At first I thought about "to make something a reality", but since the context is "psychology", the author might rather be talking about someone (a patient for ex.) gathering his/her thoughts and giving concrete expression to it?

so I would go for > "gathering one thoughts and giving concrete expression to it".

or even just > "gathering ones thoughts and expressing it"

My English is not perfect, hope you got the idea!
