It is the person who notifies others about a defective part. For 'Entdecker' I came up with 'the person who discovered the defect', but I'm struggling with 'Melder'. Since it is a person it can't be a detector or warning device.

Das fehlerhafte Teil wird mit diesem Formular gekennzeichnet und ist damit für die weitere Verwendung gesperrt (Formular dient als Sperrzettel).
Es ist verboten gesperrte Teile weiterhin zu benutzen.
