Without knowing exactly what kind of leads (connecting wires) are involved here, it seems to me that if the sheath tip gets caught behind a bend in one of the leads it (also) does not move forward.
Whereas the first 3 sentences refer to the phenomenology of the sheath not advancing, the last sentence intends to distinguish between tissue obstruction and other causes.
