1) Participation
You are being invited to participate in this research project because your doctors would like to include samples of some of your tissue removed during surgical procedures as part of your treatment, some blood and some saliva, in a research study. You have the right to know that consent is purely voluntary, that your permission to participate in research may be withdrawn at any time, and that you may discontinue participation in all or part of the research without affecting your present or future medical care. This document contains important information that you should know before allowing your blood to be studied. Please take your time to review this form.

2) Purpose of the study
The purpose of the study is to find better ways to treat head and neck cancers. The study has special significance for Indians as cancers ofe head and neck are among the commonest cancers seen in Indians.
3) Approximate number of Participants and the Expected Duration of your
Participation in the study.
About 150 patients are expected to take part in this study. You will be part of this study for two years from the date of your enrollment, but the research activities will continue for three and a half years .
4) Study Procedures
If you agree to participate in this study, you will be asked to sign this consent form. A small sample of the tissue, blood and saliva obtained from you will be collected, frozen, and kept in the tissue repository at Prince Aly Khan Hospital under the direction of Dr. ______________. The blood placed in this area will not be needed for the diagnosis or management of your health. In terms of blood collection, we may ask for no more than 20 ml of blood during the entire duration of the study, which is equal to two teaspoons.
Blood, tissue and saliva samples obtained for this research study may be tested immediately or may be frozen and examined later. The frozen samples of your blood cells will be kept in the above-mentioned tissue repository. Samples to be used right away will be given to the researchers at Prince Aly Khan Hospital for histopathological work and some samples will be sent to partner laboratories for research work. Samples may also be stored for research purposes at a later date. Some material may be used to establish a cell line that may be shared in the future with other researchers. A cell line is a group of cells that will grow continuously in the laboratory. Cell lines may be useful because of the special actions of the cells and/or the products they may produce.
If you prefer not to allow storage of your blood cells for research tests, you may refuse at any time. Your frozen samples will then be destroyed. To withdraw from the study please contact (in writing) any of the individuals listed on the first page of this document.
Blood stored in the tissue bank will be linked with disease information including patient identification known only to the Principal Investigator. This is necessary to allow for linking patient disease and disease outcome. While your blood is being studied, the tissue specimen will be coded so that personal information will not be available to the investigators.
Your personal contact information is important for Prince Aly Khan Hospital research team to contact you during the study. Your health information and results of tests and procedures are being collected as part of this research study and for the advancement of medicine and clinical care. The Principal Investigator may also use the results of these tests and procedures to treat you.
In addition to samples, any information that is felt to be disease related will be collected. This will include, but not be limited to, history on smoking and alcohol use, location and extent of your disease, and any previous treatment. Information on the outcome of therapy will also be maintained.
All of this information will be stored in a computer database and used for research purposes only. Your research information will be coded with a code number in order to protect your confidentiality. Your name will not be used at any time in association with the research materials and the data will be maintained under password protection and lock and key.
5) WHAT ARE THE RISKS OF THIS STUDY?
Donating blood, tissue and saliva samples for these research purposes will not add any additional risks to the surgery, procedures or other treatment or diagnostic procedures you have had or will have. It is emphasized that you will not undergo any surgical operation or procedure, due to your participation in this study, that is not part of the treatment planned or to be planned for you.
When blood and saliva samples are requested, a trained technician, nurse or doctor will obtain the samples. Tissue will be obtained only by a trained doctor. You will be checked closely to see if complications due to blood drawing or tissue collection occur. Complications that may arise as a result of blood collection are typically minimal and may include bruising, swelling, fainting, and infection and that of tissue sample
collection will be bleeding, infection or pain at the site of collection. Medical care will be provided if the need arises.
6) WHAT ARE THE BENEFITS OF THIS STUDY?
Your contribution to this study will be for research purposes only. You will not receive personal medical information from this study. You will not be paid for participating in this study.
You should not expect your condition to improve as a result of participating in this research study. However, new information learned from this study about cancer could help doctors identify new methods for early detection, new treatments, and possibly new cures. This study may provide information leading to the discovery of possible markers for detection of aggressive forms of cancer. You will not receive any personal benefit from being in this study.
