Medical - Medical: Pharmaceuticals
Russian term or phrase: ДОПОЛНИТЕЛЬНОЕ СОГЛАСИЕ
In this case it is a caregiver's consent (supplemental to an ICF). Does it have an official name in English? (additional, supplemental, supplementary. All I have found is Caregiver's Consent Form)

In Russian: ДОПОЛНИТЕЛЬНОЕ СОГЛАСИЕ ЛИЦА, ОСУЩЕСТВЛЯЮЩЕГО УХОД ЗА ПАЦИЕНТОМ

Thank you again.

neutral  The Misha: Except we don't call it that. We call it a consent form, whichever one of them it might be. This phrase alone screams non-native, I wouldn't need to know anything else.

Legal Guardian Consent Form

Explanation:
Taken from listed web reference, the definition of a caretaker: " 'Caretaker' means a person... who serves as a legally appointed guardian, limited guardian, or conservator."

I think that if you don't want to use the term "caretaker," then "legal guardian" would be the next best choice. This is also the legal title that my relatives were referred to as when caring for our elderly family members (i.e., in the state of Oregon).

A legal guardian can be both the caretaker and the individual's representative, such as in the case of adult care. It can be both, or one or the other. It just depends on the laws of the state. Here in Oregon, the caregiver IS the legal guardian in cases such as with Alzheimer's patients that can't make decisions for themselves. The caregiver can both take care of them, and manage their affairs for them.

Notes to answerer
Asker: Thank you, Angelina.

Asker: Thank you, Angelina. That wouldn't fit here since there is nothing legal about the situation other than the caretaker's consent to participation in the study.

  -> Thank you.

disagree  Natalie: Legal guardian - это законный опекун, а не лицо, осуществляющее уход за пациентом. Для ухода за пациентом не обязательно иметь законным образом оформленные права, можно быть просто родственником, к примеру.

  -> Yes, but here that is what a legal guardian does in the case of adult care. They are both the caregiver and the representative.
