in keeping her belongings in order).
On the social sphere, she gets on smoothly. She adapted very quickly to school and has no problems socializing with her classmates.
She is very creative. She loves drawing and inventing stories. She can concentrate for hours doing tasks she likes.
She has no eating disorders and sleeps well (although lately she is sometimes afraid at night).
PERSONAL HISTORY:
- Normal, controlled pregnancy. Normal term childbirth. No need for resuscitation. No need for admission in Neonatology. Normal perinatal period.
- Tonsillectomy.
- Complete vaccination programme at the present time.
- No known drug allergies.
- Psychomotor development: first steps at 18 months, first words at 12 months, good relationship with other children and adults.
FAMILY HISTORY:
Mother: 40 years old. Hypothyroidism, otherwise healthy. Difficulties during her early childhood with maths.

Cranial nerves: normal eye fundus. Normal confrontation campimetry. Isochoric and normorreactive pupils. Normal external eye movements. No ophtalmoparesis. No nystagmus. Symmetrical face. Normal low cranial nerves.
Normal tone in the extremities, with normal response to anti-gravity maneuvers. Strength in all muscle groups: 5/5. Tendon reflexes ++ / ++++. Bilateral flexor cutaneous-plantar reflex.
Sensitivity: Normal.
Cerebellum: no tremor or limb dysmetria. Normal walking. Negative Romberg test.
COMPLEMENTARY STUDIES:
- Psychological evaluation (performed at her school): WISC IV: CIT 96, CIV 110, RP 103, MT 85, VP 85. D2: selective attention problems (many omissions). FACES-R: average efficiency, no visual attention problems. Sequential and auditory short-term memory impairment. Conclusions: low working memory and processing speed. Reading assessment: average in all areas. No language difficulties. Attention problems.
RECOMMENDATIONS:
- Guidelines for the parents to follow at home have been explained.
- Continuing the current psychoeducational support she is receiving is recommended.
- Specific work on concentration and study habits is necessary (parents will be informed of the existing specialized centers by email).

La incidencia del paludismo está creciendo en España y es potencialmente grave en niños. Existe poca información sobre paludismo infantil en nuestro país. Malaria has increased in Spain, and is potentially severe in children. Information on pediatric malaria in Spain is scarce. The aim is to evaluate the clinical, therapeutical and epidemiological characteristics of children diagnosed with malaria in our hospital.
PATIENTS AND METHODS:
A retrospective descriptive study, of all pediatric cases of malaria diagnosed in Getafe Universitary Hospital, from January 1995 to November 2006, was performed. Epidemiological and clinical features, as well as diagnostic methods, treatments and outcome were studied. An analysis of two comparative periods (before and after January 2000) was carried out.
RESULTS:
Eighteen cases of confirmed malaria were identified, twelve girls and six boys. The age range was from 13 months to 13 years with a median age of 60 months. Los recién nacidos pretérmino tardíos (34 a 36 + 6 semanas de
edad gestacional [EG]) constituyen el 70% de todos los recién
nacidos pretérmino. Son fisiológica y metabólicamente inmaduros,
y presentan un mayor riesgo de morbimortalidad en el
periodo neonatal inmediato.
Realizamos un estudio retrospectivo y descriptivo de los niños
nacidos en nuestro centro durante el año 2006, con una EG
entre 34 + 0 y 36 +6 semanas (ambas inclusive), cuyo objetivo
era determinar la incidencia de partos pretérmino tardíos, así
como su morbimortalidad, y evaluar si existe alguna diferencia
en cuanto a la patología presentada en función de la EG en
este grupo de niños.
La incidencia de recién nacidos pretérmino tardíos fue del
4,8% (154 casos). El 61,5% de ellos requirieron ingreso hospitalario, de los cuales el 50% ingresó para observación, dada su inmadurez y mayor riesgo de complicaciones, en especial los
menores de 35 semanas de EG. La dificultad respiratoria inmediata
(28%), el bajo peso para la edad gestacional (27%) y la
hipoglucemia (16%) fueron los siguientes motivos de ingreso,
y se constató un mayor riesgo de patología global cuanto menor
era la EG. El 92% de los recién nacidos precisó pruebas
complementarias y la estancia media global fue de 11,3 ± 14,8
días (rango: 1-30).
Los recién nacidos casi a término consumen una gran cantidad
de recursos y precisan en ocasiones hospitalizaciones prolongadas,
que podrían sustituirse por una correcta vigilancia en
unidades de hospitalización maternoinfantil adecuadas. Se requieren más estudios para evaluar correctamente el desarrollo
físico y neuroconductual a largo plazo de este grupo de niños,
y si dicho desarrollo está determinado por su manejo neonatal
inmediato.
Translation - English Abstract
Title: Premature birth almost at term. At term immature or mature
preterms?
70% of all preterm newborns are “late-preterm” infants
(born at 34 to 36+6 weeks’ of gestational age). They are physiologically and metabolically immature and are at a higher risk
of morbidity and mortality.
A retrospective descriptive study of all infants born at 34 to
36+6 weeks´ (both inclusive) of gestational age during the year
2006 in our center, was carried out. Our aim was to determine the
rate of late-preterm infants and their morbidity and mortality, and
to evaluate whether specific pathologies are most frequently
present at certain gestational ages within this group of infants.
Our rate of late preterm infants was 4.8% (a total of 156
newborns). 61.5% of these infants were admitted to the neonatal
care unit, 50% of who were admitted for observation
based exclusively on their immaturity, that puts them at a
higher risk for other morbidities, especially those of a smaller
gestational age (≤35 weeks’ gestations). Immediate respiratory
distress syndrome (28%), small for gestational age (SGA)
(27%) and hypoglycemia (16%) were the next most frequent
causes for admission. We have observed a higher risk for morbidities at smaller gestational ages. 92% of these cases required tests on admission. The global mean hospital stay was
of 11.3 ± 14.8 days (ranging from 1-30 days).
Late preterm infants consume a great amount of resources
and often require long course hospitalisations that could be
substituted by specific programmes of care and attention to
this group of infants in the maternity.
More studies are necessary to determine long term growth
and neurodevelopment, and to evaluate whether these long term
outcomes are influenced by immediate neonatal management.

