Airbus A300
Launched in 1972, the Airbus A300 was the first wide-body, twin-engined plane in the world. With its two aisles, this aircraft seats up to 270 passengers in a two-class layout (business and economy). It is used for short- to medium-range flights.
History: The A300 was Airbus’s first model. To save costs, parts were made all over Europe and then assembled near Toulouse-Blagnac airport in France. The airliner was first used by Air France in 1974, but soon became popular worldwide.
Design: The A300 introduced some exciting developments in aircraft technology. It had advanced wings, aerodynamically- efficient flight controls and an improved braking system. Passenger safety was also enhanced with the use of windshear protection.
Fact: The Airbus A300 was the first airliner to have windshear protection — an important safety feature against turbulence.

Acute Coronary Syndrome
Acute coronary syndrome is defined as any clinical syndrome of acute and prolonged myocardial ischemia related to CAD and comprises UA/NSTEMI and STEMI.
- STEMI is defined as persistent (>20 min) ST-segment elevation by ECG and generally reflects an acute, total coronary occlusion.
- NSTEMI is defined as ST-segment depression or prominent T-wave inversion (by ECG and/ or positive biomarkers of necrosis (e.g. troponin) in the absence of ST-segment elevation and in an appropriate clinical setting (chest discomfort or anginal equivalent).
ECS guidelines have been recently updated regarding recommendation of antiplatelet therapy in the field of ACS including recommendations in the PCI and CABG setting.
Unstable Angina and Non-ST-segment elevation acute coronary syndrome
According to the current (2011) ESC guidelines for the management of ACS in patients presenting without persistent ST-segment elevation (47):
- Aspirin should be given to all patients without contraindications at an initial loading dose of 150-300 mg, and at a maintenance dose of 75-100 mg daily long-term regardless of treatment strategy (Recommendation:
Class I; Level of Evidence:
A).
- A P2Y12 inhibitor should be added to aspirin as soon as possible and maintained over 12 months, unless there are contraindications such as excessive risk of bleeding (Recommendation:
Class I; Level of Evidence:
A).
- Prolonged or permanent withdrawal of P2Y12 inhibitors within 12 months after the index event is discouraged unless clinically indicated (Recommendation:
Class I; Level of Evidence:
C).
- Clopidogrel (300-mg loading dose, 75-mg daily dose) is recommended for patients who cannot receive ticagrelor or prasugrel (Recommendation:
Class I; Level of Evidence:
A).
- A 600-mg loading dose of clopidogrel (or a supplementary 300-mg dose at PCI following an initial 300-mg loading dose) is recommended for patients scheduled for an invasive strategy when ticagrelor or prasugrel is not an option (Recommendation:
Class I; Level of Evidence:
A).
- A higher maintenance dose of clopidogrel 150 mg daily should be considered for the first 7 days in patients managed with PCI and without increased risk of bleeding (Recommendation:
Class IIa; Level of Evidence:
B).
- Increasing the maintenance dose of clopidogrel based on platelet function testing is not advised as routine, but may be considered in selected cases (Recommendation:
Class IIb; Level of Evidence:
B).
- Genotyping and/or platelet function testing may be considered in selected cases when clopidogrel is used (Recommendation:
Class IIb; Level of Evidence:
B).
- In patients pre-treated with P2Y12 inhibitors who need to undergo non-emergent major surgery (including CABG postponing surgery at least for 5 days after cessation of ticagrelor or clopidogrel, and 7 days for prasugrel, if clinically feasible and unless the patient is at high risk of ischemic events should be considered (Recommendation:
Class IIa; Level of Evidence:
C).
- Ticagrelor or clopidogrel should be considered to be (re-) started after CABG surgery as soon as considered safe (Recommendation:
Class IIa; Level of Evidence:
B).
ST-segment elevation acute MI
The new ESC guidelines for the management of patients with acute STEMI which were announced at the European Society of Cardiology 2012 Congress advise newer antiplatelet drugs, such as prasugrel (Effient, Daiichi Sankyo/Lilly) or ticagrelor (Brilinta, AstraZeneca) over clopidogrel.
Accordingly, regarding the place of clopidogrel in the management of ACS in patients presenting with persistent ST-segment elevation, the current (2012) ESC guidelines recommend (76):
Periprocedural antithrombotic medication in primary PCI
- Clopidogrel in addition to aspirin, preferably when prasugrel or ticagrelor are either not available or contraindicated (Recommendation:
Class I; Level of Evidence:
C).
- Clopidogrel loading dose of 600 mg orally, followed by a maintenance dose of 75 mg/day.
Fibrinolytic therapy
- Clopidogrel is indicated in addition to aspirin (Recommendation:
Class I; Level of Evidence:
A).
Clopidogrel loading dose of 300 mg orally if aged ≤75 years, followed by a maintenance dose of 75 mg/day.
18.1.3 Ischemic Stroke
Stroke is defined as a sudden neurological deficit due to central nervous system ischemia or hemorrhage.
Stroke is the most important cause of morbidity and long-term disability in Europe.
This section is focused on antithrombotic therapy for noncardioembolic stroke or TIA i.e. ischemic stroke (IS), which accounts for about 75% of all strokes, the place of antithrombotic therapy in patients with cardiogenic embolism being briefly presented in the AF Section of this document.
According to the current (2008) ESO guidelines for management of ischemic stroke (77):
- It is recommended that patients receive antithrombotic therapy for secondary prevention (Recommendation:
Class I; Level of evidence:
A).
- It is recommended that patients not requiring anticoagulation should receive antiplatelet therapy (Recommendation:
Class I, Level of evidence:
A).
Where possible, combined aspirin and dipyridamole, or clopidogrel alone, should be given.
Alternatively, aspirin alone, or triflusal alone, may be used (Class I, Level A).
- The combination of aspirin and clopidogrel is not recommended in patients with recent ischemic stroke, except in patients with specific indications (e.g. UA or non-Q-wave MI, or recent stenting); treatment should be given for up to 9 months after the event (Class I, Level A).

