I've just found site that gives a detailed definition of 'vrij assistent' : http://195.234.184.64/web-Ned/nl/a034/a034029n.htm.
I seems to be not unlike and 'article clerk' in the law, i.e. a qualified doctor working for/understudying an experienced specialist.