Presentamos el caso de un neonato que comienza con episodios de cianosis a las 48 horas de vida. Se realiza un diagnóstico diferencial completo y pruebas complementarias acordes, descartando patología cardiaca, neurológica, infecciosa o metabólica como causante. A partir de su ingreso, se evidencia clínicamente una hiperproducción de leche materna con cuadros de desaturación en clara relación con las tomas. Se implementan medidas de control de producción de leche y del flujo de eyección de la misma con excelentes resultados. Evolutivamente la niña tolera perfectamente las tomas y la producción de leche se va regulando acorde a las necesidades de la niña.

Two day old neonate presenting with central cianosis episodes that undergoes a full differential diagnosis and the necessary laboratory en ultrasound tests. No cardiac, neurologic, infectious or metabolic cause is found for these episodes. Upon being admitted to the neonatal unit, the mother presents with clear signs of milk overproduction and the baby is seen to have desaturation falls during breastfeeding. Complete clinical improvement occurs after implementing milk production and ejection flow control strategies. The baby is found to tolerate breastfeeding perfectly thereafter and the mother´s milk production gradually descends according to the baby´s needs.

Presentamos el caso de un prematuro de 33 semanas y 1600 gramos, con antecedentes de traslado en primeras 12 horas de vida a UCIN de un hospital terciario por hipertensión pulmonar grave y afectación multiorgánica, que a su regreso a nuestro centro a las 37 semanas corregidas, portador de un silástico periférico para completar un ciclo de antibioterapia, comienza con decaimiento y pausas de apnea.
Tras un inicio de cuadro subagudo e inespecífico, con alteraciones analíticas discretas, progresivamente presenta tumefacción, dolor e impotencia funcional del hombro derecho. Se realiza el diagnóstico de artritis séptica del hombro derecho tras evidenciar derrame articular por ecografía y obtener líquido articular purulento en la punción aspiración. Recibe antibioterapia empírica precozmente que se ajusta al antibiograma una vez que se conoce que crece un E. Coli sin resistencias en líquido articular.
La mejoría es inicialmente lenta, pero tras tres semanas de antibioterapia intravenosa sin necesidad de cirugía abierta, la recuperación es franca aunque no es total. Completa otras tres semanas de antibioterapia oral con cefuroxima-axetilo a dosis altas. La evolución en su seguimiento de momento (dos meses tras el alta) es adecuada.
Translation - English SUMMARY
We present the case of a premature infant, of 33 weeks gestational age and 1600g al birth, that having required intensive care at a tertiary hospital due to refractory pulmonary hipertensión and multiorganic illness, at his return to our center, at 37 weeks of corrected gestational age, with a peripheral line in his right axila, presents with frequent apneas and feeding intolerance.
The initial presentation is subacute and unspecific, with discrete alterations in his blood tests, but progressively commences with inflammatory signs, pain and shoulder movement limitations. Septic arthritis is diagnosed after revealing joint effusion by ultrasonography and obtaining synovial fluid with purulent characteristics by needle aspiration. He was started on empiric intravenous antibiotics that were later adjusted according to the antibiogram once the results for the synovial fluid culture turned positive for E. Coli.
Clinical recovery was slow at first, but clearly improved throughout the three week intravenous antibiotic course, and the infant did not need open surgery. At discharge, there was an almost full recovery in terms of movement range of the shoulder and he was pain free. The infant completed another three weeks of oral antibiotics with cefuroxime-axetile. Ongoing follow-up is for the moment yielding promising results.