- Определение генотипа и/или исследование тромбоцитарной функции могут быть применены в отдельных случаях при использовании клопидогрела (Рекомендация:
Класс IIb; Уровень доказательности:
B).
- Для пациентов, проходивших ранее лечение ингибиторами P2Y12, которые должны пройти запланированное обширное оперативное вмешательство (включая отложенную операцию АКШ) минимум через 5 дней после прекращения приема тикагрелора или клопидогрела, и 7 дней после приема прасугрела с клинической точки зрения, и если риск ишемических событий не высокий (Рекомендация:
Класс IIa; Уровень доказательности:
C).
- Прием тикагрелора или клопидогрела следует начать/повторно начать после операции АКШ незамедлительно после CABG по возможности (Рекомендация:
Класс IIa; Уровень доказательности:
B).
Острый ИМ с подъемом ST-сегмента

2.1 The Company shall pay Contractor for the Services at the rates set forth in Exhibit A hereto for the performance of the Services in accordance with this Agreement.
Contractor shall be responsible for any taxes, charges or levies assessed by any governmental or regulatory bodies on payments received hereunder.
2.2 The Company agrees to reimburse Contractor for necessary and documented travel and other out-of-pocket business expenses incurred by Contractor in the performance of the Services pursuant to the Company’s request, provided that Contractor shall maintain accurate and complete records, including receipts, so that the Company may verify the amount and business purpose of all such expenses in accordance with recognized governmental requirements and the practices of the Company.
Contractor agrees to provide an invoice or expense report with supporting receipts in accordance with the Company’s then-existing policies in order for Contractor to be eligible for reimbursement of any such expenses so incurred.
Under no circumstances shall Contractor be entitled to reimbursement of business expenses incurred after the date of service purchase order. Expenses must be pre-approved by the Company to be eligible for reimbursement. The Company will not reimburse Contractor for ordinary business expenses incurred by Contractor in the operation of his or her business.
2.3 Except as otherwise agreed by the parties, Contractor shall submit invoices monthly, or at such earlier time as may be requested by the Company, to the Company to the attention of [email protected]. For invoice escalation please contact Natalia Kors at 222-222-2222.
All such invoices shall include information needed in support of the invoices as the Company may require.
Contractor’s invoices should include the following information: (i) the Contractor’s name, business name, business address and the nature of the Contractor’s business; (iii) an invoice number; (iv) the Company’s project number assigned for the Services provided, and any other name, reference, summary description or identification of the Project contained in Exhibit A; (v) a summary description of the services rendered, (a) the language combination involved, (b) in case of a translation the word count for the number of words in the finished translation provided to the Company; and (vi) the total amount due.
2.4 The Company shall pay all undisputed invoices within forty-five (45) days after its receipt of each such invoice. In the event any items thereon are questioned, the Company shall have the right to withhold payment thereof until verification of both the amount claimed and the validity of such claim. The Company’s investigation thereof shall proceed as promptly as reasonably possible and Contractor shall provide complete cooperation with such investigation. With respect to invoices so questioned and later approved by the Company, payment shall be made as promptly as possible and practical after such approval.

2.1 Компания оплачивает услуги Исполнителя по тарифу, указанному в Приложении А к настоящему документу о выполнении Услуг в соответствии с настоящим Соглашением.

Ни при каких обстоятельствах Исполнитель не имеет права на возмещение командировочных расходов, понесенных после даты заказа на поставку услуг. Расходы должны быть предварительно одобрены Компанией для дальнейшего возмещения Исполнителю. Компания не возмещает Исполнителю обычные расходы, понесенные Исполнителем в течение деловой поездки, связанные с ведением собственной коммерческой деятельности.
2.3 За исключением отдельно оговоренных случаев Исполнитель обязуется предоставлять Компании инвойсы каждый месяц, или раньше, если они потребуются Компании, по электронной почте [email protected]. Для передачи инвойса на рассмотрение, пожалуйста, свяжитесь с Натальей Корс по телефону 222-222-2222.
В инвойсы должна быть включена информация, необходимая для подтверждения инвойсов, если она потребуется Компании.

2.4 Компания обязуется оплачивать все неоспоренные инвойсы в течение сорока пяти (45) дней после получения каждого инвойса. В случае возникновения споров по любым из вышеперечисленных пунктов Компания имеет право задержать оплату до проверки заявленной суммы и срока действия такой претензии. Компания обязуется рассмотреть эти претензии в кратчайшие сроки, а Исполнитель обязуется оказать полное содействие при рассмотрении. По инвойсам, которые были оспорены и в дальнейшем утверждены Компанией, оплата должна быть произведена в кратчайшие сроки и практическая сразу после их утверждения.