Use of Specimens:
Any blood or tissue sample obtained for the purposes of this study becomes the property of Prince Aly Khan Hospital. The hospital and / or its research partnersmay retain, preserve, or dispose of these specimens and may use these specimens in research which could result in patent filing or commercial applications. You will not receive money for donating blood or tissue sample nor will you receive money for any future commercial ventures. Your blood and tissues will be used for research only.
7) WILL IT COST ME ANYTHING TO BE IN THIS STUDY?
Specimens [blood, tissue and saliva] will be drawn in conjunction with routine lab work and thus no additional charges will be incurred. As the research activity is confined to the laboratory, and involves no procedure, surgery, investigation or treatment outside the scope of the planned diagnostic workup and treatment, no funds have been set aside as compensation due to injury resulting from the research study.
If you agree to participate in this study, your specimens will be stored at no cost to you. Any research tests involving your samples will be performed at no cost to you.
8)WILL I BE PAID FOR PARTICIPATING?
There will be no financial compensation for participation in this study.
9)WHO IS FUNDING THIS STUDY?
The study is being funded by the Council for Scientific and Industrial Research, Government of India
10) WHAT ABOUT CONFIDENTIALITY?
All information collected in this study will be kept strictly confidential, except as may be required by law. Your blood, tissue and saliva samples used in this research study will be stored with a confidential code and your name will not be included with any data shared with other investigators. The medical information produced as a part of this study will not become part of your medical record.
11) IS BEING IN THIS STUDY VOLUNTARY?
Taking part in this study is voluntary. You may choose not to take part or may leave the study at any time. Leaving the study or choosing not to take part will not result in any penalty or loss of benefits to which you are entitled.
Disclaimer/Withdraw:
Your participation in this research study is completely voluntary. If you choose not to participate, that will not affect your present or future care at Prince Aly Khan Hospital. If you decide to participate, you are free to withdraw your consent and discontinue participation at any time without prejudice to your present or future care. The investigators may retain the identified samples as part of your routine clinical care but not for additional research. Alternatively any information linking your identity to the blood, tissue or serum sample can be permanently removed so that the samples may still be used for other research that does not require clinical data.

2) अध्ययन का उद्देश्य
इस अध्ययन का उद्देश्य सिर और गले के कैंसर का इलाज करने के बेहतर तरीकों का पता लगाना है. यह अध्ययन भारतीयों के लिए खास मायने रखती है क्योंकि भारतीयों में सिर और गले के कैंसरों का पाया जाना अब आम बात हो गई है.
3) प्रतिभागियों की अनुमानित संख्या और इस अध्ययन में आपकी भागीदारी की प्रत्याशित अवधि.
इस अध्ययन में लगभग 150 रोगियों के भाग लेने की उम्मीद है. आप अपने नामांकन की तारीख से दो साल तक इस अध्ययन का हिस्सा होंगे लेकिन अनुसंधानात्मक गतिविधियां साढ़े तीन साल तक चलती रहेंगी.
4) अध्ययन प्रक्रिया
अगर आप इस अध्ययन में भाग लेने के लिए राजी हैं तो आपसे इस सहमति फॉर्म पर हस्ताक्षर करने के लिए कहा जाएगा. आपसे ऊतक, रक्त और लार का एक छोटा सा नमूना संग्रह किया जाएगा, उसे जमा दिया जाएगा और डॉ. _______________________ के दिशा निर्देश के तहत प्रिंस अली खान हॉस्पिटल के उतर भंडार में रख दिया जाएगा. आपके स्वास्थ्य के प्रबंधन या डायग्नोसिस (निदान) के लिए इस क्षेत्र में रखे जाने वाले रक्त की जरूरत नहीं पड़ेगी. रक्त संग्रह के मामले में अध्ययन की सम्पूर्ण अवधि के दौरान हम आपसे आपका 20 मिली से ज्यादा रक्त नहीं ले सकते हैं जो लगभग दो चम्मच के बराबर होता है.
