Operationssköterska, was my first thought, but I can hardly say I'm sure.

The abbreviation appears quite a few times in a printout of some medical records.

E.g.

Administrativ åtgärd -> [OS] XXX-Test taken....

Anamnes -> [OS] ...various information...

Socialt -> Maria Svensson / line break / [OS] ...various information...

Maria Svensson is a fictional name, but one of the reasons for doubting that [OS] stands for Operationssköterka is that the same person has repeatedly been mentioned at earlier points in the document as Barnmorska/BM.

Could mean "osignerad" ie an entry that has not been confirmed by the writer.

I would not expect the Operationssköterska to be taking a pre-op medical history, ordering tests etc

Asker: I think this might be it. It appears to be one out of three possible states that each entry may be in, by means of some software called ProReNata. Still I believe it would be a bit clunky to write (or read) "unsigned", next to an entry where a name is actually stated, so I guess I'll have to figure out a workaround; leave it as it is, perhaps, and add a translator's note at the footer... Anyway, thank you.
