I believe the dose they are talking about is for each individual who resides in Hamedan. It seems impossible to measure the radiation for each city.

Please see the link below, it explains what the effective dose means. It is measured for the people who live in a region not the region itself. The word ساکنان is omitted when they talk about each city.
