Explanation:
Both in Sweden and in Britain (and no doubt other countries) senior/top managers of hospitals, clinics, etc. are no longer necessarily medics. Therefore responsibility for medical/clinical matters gets split from responsibility for overall running of the organisation.

Further to Chris' comment, here are a couple of pieces from the East Kent Hospitals chart:

Mr NG, Medical Director (Mr, 'cause he's a surgeon)

So it looks as if the term Medical Director can be used further down the tree as well.

neutral  Christopher Schröder: I don't think this is what it is // medical director would be in charge of a whole group of hospitals. Clinical director or lead clinician is what I think it is. That is UK specific though and I'm not sure either.
