Hello everybody! This is my contribution to the discussion:
a) I found a few indicators supporting the client's distinction, e.g. „Als ‚Spill Over’ (auch Ausstrahlungseffekt) bezeichnet man die Nachverordnung von Präparaten durch Allgemeinärzte, auf die der Patient primär bei einem Facharzt oder in einer Klinik eingestellt wurde.“
b) "Refill" (wiederholte Abgabe eines verschreibungspflichtigen Arzneimittels auf dasselbe Rezept) is a concept that we don't seem to have in Germany. (http://yalehealth.yale.edu/faq/what-s-difference-between-pre...
c) Definition, I found: Follow up prescription: The prescription that follows the choice of the original drug

Based on the above: If no US expert is available, I would go with Anne: Folgerezept (= Folgeverordnung): "repeat prescription", Nachverordnung: "follow-up prescription"

Being a die-hard (or die slowly) advocate of homeopathy, I have never had the honor of refilling a prescription. My Granny, however, was a guinea pig for the NIH and had constant refills and adjusted medications. This is what leads me to believe that Nachverordnung is an adjusted medication and Folgerezept a simple refill.

I am afraid, a reasonable answer can only be posted from someone who is thoroughly familiar with the trifles of the American health system, and is able to reproduce the distinction which David's client makes for the German terms, in English. Are there such two types of prescriptions at all in the U.S.?

You may also use "refills ordered by the same physician" and "refills ordered by another physician" (or a similar translation, e.g. doctor, clinic etc.). If these terms do not repeatedly appear in your file, this would be a safer solution.

Curious, "Nachuntersuchung" would be IMO subsequent, post procedure or follow-up examination. correct? So are the "Nachverordnungen" prescribed in result of such examination?

I don't see a (defined) difference between a Folgerezept and a Nachverordnung even in German. The source text itself does not seem to make a difference ("Folgerezept...selbst verordnet (nur Nachverordnungen)"), but the client may certainly apply different definitions to the two terms. In that case, "repeat prescription" and "follow-up prescription" may work just fine in order to have two different English terms at hand as well.

A Folgerezept, as far I know (AE), is a refill of a current prescription, whereas a Nachverordnung is a new or adjusted prescription of a different/adjusted medication.
