This is in a box with the patient's details in a report on the patient's biopsy following hysterectomy and unilateral salpingo-ophorectomy due to cancer. Also in the box are the record number, patient's name, DOB and sex. "Nom de J.F." unfortunately is not followed by anything as I assume it has been redacted like the rest of the patient's information so can't be used to work out the meaning.
