Could you please explain me what the difference is?

I think that anatomic design is available in right OR left configuration and neutral design is suitable for left and right leg as well. Straight design is straight(?).

CONTEXT

Total Hip Prosthesis
Femoral Stems
A wide range of femoral stems is available in straight, neutral, or anatomic designs; collared or collarless configurations; with and without coatings; and with preformed cement mantle manufactured from one component of Surgical Simplex® P Bone Cement with the addition of barium sulfate (Ba SO4). The materials used to manufacture these stems are certified to ASTM Standards:

my understanding below
Explanation:
As far as I understand, they all refer to the shape of material utilized in the Femoral Stems in the hip prosthesis (artificial hip)

Anatomic -- conforms or fits the actual shape of the femoral stem of the patient
Neutral -- a mix between straight and anatomic, somewhere in-between
Straight --- of a straight shape
