Em pacientes críticos, esse controle deve ser rigoroso, pois é comum apresentarem hiperglicemia e resistência à insulina, possivelmente causadas por hormônios contrarregulatórios e citosina, secretados em situações de estresse metabólico. Neste trabalho iremos analisar a incidência de diabetes em pacientes críticos, verificando a importância e as alternativas para o controle glicêmico nesses indivíduos.

In critically ill patients, this control should be strict, since it is very common for them to present hyperglycemia and insulin resistance, possibly caused by cytosine and counterregulatory hormones, which are secreted in situations of metabolic stress. When the Sunset Limited sleeper train to Savannah, Georgia derailed, 47 people died in the horrific rash that followed. Twenty years later, one of the passengers looks back at that night with mixed emotions.

It was the midnight train to Georgia. I was 20 years old, a student travelling after a working holiday. Now I was leaving New Orleans, on board the sleeper bound for Savannah. This was the Sunset Limited, "the only true transcontinental passenger train in American history,"Wikipedia now tells me. At the time, it was just another train I was free to ride with my Amtrak pass, as I zigzagged the US (Grand Canyon to Chicago, Chicago to Louisiana) in the last weeks before second year at university. It was the middle of the night. I got as comfy as I could in my upright seat – not very comfy, that is – and settled in for a long journey.
I never got to Savannah, Georgia; I've never been yet. At 2.45 that morning, a towboat got lost in thick fog in a bayou in Alabama. The barges it was pushing struck a rail bridge that crossed the bayou, and dislodged the track. At 3am, my train hit that twisted track at 70mph, and derailed, destroying the bridge. The lead locomotive crashed into the mudbank and exploded. Several other carriages plunged into the water, causing a fuel spill and a fire. Forty-seven people were killed, some by drowning, some by fire. It was, and remains, the deadliest train crash in Amtrak's history.
That was 20 years – or exactly half my lifetime – ago, which is a realisation to set the head spinning. I haven't thought of the crash much recently, although I enjoy telling the story on the rare occasions I find someone I haven't told it to already. It's my party piece. It's also my pension, after a fashion. When I got home from the States, I was contacted by a Californian legal firm with the formidable name of Kananack Murgatroyd Baum & Hedlund. They wanted to sue on my behalf. I had qualms, but my mum and dad overrode them. I filled in a form recounting my experience, and a few months later was awarded thousands of dollars in compensation. It paid me through university, then allowed me to earn next to nothing for a year or two while I found myself a career.

I've always felt uneasy about that: profiting from something that killed 47 people. And about converting the experience into a ripping yarn, replete with details of my satellite link-up to Lorraine Kelly in the GMTV studio, and how the Scottish press dubbed me "Crocodile Dundee" – because the bayou was reportedly full of alligators, and I come from near Dundee. I feel that I ought to have paid for that profit and those stories with more pain or discomfort. That was certainly the expectation of others. From the moment I stepped off that burning train, the media, my friends and practically everyone I ever spoke to about it looked for trauma, or some kind of imaginative horror that my life nearly ended. But I'm not sure I ever felt any such thing.
To jog memories, I just read a news report from the time, in which I called the experience "harrowing". But I can't remember being "harrowed". I think I was just saying what the journalist wanted to hear. Here's what happened to me. I was woken, and half-thrown from my seat, by the sensation of the train pitching forwards, then violently to one side. It was dark when I opened my eyes, and the next thing I remember, the passengers on my carriage were filing up the aisle and out of the train. I retain a groggy impression of something (a blaze? A commotion?) that we were moving away from. But I don't remember panic. I thought the train had derailed, that's all. An inconvenience, not a catastrophe.
We exited the train, and sat on an embankment by the track, waiting to be told what to do. What happened next, I recall only in fragments: an awareness that we were next to water, that something was happening in the water. A group of people – myself included – gesticulating to people in the water and guiding them ashore. This was later represented in the Sun with the headline "Scot is hero of US rail crash" but – on a night when real acts of heroism did take place – I was no hero. Just an uncertain young man, following other people's lead, on a foggy night, now further fogged by passing time.

