Medical - Medical (general) / neurology
Slovak term or phrase: P.T. EMG ambulancia
Zamestnávateľ:
P.T. EMG ambulancia

This is on a referral / request form (výmenný list - poukaz). The patient is being referred for neurological tests.

What is the meaning of "P.T. EMG ambulancia", and is there any reason why it should be placed under "Zamestnávateľ"? (I don't see what it has to do with the employer).

Explanation:
P.T. = plným titulom. An old-fashioned phrase typically used for an address in Czech and Slovak if you don't know any specific person name. You can safely skip that.

Dylan, you should claim half of the credits since you were the first one with the EMG part. :)

Asker: Thanks! My question was really about 'P.T.' - I just added 'EMG' to give more context. I thought 'P.T.' was probably something medical. Another mystery solved :)
