GENERAL CONSENT FOR MEDICAL SERVICES
I request and authorize ABC Hospital, its agents and employees (“Hospital”) and my physicians, their associates and assistants (“Physicians”) who may attend to me during the hospitalization, emergency service, or outpatient visit to provide and perform such medical and surgical care, tests, procedures, drugs and other services and supplies as are considered advisable by my Physician for my health and well being. I understand this may include, but is not necessarily limited to anesthesia, pathology, radiology, services and other special services and tests, including tests for communicable diseases, ordered by my Physician.
I acknowledge that neither the Hospital nor my Physicians have made any representations, warranties or guarantees as to results or cures to me, and I have not relied upon any such representations, warranties or guarantees. I also agree to take full responsibility for leaving the Hospital prior to being discharged by my Physician and hold the Hospital harmless for any liability in connection with my leaving the Hospital against medical advice.
ACKNOWLEDGEMENT OF LIMITATIONS UPON HOSPITAL’S RESPONSIBILITY
FOR ACTS OF INDEPENDENT CONTRACTORS/NON-EMPLOYEES
I acknowledge that as a patient of the Hospital, I may receive treatment, services and/or care from health care providers who: (1) are not employees of the Hospital, but have a contract with the Hospital to provide services, including, but not limited to, emergency physicians, anesthesiologists, radiologists, pathologists, and mental health providers; (2) have no employment or other contractual agreement with the Hospital including, but not limited to, paramedics; and/or (3) are not employees of the Hospital but are in private practice with privileges to provide services at the Hospital.
I further acknowledge that the above-referenced health care providers are not employees or agents of the Hospital and I understand that the Hospital is not liable for the acts of these providers.
CONSENT FOR BLOOD-BORNE INFECTIOUS DISEASE TESTING
I authorize the Hospital to test for blood-borne infectious diseases, including, but not limited to hepatitis, Acquired Immune Deficiency Syndrome (“AIDS”), and Human Immunodeficiency Virus (“HIV”) if a Physician orders such test(s) or if ordered by protocol. The results of these tests will become part of my confidential medical record. Failure to consent to these tests will not result in denial of admission to the Hospital.

RELEASE OF INFORMATION
The Hospital may disclose medical information about you to a friend or family member who is involved in your medical care. If admitted, unless you tell the Hospital otherwise, the Hospital will list in the patient directory, your name, and location in the Hospital. Your religious affiliation may be disclosed only to a clergy member, even if they do not ask for you by name (See Notice of Privacy Practices for more information on disclosures and uses of your protected health information).

PERSONAL VALUABLES
The Hospital requests that all items of personal property, including but not limited to purses and wallets, which are not required during your visit be sent home with a relative or friend. If this is impossible, the Hospital will place valuables in the Hospital’s safe upon request. I also understand that all valuables and personal items including clothing, dentures, eyeglasses and hearing aids, are my responsibility and the Hospital will not be liable for any loss or damage to such items.

FINANCIAL CONSENT/ASSIGNMENT OF BENEFITS AND RELEASE OF INFORMATION
I hereby assign payment to (1) the Hospital; (2) health care providers who are not employees of the Hospital, but who have a contract with the Hospital to provide services, such as Emergency Physicians, Anesthesiologists, Radiologists, Pathologists, and Mental Health providers; and (3) health care providers who have no employment or other contractual agreement with the Hospital, such as paramedics, and authorize them to release a copy of my medical records and to release any other information necessary for them to obtain my assigned payment from my insurance, Medicare, Medicaid, workers’ compensation carriers, and social security administrators with whom I have coverage or form whom benefits are, or may become, payable to me, including settlements or judgments from the incident for which I am receiving treatment. I acknowledge that the providers in categories two (2) and three (3) above are not employees or agents of the Hospital and I understand that the Hospital is not liable for the acts of the providers in categories two (2) and three (3). I agree to pay, when billed or requested by the Hospital, any amount charged for Hospital services not covered by the above payers.
ACKNOWLEDGEMENT OF FINANCIAL RESPONSIBILITY
In consideration of the services provided by the Hospital and the Physicians, I agree to pay all the charges promptly when due, including if services are not covered or my private insurance has determined the services I received are not medically necessary. For non-installment payment arrangements, a late fee of $20.00 may be added to my account each month that the payment is not posted by the due date as indicated on my statement. If my account is forwarded to a collection agency or attorney: (1) whether or not legal proceedings are instituted, a collection agency fee not to exceed 20% of the account balance or One Thousand Dollars ($1,000), may be added to my account balance forwarded; and (2) I will be responsible for any court costs, reasonable attorney fees, and interest as allowed by Indiana statute, incurred in the collection of my account.
I understand that my insurance company or other third-party payer may require prior authorization before I receive healthcare services at the Hospital, and that it is my responsibility to obtain any prior authorization required.
A written copy of my patient bill of rights is available at each visit.
In the event that I have been admitted, information regarding in patient admission and observation admission status has been made available to me.