After several hours, ambulances and coaches arrived. We were wrapped in blankets and transferred to a hotel in Mobile, Alabama, 10 miles away. We spent a couple of days here, dressed in other people's emergency-relief clothes while waiting to see if our bags would be recovered. It was here that I first saw TV footage of the incident – and appreciated the scale of what I'd been involved in. I saw the collapsed bridge, the train carriages jack-knifed, half-submerged in a swamp and in flames, and another carriage – mine – teetering on what was left of the bridge. (A coastguard official involved in the rescue later professed amazement that any of the coaches remained on the bridge. After this crash, he said, "everybody should be dead. You shouldn't have anybody alive.")
In my hotel room, I phoned home and told my family not to panic when they heard the news. I took a call from ABC's Good Morning, America, asking for an interview, and turned it down. (Who would see it, after all?) I was then contacted by GMTV, asking if I would return to the crash site for a three-way live link-up with Lorraine Kelly in London and my mum and dad in the Dundee studio. But of course, Lorraine, but of course.
That interview is now celebrated in my family not for my grace under pressure as I fielded Lorraine's questions, but because my dad – having risen early at GMTV's request – almost nodded off on live television. It also led to our favourite headline of that period, as the Glasgow Evening Times parlayed my promise of an imminent return into the imperishable front-page phrase "I'LL BE HOME MUM, VOWS GATOR BOY". The bayous of Alabama are home to alligators, but they played no role in this incident. Nevertheless, for weeks after my return home to TV crews at Glasgow airport, I would be hailed as "Crocodile Dundee" in the street and in pubs by strangers and friends.

Then as now, people would ask me about the experience and seem to want to hear how "harrowing" it was. Or at least, that I had some emotional response proportionate to my proximity to death – my own, and that of other people. When one of my closest friends saw a news bulletin about the crash, he heard my name without hearing that I'd survived. Momentarily, he thought I'd been killed, and it affected him very strongly. He couldn't travel by train for a long time afterwards. By contrast, my GMTV interview – preserved for ever in the family annals – reveals an almost inappropriately carefree survivor, cheerfully dismissing reports of death-by-alligator and issuing ironic thanks to Amtrak for their efforts to recover my luggage.
It's only now that for the first time I've made space to think about the crash. At the time, I was 20, and had no reason to dwell on it. The media was less emotive than now; there were no front pages with names and faces of the victims. And my own practical experience hadn't been of a dice with death. It was just confusion, a lot of waiting, some communal effort to assist people in dimly understood need, and only retrospective surprise at what I'd been mixed up in. That was easy enough to shrug off. I wasn't – I'm still not – given to fretting over what might have been. Maybe that's a no-nonsense Scottish upbringing, maybe it's a limited imagination.
Two recent graduates of Exeter University, both aged 22, died on the Sunset Limited. They didn't get the chance I've had, to look back on the experience, put it in the context of a life that long survived it. I didn't know their names until now – nor that a three-year-old girl (the same age as my daughter now) was killed. Finding this out, I feel the horror of the crash far more sharply than before.
But for me, the experience was – strange though this sounds – almost a positive one. It'll always be the Most Remarkable Thing that Happened to Me. It gave me financial security, 15 eye-opening minutes of local fame, and a healthy scepticism of tabloid tales of heroism. Mine is the story you don't hear about after train crashes and act-of-God tragedies – which is fair enough, because it doesn't merit sympathy, nor does justice demand that it be told. But every public tragedy may simultaneously be a personal adventure, and behind every casualty there may be someone – some Teflon-coated kid, some lucky, lucky bit-part player – who's just had the most extraordinary experience of his life.

