I think it would almost certainly be OK to translate "constatazione amichevole d’incidente e/o denuncia di sinistro" simply as "accident report".

Are the guidelines being translated into UK English because the cars will be driven in the UK? If so, it would be weird to reference a document that doesn't exist in the UK. On the other hand, if the cars are being driven in Italy, you would probably be better off just quoting the name of the document and adding an explanatory note.

Hello Phil. I'm translating a set of guidelines for the assignment of a company car. Specifically, this is the clause relating to what to do in case of an accident. They want UK English, which is why asked about the UK. Regarding "amicable", I agree that it's important, but am unsure how to render it. I was thinking of "joint accident report" or something like that...

You might have to use something long-winded like "report concerning an accident whose circumstances are undisputed". I'm not sure "amicable accident report" sounds quite right.

I would have thought "amichevole" is quite important, and I wouldn't normally omit it, but you could possibly get away with doing so depending on the context. Are you translating the form itself, or something else containing a reference to it?
