Translation - English TETRALOGY OF FALLOT (TOF)
TOF is a congenital heart defect given by a combination of abnormalities that cause poor pulmonary blood circulation and low oxygen levels in the blood.
In a healthy heart, the oxygen-poor blood returns to the right atrium and goes through the right ventricle. The right ventricle then pumps the blood to the lungs to be oxygenated. The oxygen-rich blood in the lungs returns to the left atrium and it is pumped to the body through the left ventricle.
HEART WITH TOF
A heart affected by TOF presents four abnormalities: ventricular septal defect or VSD (a hole between the two ventricles); a narrowing of the muscle under the pulmonary valve that may restrict the pulmonary circulation resulting in a right ventricular hypertrophy; abnormal position of the aorta (the aorta valve is positioned above the VSD); the right ventricle swells (ventricular hypertrophy) given it is working against the obstruction in the outflow towards the pulmonary artery.
The Tetralogy of Fallot symptoms may vary from one patient to another. Children presenting a major narrowing of the area leading to the pulmonary artery will have reduced amount of blood circulating towards the lungs. This decrease of the blood circulation to the lungs may lead to lower blood oxygen levels and cyanosis (bluish color to the skin). They can also undergo extreme cyanosis periods, also known as "Tet spells" or "hypercyanotic attacks", which may put their lives at risk.
TREATMENT
A medicine can be used to keep open a vessel between the aorta and the pulmonary artery that normally gets closed after birth, in order to make blood circulate to the lungs in patients with severe obstruction of blood circulation, from the right ventricle to the pulmonary artery.
In some cases, a provisional tube or shunt can be surgically placed to connect the aorta to the pulmonary artery. Normally, a complete surgical reconstruction can be done during the first six months of life.
