We have nurses, care coordinators, social workers and others to help you and your health care team. Our goal is to help improve your quality of life. Would you like our help? Call us if you have one or more of these health problems: • Diabetes • Asthma or COPD • High blood pressure, coronary artery disease, or congestive heart failure • A history of many health problems which are hard to control • Special needs and/or disabilities • Trouble identifying or coordinating the services you need Here is what we can do for you: • Provide personal care and coordination of services by our nurse case manager and care coordinators • Educate on insurance benefits for you and your health care team • Educate you about your condition(s), treatment and medications • Provide you with ways to self-manage your condition(s) • Assist you in communicating your needs and questions to your health care team • Help with getting services near you, including those offered by mental health providers and/or schools • Work with you and your provider(s) to arrange the best possible care • Make sure you get all of the services you need At Meridian Health Plan, our Complex Case Manager helps you to: • Understand your situation and take care of your health • Learn more about the medicines prescribed by your provider(s) • Create and fulfill your own plan of care Want to learn more?
