Last line: "Pat vereinbart neuen Termin zur KVS nach Periode".

I am not sure what "KVS" means here. Clinic where examination was done is in Bavaria (I say that because "KVS" can mean "Krankenversicherung Sachsen" but I doubt that applies here).