इस अनुसन्धान अध्ययन से प्राप्त रक्त, ऊतक और लार का तुरंत परीक्षण किया जा सकता है या बाद में उनका परीक्षण करने के लिए उन्हें जमा कर रखा जा सकता है. आपकी रक्त कोशिकाओं के जमे हुए नमूनों को ऊपर उल्लिखित ऊतक भंडार में रखा जाएगा. इस तरह से इस्तेमाल किए जाने वाले नमूनों को हिस्टोपैथोलॉजिकल कार्य के लिए प्रिंस अली खान हॉस्पिटल के अनुसन्धानकर्ताओं को दे दिया जाएगा और कुछ नमूनों को अनुसन्धान कार्य के लिए साथी प्रयोगशालाओं में भेज दिया जाएगा. बाद में अनुसन्धान करने के उद्देश्य से भी इन नमूनों को भंडारित किया जा सकता है. एक कोशिका रेखा (सेल लाइन) को स्थापित करने के लिए कुछ सामग्रियों का इस्तेमाल किया जा सकता है जिन्हें अन्य अनुसन्धानकर्ताओं के साथ भविष्य में शेयर (साझा) किया जा सकता है. यह कोशिका रेखा प्रयोगशाला में लगातार विकसित होने वाली कोशिकाओं का एक समूह है. कोशिकाओं और/या उनसे उत्पन्न होने वाले उत्पादों की विशेष क्रियाओं की वजह से ये कोशिका रेखाएं उपयोगी साबित हो सकती हैं.

5) इस अध्ययन के कौन-कौन से जोखिम हैं?
इन अनुसन्धान उद्देश्यों के लिए रक्त, ऊतक और लार के नमूनों का दान देने से आपकी सर्जरी, प्रक्रियाओं या अन्य इलाज या डायग्नोस्टिक प्रक्रियाओं में कोई अतिरिक्त जोखिम शामिल नहीं होगा. इस बात पर जोर देकर कहा जा सकता है कि इस अध्ययन में आपकी भागीदारी की वजह से आपको किसी सर्जिकल ऑपरेशन या प्रक्रिया से नहीं गुजरना पड़ेगा जो आपके लिए बनाई गई योजना के तहत किए जाने वाले इलाज का हिस्सा न हो.
रक्त और लार के नमूनों को लेने के दौरान एक प्रशिक्षित तकनीशियन, नर्स या डॉक्टर द्वारा ये नमूने लिए जाएंगे. ऊतक को केवल किसी प्रशिक्षित डॉक्टर द्वारा ही लिया जाएगा. आपकी बारीकी से जांच की जाएगी कि कहीं रक्त या ऊतक के संग्रहण की वजह से कोई जटिलता तो उत्पन्न नहीं हो रही. रक्त संग्रहण के परिणामस्वरूप उत्पन्न होने वाली जटिलताएं आम तौर पर काफी मामूली होती हैं और इनमें थकावट, सूजन, बेहोशी और संक्रमण शामिल हो सकता है और ऊतक के नमूने का संग्रह लेते समय संग्रहण स्थल पर रक्त स्राव, संक्रमण या दर्द होगा. जरूरत पड़ने पर चिकित्सीय देखभाल की व्यवस्था की जाएगी.
6) इस अध्ययन से कौन-कौन से लाभ हैं?
इस अध्ययन में आपका योगदान केवल अनुसन्धान के उद्देश्य के लिए होगा. इस अध्ययन से आपको व्यक्तिगत चिकित्सीय जानकारी प्राप्त नहीं होगी. इस अध्ययन में भाग लेने के लिए आपको कोई भुगतान नहीं किया जाएगा.