Era o trem da meia-noite para a Georgia. Eu tinha 20 anos, era mais um estudante viajando depois de um feriado. Neste momento, eu estou deixando Nova Orleans a bordo do trem noturno com destino à Savannah. Era o Sunset Limited, "o único trem de passageiros verdadeiramente transcontinental da história dos Estados Unidos". Naquele momento, ele era apenas mais um dentre muitos outros trens em que eu poderia ter embarcado utilizando o meu passe da Amtrak, visto que eu estava "ziguezagueando" pelos Estados Unidos (do Grand Canyon a Chicago, de Chicago à Louisiana) aproveitando minhas últimas semanas de férias antes do início de meu segundo ano na Universidade. Às 3 horas da madrugada, meu trem se chocou com aquele trilho retorcido a 115km/h, destruindo a ponte. A locomotiva colidiu com um banco de lama e explodiu. Diversos vagões caíram dentro da água, causando um derramamento de combustível seguido de uma explosão. Quarenta e sete pessoas morreram, algumas por afogamento, outras pelo fogo. Esse foi e é até hoje, o pior acidente de trem da história da Amtrak.
Isso aconteceu há 20 anos - exatamente na primeira metade da história da minha vida - o que é algo para se refletir muito a respeito. Recentemente, eu não tenho pensado tanto sobre o acidente, embora me sinta bem ao relatá-lo nas raras ocasiões em que encontro pessoas para as quais eu ainda não o tenha feito. É quando me exibo um pouco ao entretê-los, deixando aflorar o meu lado artístico. Até certo ponto, isso acaba me distraindo. Quando cheguei em casa depois de minha viagem aos Estados Unidos, fui contactado por um importante escritório jurídico da Califórnia chamado Kananack Murgatroyd Baum & Hedlund. Sempre me senti desconfortável quanto a isto: lucrar sobre algo que tirou a vida de 47 pessoas. E também quanto a converter a minha experiência com o acidente em um episódio cômico aos moldes de "Ripping Yarn" (série cômica da televisão britânica), repleto de detalhes sobre minha transmissão ao vivo com Lorraine Kelly dos estúdios da GMTV. Sempre me incomodou também a maneira como a imprensa escocesa me apelidou de "Crocodilo Dandee" - porque disseram que o pântano era infestado de jacarés e eu venho de uma localidade próxima a Dundee. Sinto que eu deveria ter pago por aquele lucro e por essas estórias com mais dor ou desconforto. Isso era certamente o que os outros esperavam. A partir do momento em que pus os pés fora daquele trem em chamas, a mídia, meus amigos e praticamente todos com os quais eu já conversei sobre o acidente procuravam em mim por algum trauma ou por algum tipo de terror imaginário que teria quase acabado com minha vida. Mas eu não tenho certeza se já senti algo do tipo.
Para reviver as lembranças, acabo de ler uma reportagem daquela época em que eu descrevia minha experiência como "angustiante". Mas não me recordo de me sentir "angustiado". Acho que simplesmente disse o que o jornalista queria ouvir. Eis o que aconteceu comigo: Eu acordei e fui parcialmente arremessado de meu assento, com a sensação de que o trem havia sido empurrado para frente e depois violentamente para o lado. Estava escuro quando abri os olhos e a próxima coisa de que me lembro são os passageiros do meu vagão enchendo o corredor e saindo do trem. Ainda guardo uma lembrança meio entorpecida de alguma coisa (uma chama? uma comoção?) da qual nós nos distanciávamos. Mas não me lembro de pânico. Eu achava que o trem tinha descarrilado, somente isso. Um inconveniente, não uma catástrofe.
Saímos do trem e sentamos sobre uma barragem perto do trilho, esperando que nos dissessem o que deveríamos fazer. O que aconteceu a seguir eu me lembro apenas de fragmentos: uma consciência de que nós estávamos perto da água, de que alguma coisa estava acontecendo dentro da água. Um grupo - incluindo eu - gesticulava para as pessoas que estavam na água e as guiava até a margem. Esse fato foi posteriormente noticiado no The Sun com a manchete "Scot é um heroi do acidente de trem ocorrido nos Estados Unidos"- mas - em uma noite em que se presenciaram atos de heroísmo - eu não fui um heroi. Depois de muitas horas, ambulâncias e ônibus chegaram. Fomos envoltos em cobertores e transferidos para um hotel em Mobile, Alabama, a 16km de distância. Um oficial da guarda-costeira envolvido no acidente ficou espantado ao ver que alguns dos vagões permaneceram na ponte. Depois de um acidente deste tipo, "todos deveriam ter morrido, não se poderia esperar ver ninguém vivo", afirmou.
Do meu quarto no hotel, telefonei para minha casa e pedi a meus familiares que não entrassem em pânico quando assistissem ao noticiário. Recebi uma ligação do jornal ABC's Good Morning, do grupo America News, convidando-me a conceder uma entrevista, convite o qual recusei. (Afinal, quem assistiria?). Contraditoriamente, a minha entrevista concedida à GMTV - preservada para sempre nos anais da família - revela um sobrevivente quase que inadequadamente despreocupado, desconsiderando alegremente os relatos sobre mortes por jacarés e tecendo agradecimentos irônicos à Amtrak por seus esforços em recuperar sua bagagem.

The M2500 incorporates a feed hopper with a capacity of 9m3 and feed conveyor which delivers material to the integrated Prograde P2-75 double deck rinsing screen. From here material is sent to the on board Evowash sand washing plant. Material is stockpiled via the four integrated stockpile conveyors and total plant capacity is 250 tons per hour with a washed sand capacity of 120 tons per hour. With this new system Wakoz Beton are producing 4 washed products – 0-2mm fine sand, 2-8mm grit, 8-16mm aggregate and a 16mm aggregate. Malcolm Construction, Scotland. Malcolm Construction, a part of the Malcolm Group, has become the first Scottish company to benefit from a C&D Waste recycling plant from CDE Global. The plant includes an M2500 E4, an Aggmax 83 producing 2 grades of sand and 4 aggregates, and an Aquacycle system for water recycling and waste management. Malcolm Construction is a recognised leader in the waste management, reuse and recycling of construction and demolition waste. The 100 ton per hour recycling plant is based at the Malcolm Construction landfill site in Irvine, near Prestwick airport.
Wlodan, Poland. Wlodan have introduced the M2500 mobile washing plant from CDE Global to their operations on the outskirts of Lodz in central Poland. The new washing plant is being used to process a variety of materials including sand and gravel from Wlodan’s own quarry as well as crushed granite. The M2500 washing plant is processing -32mm material which has been pre-screened by Wlodan. The plant includes a feed system, double deck rinsing screen, integrated Evowash sand washing plant and stockpile conveyors. With between 70% and 90% of the sand and gravel feed material falling in the -2mm range the bulk of production is a -2mm sand product. In addition the M2500 produces a 2-16mm and 16-32mm washed aggregate. The 2-16mm material is subsequently screened to form a 2-8mm and 8-16mm product. Although at present the new washed material is being used by Wlodan within their own asphalt and readymix concrete plants there has been significant interest in the washed -2mm sand product from companies involved in the manufacture of precast concrete.
DP Williams, UK. This 100tph Sand and Aggregate Washing plant, situated in North Wales, consists of a M2500 E4, an Aggmax Portable Logwasher and an Aquacycle Thickener. The feed material is 40% silt and heavily clay bound. The plant produces 10mm, 20mm and 40mm clay free aggregate suitable for concrete sand.