Doy constancia de que ni el Hospital ni los Médicos me han prometido ni garantizado nada respecto a un resultado favorable o a la cura de mi dolencia, y que este consentimiento no depende de promesa ni garantía alguna. Voluntariamente y sin reservas doy mi consentimiento para que se me suministren los servicios mencionados en el párrafo anterior. Asimismo, aviso que estoy al tanto de que los proveedores de atención médica a que se refiere el párrafo anterior no son ni empleados ni representantes del Hospital, y que por lo tanto el Hospital no se responsabiliza de sus acciones.
CONSENTIMIENTO PARA ANÁLISIS DE ENFERMEDADES TRANSMITIDAS POR CONTACTO CON LA SANGRE
Si el médico lo ordena o se exige por protocolo, autorizo al Hospital para que se me hagan análisis para descartar enfermedades que se transmiten por contacto con la sangre, entre otras: la hepatitis, el síndrome de inmunodeficiencia adquirida (SIDA) y el virus de inmunodeficiencia humana (VIH). Los resultados de dichos análisis se incluirán en mi expediente médico confidencial. No se me negará el ingreso en el Hospital aunque no dé permiso para que se me hagan estos análisis.

ACUERDO FINANCIERO / ASIGNACIÓN DE PRESTACIONES Y DIVULGACIÓN DE INFORMACIÓN
Por la presente asigno (1) al Hospital; (2) a los proveedores de atención médica que no son empleados del Hospital, pero que tienen contrato con el mismo para brindar servicios, tales como los médicos de la sala de emergencia, anestesiólogos, radiólogos, patólogos y proveedores de cuidados de salud mental; y (3) a los proveedores de atención médica que no son empleados del Hospital ni tienen ningún tipo de contrato con el mismo —por ejemplo, los paramédicos— para que reciban los pagos que asignen el seguro, Medicare, Medicaid, cualquier portador del seguro de compensación laboral o cualquier administrador del Seguro Social que cubra dichos pagos o cuyas prestaciones sean ahora o en el futuro pagaderas a mi persona. Esta asignación incluye pagos derivados del suceso a razón del cual esté recibiendo tratamiento, entre otros, aquellos pagos acordados por convenio o asignados por orden judicial. Por la presente también los autorizo para que entreguen una copia de mi historial médico y divulguen cualquier otra información que sea necesaria para poder recibir los pagos asignados de las entidades aquí mencionadas. Doy constancia de que entiendo que los proveedores de las categorías (2) y (3) no son empleados ni representantes del Hospital. Asimismo entiendo que el Hospital no se responsabiliza por las acciones de los proveedores de las categorías (2) y (3). Cuando el Hospital me pase la factura o me lo pida, me comprometo a pagar toda suma que me cobre por los servicios que no cubran las entidades de seguros aquí mencionadas.
CONSTANCIA DE RESPONSABILIDAD FINANCIERA
En consideración de los servicios suministrados por el Hospital y por los Médicos, acuerdo pagar todos los cargos a su debido tiempo, incluso los servicios que mi seguro médico particular no cubra o que haya determinado que no son médicamente necesarios. Si no tengo un acuerdo con el Hospital para hacer el pago a plazos, por cada mes de retraso se le añadirán $20.00 a mi deuda. La fecha de pago está indicada en la factura. Si hay que enviarle la cuenta a una agencia de cobros o a un abogado: (1) ya sea que se entable o no proceso legal, se le añadirá a la cuenta la comisión de cobros de la agencia, la cual no pasará del 20% del saldo de lo que debo o de mil dólares ($1.000); (2) me responsabilizaré de pagar los costos contraídos para cobrar la factura; estos podrían comprender: cargos de los tribunales, honorarios razonables de abogado, e intereses, conforme lo permita la ley de Indiana.
Entiendo que puede que el seguro médico y otros terceros que se responsabilizan de pagar la factura exijan que se obtenga autorización previa para los servicios que yo reciba en el Hospital y que soy el responsable de obtener dicha autorización.
En cada visita al Hospital tendré acceso a una copia de los derechos del paciente.
En caso de que haya ingresado en el hospital, se me dará información acerca de la diferencia entre ingresar en el Hospital como paciente e ingresar para propósitos de observación médica.

Montserrat Zuckerman Biography
Montserrat Zuckerman has been a freelance translator and interpreter working in English and Spanish since 1998. She lives in the Greater Boston area and counts several midsize businesses and not-for-profit organizations among her clients. Her translation work focuses mainly on health care, legal and general business texts.
Zuckerman immigrated to the United States from Venezuela, where she was born and raised and where she graduated from the Liceo "Francisco de Miranda" in Los Teques. She received a B.A. in Biology from New York University and a B.S. in Nursing from the University of Minnesota. Throughout her university career Zuckerman studied languages and attained high degrees of proficiency in French, Swedish and Portuguese. In 2005 she received a Certificate in Translation Studies from IUPUI. In addition to being a certified court interpreter (Indiana Supreme Court program), Zuckerman is a Certified Healthcare Interpreter™.
Zuckerman is a member of the American Translators Association, National Association of Judicial Interpreters and Translators, New England Association of Translators and Interpreters, Certification Commission for Healthcare Interpreters, International Medical Interpreters Association, and founding member and former secretary of the Midwest Association of Translators and Interpreters.
