Am I right in thinking this is a statement that the patient now feels well?

Or whatever you prefer for "está bien"; for example "that he/she is fine now". "Refiere" is a very common verb for patients giving information during consultation. You could imagine a comma after "bien": "Ahora está bien, refiere": He/she is now well, he/she reports.

Of course, you'll use he or she according to the patient's sex, which is presumably clear from the document.
