This is at the beginning of a medical report and concerns the patient's admission to hospital, and the status of the patient on admission:

TO: Pacient privezený RZP, ... pri vedom, ... zvracal, ... Bolí ho hlava, pravé rameno ...

There are many other standard abbreviations used in this kind of report, but I don't remember seeing TO before.

Thank you. I've definitely seen 'current illness' (in my experience, several items down, after 'personal history', 'family history' etc.). In this case, it comes before all those other items. Could be 'status on admission', I suppose.

Asker: Thanks very much. Of course that's it! - and I think 'current' here means 'at the time of admission'. I'd also suggest 'medical condition on admission'. (I see that 'status praesens' came up as a Cz-En question some years ago).