आपको इस अनुसन्धान अध्ययन में भाग लेने के परिणामस्वरूप अपनी हालत में सुधार होने की कोई उम्मीद नहीं करनी चाहिए. हालांकि, कैंसर के बारे में इस अध्ययन से हासिल होने वाली नई जानकारी से डॉक्टरों को कैंसर का जल्दी पता लगाने, उसके नए उपचार और संभवतः उसके देखभाल के नए तरीकों की पहचान करने में मदद मिल सकती है. हो सकता है कि इस अध्ययन से कुछ ऐसी जानकारी हासिल हो जाए जिससे कैंसर के आक्रामक रूपों का पता लगाने के लिए संभावित मार्करों की खोज की जा सके. इस अध्ययन में शामिल होने की वजह से आपको कोई व्यक्तिगत लाभ प्राप्त नहीं होगा.
नमूनों का इस्तेमाल:
इस अध्ययन के उद्देश्य से प्राप्त किया जाने वाला रक्त या लार का कोई भी नमूना प्रिंस अली खान हॉस्पिटल की संपत्ति बन जाता है. अस्तपाल और/या इसके अनुसन्धान साथी इन नमूनों को बनाए रख सकते हैं, संरक्षित कर सकते हैं या नष्ट कर सकते हैं और इनका इस्तेमाल किसी ऐसे अनुसन्धान में कर सकते हैं जिससे पेटेंट फाइलिंग या वाणिज्यिक आवेदन का परिणाम प्राप्त हो सके. रक्त या ऊतक के नमूने का दान करने के लिए आपको कोई रकम नहीं दी जाएगी और न ही किसी भावी वाणिज्यिक उद्यमों के लिए आपको कोई रकम हासिल होगी. आपके रक्त और ऊतकों का इस्तेमाल केवल अनुसन्धान के लिए किया जाएगा.
7) क्या इस अध्ययन में भाग लेने से मुझ पर कोई खर्च आएगा?
नमूनों [रक्त, ऊतक और लार] को नियमित प्रयोगशाला कार्य के संयोजन के रूप में प्राप्त किया जाएगा और इसलिए इसके लिए कोई अतिरिक्त व्यय नहीं होगा. चूंकि अनुसन्धान गतिविधि प्रयोगशाला तक ही सीमित है और इसमें योजनाबद्ध डायग्नोस्टिक कार्य और उपचार के दायरे से बाहर कोई प्रक्रिया, सर्जरी, अन्वेषण या उपचार शामिल नहीं है इसलिए अनुसन्धान अध्ययन के परिणामस्वरूप होने वाली क्षति के मुआवजे के लिए कोई धनराशि अलग से निर्धारित नहीं है.

Asset Allocation
Investors should periodically review their asset allocation across different assets as the portfolio can get skewed over a period of time. This can be largely due to appreciation/depreciation in the value of the investments.
For example: an investor who had invested Rs. 70 into equities and balance in income funds in June 2004.
During last one year, the BSE Sensex has moved up by 41% while the income funds have grown by 1%. As of end May 05, the equity asset allocation would be 67% into equities and rest into debt – equity exposure would have gone up by 7% (due to appreciation in stock indices). An investor who wants to keep the allocation at 60% should reduce the exposure in equities and shift to debt.
Why is Asset Allocation important?
As the financial goals are diverse, the investment choices also need to be different to meet those needs. No single investment is likely to meet all the needs, so one should keep some money in bank deposits and / liquid funds to meet any urgent need for cash and keep the balance in other investment products/ schemes that would maximize the return and minimize the risk. Investment allocation can also change depending on ones risk-return profile.
Asset allocation decides 70%-80% of the returns over the long term and defines risk for the portfolio. Security selection is secondary and decides only 20-30% of the returns over the long-term.
Investment Strategy-Portfolio rebalancing-Why is it required?
Over the course of the year, the market value of each security within ones portfolio earned a different return, resulting in a change in the allocation pie. This might change the investor's risk profile. While in the short term this may not have an adverse impact, such changes in risk profiles can have a far-reaching impact in the longer run. Portfolio rebalancing is a strategy that allows individuals to keep their risk level in check and minimize risk.
What is re-balancing?
Re-balancing is the process of buying and selling portions of your portfolio in order to set the weight of each asset class back to its original state. In addition, if an investor's investment strategy or tolerance for risk has changed, he or she can use rebalancing to readjust the weight ages of each security or asset class in the portfolio to fulfill a newly devised asset allocation.
