Dear Irshad Sb, IMO,'Ijazat Nama' gives the meaning of 'Permit' in English and 'Consent Form' is 'رضامندی فارم', like we translate 'Informed Consent Form' as 'باخبر رضامندی فارم' in the translation of Medical Studies. I don't think 'Consent Form' and 'License' are one and the same thing.
Dear Ikram Sb, I have also seen 'پروانگی' and 'پروانہ' being used as the translation of 'license' but more often 'پروانہ', which is still in use in some old Court Papers and Government documents. But unfortunately, this word is also going towards extinction as a meaning of 'license'. like so many other Urdu words.
Having said that, I agree with both of you that it is better to use the transliteration of 'license' as it will be more comprehensible to an average reader. You will appreciate that our utmost effort, as a translator, should be to make the reader understand what he/she is reading, be it translation. transliteration or the English word itself.
