I usually translate this simply as nurse. There are "health visitors" in the UK, but their remit is very different from that of a Gesundheitspfleger/in. From what I can make out about the responsibilities of Gesundheitspfleger, I'd say they are very similar to those of a nurse in the UK. I don't know about the US, though.
