A acquis la nationalité française par déclaration de nationalité souscrite le xx mars xxxx devant le juge d'instance de xx au titre de l'article 21-11 alinéa 1 du code civil enregistrée le xx mai xxxx sous le n°xxx

You shouldn't repeat "nationality" as the French has done, and I would just say "declaration". But if you're really intent on putting something before it, you could say "citizenship".
