It then lists the average as "Der Mittelwert (Begründungsschwelle) ist bei ärztlichen Leistungen das 2,3fache". Is there an equivalent English term for "Begründungsschwelle"?

The rates they're allowed to charge are governed by law. As Celia explains in the discussion notes, there's a basic rate which can be marked up depending on the complexity of the case and other factors. But if they charge more than 2.3 times this rate, they have to explain why.

"1982 wurde der vorher vom Einfachen bis Sechsfachen reichende Gebührenrahmen auf das maximal 3,5fache des Gebührensatzes reduziert. Innerhalb dieses Gebührenrahmens wurde eine Regelspanne bis zum 2,3fachen des Gebührensatzes eingeführt - **überschreitet der Arzt das 2,3fache (Begründungsschwelle), so muss er dies durch die Besonderheiten des Einzelfalls begründen können***."
