Имущество, вносимое членами или ассоциированными членами кооператива в счет их пая, вносится ими на основе акта приемки-передачи имущества (далее - Акт) после государственной регистрации кооператива. Акт должен содержать опись вносимого имущества, его денежную оценку, дату, на которую имущество вносится в кооператив, подпись члена кооператива, который вносит указанное имущество, подпись главы кооператива и печать кооператива.

I almost agreed with Jack but Alexander's comment made me think twice:-) I guess this would be more appropriate in this context.

I double-checked for Report, lots of references, at least for the U.S. Maybe in UK deed would be more common. But not certificate, the context points at otchet-protocol type of document. IMHO
