Source text - Javanese 経口投与後、消化管から吸収された薬物は、門脈を経て肝臓に運ばれる。肝臓に到達した薬物はその一部が薬物代謝酵素により代謝され、残った薬物のみが体循環血液中へと運ばれる。また薬物によっては、消化管内の腸内細菌や小腸粘膜に存在する酵素により代謝を受けるものもある。したがって、薬物を経口投与した場合には、どのくらいの量の薬物が代謝を受けずに未変化体のままで体
循環血中に移行するかがその後の薬理作用の発現に大切であり、これを生体内利用率という。経口投与された薬物は吸収された後、必ず門脈から肝臓を通過し代謝を受けるので、これを初回通過効果といい、経口投与された薬物の生体内利用率を決める最も重要な因子である。ニトログリセリンなどのように初回通過効果を受けやすい薬物を経口投与しても薬理作用はほとんど期待できない。
比較的初回通過効果を受けやすい薬物にはパーキンソン病治療薬レボドパやペプチド性の医薬品、β遮断薬プロプラノロール、抗不整脈薬リドカイン、麻薬性鎮痛薬モルヒネ、抗うつ薬イミプラミンなどがある。
Translation - English After being orally administered, the drug absorbed from the digestive tract is delivered to the liver via the portal vein. Part of the drug reaching the liver is metabolized by a drug metabolizing enzyme and only the remaining quantity of the drug is delivered into the circulatory system. Also, depending on the drug, some drugs are metabolized by enzymes existing in intestinal bacteria or intestinal mucosa of the internal digestive tract. Therefore, when a drug is orally administered, the amount of the drug that is not metabolized and is delivered as it is into systemic circulation is important to the expression of the medicinal action. It is called the extent of bioavailability. After the orally administered drug is absorbed, it is metabolized to some extent, since it must pass through the liver via the portal vein. This is called the first pass effect; and it is the most significant factor to determine the extent of bioavailability of an orally administered drug. Significant medicinal action will not necessarily be expected if a drug which is subject to the first pass effect is orally administered, such as nitroglycerin. Some drugs which are relatively subject to the first pass effect are Levodopa, as treatment drug for Parkinson's disease; peptidic drugs; β-blocker propranolol; the antiarrhythmic agent lidocaine; the narcotic analgesic morphine; and the antidepressant imipramine.

シマブ(製品名レミケード)などである。さらに、多くの抗体薬が欧米で臨床開発中であり、近い将来、抗体薬はさらに増加していくものと期待されている。抗体は、生物が進化の過程で獲得した薬ともいえる。環境の変化とともに出現してくるいろいろな病原体に対して働く。病原体のみならず、がん細胞や不要となった自己細胞にも結合し、それらを排除して生体の恒常性を維持している。だから、抗体は元来、優れた抗菌剤、抗ウイルス剤、抗が
ん剤の特性を持っている。さらに、ほとんど無限とも思えるいろいろな抗原に対して選択的な抗体が得られる。この選択性こそが薬のもっとも基本的な特性である。薬は疾患に関与する標的タンパク質に選択的に結合して、その機能を阻害、あるいは促進することにより治療効果を発揮する。
Translation - English Treatment drugs using antibodies (“Antibody drugs”) are getting more attention in Japan. It is because Antibody drugs are emerging with significant treatment effects against cancer and autoimmune disorders,
where low molecular weight compound drugs were not as effective as it was hoped. Examples of antibody drugs are a treatment drug for non- Hodgkin's lymphoma (rituximab - drug product name: Rituxan), a treatment drug for breast cancer (trastuzumab - drug product name: Herceptin), and a treatment drug for rheumatoid arthritis (infliximab - drug product name: Remicade). Furthermore, many of the antibody drugs are already under clinical development in Europe and the U.S., and in the near future it is expected that the numbers of antibody drugs will increase. It can be said that an antibody is a drug which an organism has acquired in the course of evolution. It works against different types of pathogens emerging through environmental changes. It bonds not only to pathogens, but also to cancer cells and the body’s own useless cells and it maintains homeostasis by excluding them. Therefore, an antibody has the fundamental characteristics of an excellent antimicrobial, antiviral and anticancer agent. In addition, selective antibodies can be acquired against almost limitless different types of antigen. This selectivity itself is the most primary characteristic of drugs. Drugs exhibit more effective treatment by selectively bonding to target proteins within the patient, either blocking their function or expediting it.
