The above meaning appears to be likely here, although I'm a bit confused regarding your mentioning a "label at the top" in conjunction with assuming the report to be "signed at the bottom".

A-ha, then it's perhaps to be initialled/signed off for final approval behind the word/label "endvidiert", regardless of any other signature.

Asker: Thanks - I only meant signed at the bottom, where a physician's surgeon usually appears, signifying he certifies the information in the report; the word "endvidiert" in this case is simply sitting at the very beginning of the report, on its own
