It's a disposition in a contract of medical insurance effected by a company for its employees. I thought of "fechas de altas y bajas", assuming that "entry into risk" meant that the Insurer started being covered by the insurance Plan.

Context: "As soon as the Contract enters into effect, XXX and YYY shall supply information indicating the names of the beneficiaries of coverage, including the dates of entry into and withdrawal from risk."