Projetos Globais realizados pela CDE. Wakoz, Polônia. Planta de Lavagem para a Wakoz Beton. A M2500 possui um funil de alimentação com capacidade de 9m3 e um transportador de alimentação que conduz o material bruto a ser processado até a Prograde P2-75, uma tela de lavagem de dois andares integrada ao sistema. De lá, o material segue em direção à Evowash, planta de lavagem de areia, passando então por quatro transportadores integrados, até ser empilhado. A capacidade total da planta é de 250 toneladas por hora, com uma capacidade para lavagem de até 120 toneladas de areia por hora. Com esse novo sistema, a Wakoz Beton está obtendo 4 produtos finais já lavados: areia fina, de 0 a 2mm; areia grossa, de 2 a 8mm; agregados, de 8 a 16mm e agregados acima de 16 milímetros de espessura. Esses produtos têm sido utilizados em plantas de produção de concreto operadas pela Wakoz Beton, que está fornecendo esses materiais para uma variedade de projetos na região, destacando-se como o mais importante a construção de um novo anel viário ao sul de Gdansk.

Essa planta incorpora uma unidade M2500 E4, uma Aggmax 83 produzindo 2 tipos de areia e 4 tipos de agregados e um sistema Aquacycle para reciclagem de água e gerenciamento de resíduos. A Malcolm Construction é uma líder reconhecida nas áreas de gerenciamento, reutilização e reciclagem de resíduos de construção e demolição. A planta de reciclagem com capacidade de 100 toneladas por hora está baseada na área do aterro que a empresa possui em Irvine, próximo ao aeroporto de Prestwick.
Wlodan, Polônia. A companhia Wlodan, localizada nos arredores de Lodz, Polônia central, introduziu em suas operações a planta de lavagem móvel M2500 da CDE global. A unidade está sendo usada para processar uma variedade de materiais, incluindo areia e cascalho (provenientes da pedreira da própria Wlodan), bem como granito esmagado. A planta de lavagem M2500 tem sido utilizada para processar o material de 32mm de espessura previamente selecionado pela Wlodan. A planta inclui um sistema de alimentação, tela de lavagem de dois andares, a planta Evowash para lavagem de areia, bem como transportadores para o material final. Com cerca de 70 % a 90 % do cascalho e areia extraídos do material bruto processado sendo classificados com 2mm de espessura, o produto final obtido é, em sua maior parte, a areia, com 2mm de espessura. Além disso, o M2500 produz outros dois produtos agregados e lavados: um de 2 a 16mm e outro de 16 a 32mm de espessura . O material de 2 a 16mm é posteriormente filtrado para formar outros dois produtos: o primeiro, de 2 a 8mm; o segundo; de 8 a 16mm . Embora, atualmente, o material recém-lavado esteja sendo utilizado pela Wlodon dentro das unidades de produção de concreto e asfalto da própria companhia, tem havido um interesse crescente pela areia lavada de 2mm por parte de empresas envolvidas na fabrição de concreto pré-moldado.
DP Williams, Reino Unido. Esta planta de lavagem de areia e agregados com capacidade de 100tph, situada ao norte do País de Gales, consiste de uma M2500 E4, um Lavador de Pedras Portátil Aggmax e um Engrossador Aquacycle. A matéria-prima bruta processada é composta de uma quantidade superior a 40% de lama e argila . A unidade produz agregados de 10, 20 e 40 milímetros de espessura, 100% limpos de lama ou argila, material ideal para ser utilizado como areia na fabricação de concreto.
QPMC, Qatar. Maior Planta de Lavagem de Areia do Mundo. Tonelagem de Alimentação de 1200 t/h. A CDE venceu uma licitação realizada pelo governo do Qatar, da qual participaram diversos concorrentes internacionais. Firmamos contrato para a realização de dois projetos (retratados nesta imagem) orçados em U$25milhões. O diferencial fundamental para que vencéssemos essa licitação foi a nossa comprovada habilidade em reciclagem de água.
