The question is - what is ON/OFF in this context? The part of the product is ON the die cut card and the other part in OFF?

"syringe, 1 (one) on card" means that they are packaged individually on a card with the blister wrap over them. and some of them are packaged so that the handle is in a top plastic pocket but the body of the syringe is attached to a card backing. the link below shows different packaging of syringes. My understanding is that Medical syringes for made up doses are 1 (one) on a card, medical with premixed doses have syringes are packed with 5 + in a plastic sealed card and cosmetic syringes are packed with with almost no card so they can be retail displayed.
