Fluoroscopy showed the main body of the bullet and metallic fragments projecting in the left anterior chest wall, in the vicinity of the coronary sinus and the aortic valve; normal epicardial coronary arteries were seen (Figs. 2 and 3). Aortography revealed moderate aortic insufficiency and a sinus of Valsalva aneurysm that had ruptured into the right atrium (RA). Right heart catheterization showed a step-up in the oxygen saturation from the inferior vena cava and pulmonary artery (inferior vena cava, 45%; superior vena cava, 38.2%; RA, 71.9%; pulmonary artery, 70%).
